Thus, it seems that detection of certain factors such as TLR molecules could be helpful for diagnosis of sepsis and guiding appropriate antibiotics. The aim of this study was comparison of TLR2 and TLR4 gene expression in negative culture bacterial septic patients with healthy people. Materials and Methods This case control study was performed on 30 negative culture bacterial septic patients and 30 healthy people, in 2012. The data were analyzed by SPSS soft ware and cutoff ratios were calculated by R soft ware. Results The levels of TLR2 and TLR4 genes expression in negative culture and healthy control did not have any significant difference (p value TLR2:0.52) and (p value TLR4:0.099). The TLR4 gene expression in patients who had gram positive cocci sample smear but negative blood cultures in comparison to healthy control was significantly higher (p value: 0.004).  Meanwhile, regarding TLR2 there was no difference (p value: 0.0928). Conclusion TLR4 gene expression in patients with SOFA SCORE 0-3 in comparison to healthy controls had significant difference (with cut-off ratio of 9.54 and sensitivity and specificity of 67% and 71%). The same was true for negative blood culture bacterial sepsis in comparison to healthy controls, with cut-off ratio of 46.19 and sensitivity and specificity of 87% and 78%, respectively. So, evaluation of TLR4 gene expression may be useful to consider anti staphylococcus antibiotic therapy for patients with negative culture bacterial sepsis.}, keywords = {SOFA score,Sepsis,TLR}, title_fa = {بررسی مقایسه ای میزان بیان ژنهایTLR-2 وTLR-4 خون در دو گروه بیماران با سپسیس باکتریال باکشت خون منفی و افراد سالم}, abstract_fa = {مقدمه تشخیص به هنگام سپسیس باکتریال از اهمیت بسزائی برخوردار است، که نظر به موارد نتایج منفی کشت، هدفی دشوار به نظر می رسد. اما بررسی عواملی از جمله مولکولهای بیان کننده TLR ممکن است تا حدی کمک کننده باشد. هدف از مطالعه حاضر بررسی بیان ژنهای TLR2وTLR4 در سلولهای تک هسته ای خون بیماران سپسیس باکتریال با کشت خون منفی در مقایسه با جمعیت سالم جهت افتراق سپسیس باکتریال کشت منفی گرم مثبت از گرم منفی می باشد. روش کار  مطالعه در سال 1391 به صورت مورد شاهدی در دو گروه 30 نفری از افراد سالم و بیماران سپسیس باکتریال کشت منفی بستری در بیمارستان امام رضا (ع) انجام شد. نتایج با نرم افزار  SPSSتجزیه و تحلیل شد و میزان کات آف بیان ژنهای مذکور نیز با نرم افزار R محاسبه گردید. نتایج در مقایسه بیان TLR2 و TLR4بیماران کشت منفی با افراد سالم ارتباط معنی دار وجود نداشت  (52/0p=TLR2:)  و (099/0p=TLR4:) در مقایسه بیان TLR2 بیماران کشت منفی با اسمیر کوکسی گرم مثبت غیر شعله شمعی و غیر انتروکوک ارتباط معنی دار وجود نداشت (0928/0p=) ولی در بیان TLR4 ارتباط معنی دار مشاهده شد (004/0p=). نتیجه‌گیری  بیان TLR4در بیماران سپسیس باکتریال باSOFA SCORE 0-3 در مقایسه با افراد سالم با کات آف 54/9 و حساسیت 76% و ویژگی 71% تفاوت داشت، و نیز در سپسیس باکتریال کشت منفی احتمالا استافیلوکوکی با کات آف 19/46 با حساسیت 87%و ویژگی78% تفاوت داشت. بنابراین شاید بتوان از بررسی بیان TLR4 در تصمیم گیری جهت شروع آنتی بیوتیک ضد استافیلوکوک در سپسیس باکتریال کشت منفی استفاده نمود.}, keywords_fa = {سپسیس,TLR,SOFA score}, url = {https://mjms.mums.ac.ir/article_1953.html}, eprint = {https://mjms.mums.ac.ir/article_1953_99cc69a1d190c4151fb00d950db125af.pdf} }
