Materials and Methods In  this  randomized clinical trial  that was performed  in Ghaem Hospital  during  a 2 years  period, we compared  routine and intermittent protocols .In routine protocol, 4-6 weeks after surgery, levothyroxin discontinued and liothyronin prescribed for two weeks, then liothyronin discontinued for 2 weeks and TSH measured. In intermittent protocol, levothyroxin prescribed every other day and after 30 days discontinued and TSH was measured. Results Fifty patients were female and 10 cases were male. Fifty-three patients had papillary and 7 cases had follicular carcinoma. Twenty-two patients performed intermittent and 38 patients were on routine protocols. There was no significant difference between two groups in pulse rate, skin dryness, constipation, paresthesia, puffiness and change in weight. There was no significant difference in TSH level between two groups at the end of two protocols. (p=0.08). We used chi-square, independent and paired t tests for analysis of data and p-value less than 0.05 was considered as significant. Conclusion In this study there was no significant difference in hypothyroid manifestations in intermittent and routine protocols. But exposure to high levels of TSH and its outcomes are less in intermittent protocol.}, keywords = {Differentiated,Hypothyroidism,Scan,thyroid carcinoma}, title_fa = {تاثیر روشهای متداول و متناوب، در آماده سازی برای اسکن تمام بدن با ید 131، بر افزایش سطح TSH و تظاهرات بالینی کم کاری تیروئید در بیماران مبتلا به سرطان تمایز یافته تیروئید}, abstract_fa = {مقدمه  دربیماران مبتلا به سرطان تمایز یافته تیروئید،  که برای اسکن تمام بدن با ید 131 آماده می شوند، رسیدن به سطح مناسب TSH با  بروزکمترین تظاهرات بالینی کم کاری تیروئید مطلوب است. روش کار در این مطالعه کارآزمایی بالینی تصادفی شده که طی مدت 2 سال در بیمارستان قائم (عج) انجام شد دو روش متداول و متناوب مورد مقایسه قرار گرفت. در پروتکل متداول، 4-6 هفته پس از عمل جراحی تیروئید، لوتیروکسین  قطع و لیوتیرونین به مدت دو هفته تجویز می شد، سپس لیوتیرونین به مدت 2 هفته قطع و TSH اندازه گیری می گردید. در پروتکل متناوب، لوتیروکسین روز درمیان، تجویز و پس از 30 روز قطع و TSH اندازه گیری می شد.اطلاعات با نرم افزار SPSS و آزمون های کای مربع، تی مستقل و تی زوجی تجزیه و تحلیل شد. نتایج پنجاه بیمار زن و 10 نفر مرد بودند. پنجاه و سه نفر مبتلا به سرطان پاپیلری و 7 نفر مبتلا به سرطان فولیکولار بودند. 22 نفر تحت پروتکل متناوب و 38 نفر تحت پروتکل  متداول قرار گرفتند.  تعداد ضربان نبض،  خشکی پوست، یبوست، پارستزی، پف آلودگی، تغییرات وزن، در دو گروه تفاوت معنی دار نداشت. تفاوت معنی داری در سطح TSH بین دو گروه در پایان  دو پروتکل نیز دیده نشد (08/0p=). نتیجه گیری در این مطالعه تفاوت معنی داری دریافته های بالینی هیپوتیروئیدی در روشهای متناوب و متداول  مشاهده نشد،  ولی تماس با سطح بالای TSH و عوارض ناشی از آن در روش متناوب کمتر بود. }, keywords_fa = {اسکن,تمایز یافته,سرطان تیروئید,کم کاری تیروئید}, url = {https://mjms.mums.ac.ir/article_141.html}, eprint = {https://mjms.mums.ac.ir/article_141_ebde656532913fd4557297628af89965.pdf} }
