By starting analgesia with spinal anesthesia suddenly reducing catecholamines and cortisol concentration causes reduction in serum level of blood sugar. Materials & Methods: Following taking history of mothers and obtaining satisfaction, monitoring and hydration (5ml / kg ringer solution), the mother's blood sugar level with glucometer controlled. Spinal anesthesia done with needle Quinke No. 24 by 3cc of bupivacaine 0.5% in sitting position and then glucose was measured 15 min after spinal anesthesia and in recovery room. Results: Ninety pregnant had a mean age of 29/93±5/9 years old with an average weight of 74/04±8/9 Kg , and the mean gestational age of 39/0±0.74 weeks. Mean changes in blood glucose during the first 15 minutes and the mean change between before spinal and in recovery room in the three groups that have different NPO time was not significant. (P 0.713 and 0.87). Average glucose before anesthesia 90.74±19 and after it 88.42±18.9 were not significantly different (P=0.103), but difference between mean blood glucose before-anesthetic and in recovery room 96.16 ± 21 was significant (P=0.001). Conclusion: In comparison between blood sugar before  anesthesia and in recovery room , significant increase of blood sugar in recovery existed not in hyperglycemic range. Therefore, during spinal anesthesia with bupivacaine, monitoring of blood sugar was not necessary.}, keywords = {Spinal anesthesia,Cesarean section,Bupivacaine,Blood sugar}, title_fa = {بررسی تغییرات قند خون بعد از بی حسی موضعی توسط بوپیواکائین در عمل جراحی سزارین}, abstract_fa = {مقدمه: بی حسی موضعی روش نوینی است که در مقایسه با بیهوشی عمومی خطرات کمتری دارد. پاسخ های متابولیک به استرس جراحی در کنار غلظتهای سرمی افزایش یافته ای از هورمونهای استرس در زنان باردار باعث افزایش گلوکز سرم در بیمار می گردد. با آغاز سریع آنالژزی به روش موضعی کاهش ناگهانی در غلظت کاتکولامینها و کورتیزول رخ می دهد که به نوبه خود باعث کاهش سطح سرمی گلوکز می شود. روش کار:. این مطالعه کارآزمایی بالینی در بیمارستان قائم (عج) مشهد در سال 1391 بر 90 بیمار انجام شد. پس از شرح حال گیری مادران و اخذ رضایت، مانیتورینگ و هیدراته نمودن مادر(ml/kg 5 سرم رینگر)، میزان قند خون مادر با گلوکومتر کنترل شد. بی حسی موضعی با سوزن Quinke شماره ی 24 با استفاده از  3 سی سی بوپیواکائین 5/0% در وضعیت نشسته انجام شد، اندازه گیری قند 15 دقیقه پس از انجام بیحسی موضعی و سپس در ریکاوری انجام شد. اطلاعات با روش های آمار توصیفی، تجزیه و تحلیل شد و برای مقایسه از آزمون Repeated Measure استفاده شد. نتایج: نود مادر باردار شرکت کننده دارای میانگین سن بیماران 9/5±93/29 با میانگین وزن9/8±04/74 و میانگین سن بارداری 74/0±39 هفته بودند. میانگین تغییرات قند خون طی 15 دقیقه ابتدایی و نیز میانگین این تغییر بین قند خون قبل موضعی و ریکاوری در سه گروه با زمان ناشتایی متفاوت، اختلاف معنی داری نداشت. (713/0 و 87/0=p) میانگین قند بیماران قبل از بی حسی 19±74/90 و پس از بی حسی 9/18±42/88 بود که تفاوت معنی داری نداشته است. (103/0=p) ولی میانگین قند خون بیماران قبل از بی حسی و میانگین قند خونشان در ریکاوری 21±16/96 تفاوت معنی داری داشته است.( 001/0p=). نتیجه‌گیری: در مقایسه بین قند خون قبل از بی حسی با قند خون زمان ریکاوری افزایش معنی دار قند خون در ریکاوری وجود دارد که البته در هیچ موردی این افزایش قند در محدوده هیپرگلیسمی نبود بنابراین در طی بی حسی موضعی با بوپیواکائین لزومی به مانیتورینگ معمول قند خون نمی باشد.}, keywords_fa = {بی حسی موضعی,عمل جراحی سزارین,بوپیواکائین,قندخون}, url = {https://mjms.mums.ac.ir/article_2425.html}, eprint = {https://mjms.mums.ac.ir/article_2425_ee45fd7c1efc324276ad13e3098f20d9.pdf} }
