The aim of risk management is to reduce the risks concerning patients and increase work safety for staffs. In this study risk management was evaluated in different steps in Imam Reza emergency department (ED). Methods: In this action research study, first we performed three focus group discussions (FGD) sessions including FGD session with ED nurses, a session with emergency medicine residents and a session with the laboratory, drug store and radiology staffs. Using these sessions, a questionnaire was design to clarify risks and to priority processing. To evaluate and define risk level in ED we used experts’ panel. For each risk, risk severity, its incidence and risk level was calculated. According to the risk level, their priority was processed and intolerable risks were defined for planning an intervention. Results: Based on expert panel, 11 subgroups with 415 risks were gathered. Among these, 3 risks including high volume of attendants, staff shortage and high patients load were recorded as intolerable risks in risk level 1, 2 and 3. The intervention was designed to reduce the attendants by giving 2 attendant cards for each patient. Conclusion: In this study we observed that intolerable risks are high volume of attendants, staff shortage and high patients load and the intervention program was designed to reduce the risks.}, keywords = {Emergency Department,Identifying Risks,management,Work Safety}, title_fa = {بررسی عوامل و خطرات موجود در اورژانس بیمارستان امام رضا (ع) تبریز}, abstract_fa = {مقدمه  مدیریت خطر یعنی پیشگیری از بروز خطر و اداره آن پس از به وجود آمدن. مدیریت خطر شامل مراحل تشخیص، ارزیابی، آنالیز، کنترل، کاهش و پیشگیری از خطر است. هدف مدیریت خطر کاهش خطرات متوجه بیماران و افزایش ایمنی کار برای کارکنان است. مطالعه حاضر با هدف تشخیص، ارزیابی و اولویت بندی خطر در اورژانس بیمارستان امام رضا (ع) شهر تبریز انجام شده است. روش کار یک مطالعه کیفی از سال 1389-1390 در اورژانس بیمارستان امام رضا تبریز با استفاده از 3 جلسه بحث گروهی متمرکز با شرکت 26 نفر از واحدهای مختلف اورژانس برگزار شد. خطرات شناسایی شده و به روش گروهی اسم دسته بندی شدند. با استفاده از اطلاعات به دست آمده از این جلسات پرسشنامه ای جهت تعیین و اولویت بندی خطرات طراحی گردید. ارزیابی و تعیین سطح خطرات شناسایی شده در اورژانس، توسط پانل خبرگان انجام شد. به طوری که شدت و احتمال وقوع هر خطر تعیین و سطح خطر محاسبه گردید. همچنین بر اساس سطح خطر، خطرات اولویت بندی شدند. نتایج در پایان جلسات FGD و بر اساس جمع بندی انجام شده، حدود 536 خطر در اورژانس بیمارستان امام رضا (ع) شناسایی گردید و بعد از نظرات پانل خبرگان 415 خطر در 11 گروه دسته بندی شدند. از موارد فوق، سه عامل تعداد زیاد همراهان، کمبود پرسنل و کادر کمکی و حجم بالای بیماران به عنوان سه خطر اصلی تعیین شدند. نتیجه‌گیری خطرات از دیدگاه صاحبان فرایند و کارکنان اورژانس در واحدهای مختلف شناسایی و دسته بندی شدند که این کار فرصت مناسبی جهت مداخله و کاهش خطرات فراهم ساخت.  }, keywords_fa = {بخش اورژانس,شناسایی مخاطرات,مدیریت,ایمنی در کار}, url = {https://mjms.mums.ac.ir/article_4998.html}, eprint = {https://mjms.mums.ac.ir/article_4998_8214c9d56e33b29c2acac1576c19d764.pdf} }
