Recent surveys have shown that the use of non- steroid anti-inflammatory drugs before ERCP could reduce risk of pancreatitis after ERCP. This study aimed to evaluate the effect of diclofenac suppository in reducing bleeding and pancreatitis after ERCP in patients with common bile duct stones. Methods: This double-blind randomized clinical trial study was performed on 160 patients with bile duct stones that were needed to ERCP during the period January-September 2013. Patients were randomly divided into two groups of 80 patients treated with diclofenac suppository (case group) and Acetaminophen suppository (control group). The incidence of pancreatitis and bleeding after ERCP were evaluated based on clinical signs and laboratory findings. Data analysis was performed by SPSS statistical software and statistical tests of Kolmogorov-Smirnov, chi square and Fisher's exact test, independent student's, and Mann–Whitney U test. A P-value of less than .05 was considered significant.  Results: The mean age of patients was 61.24 ± 17.00 years and female to male ratio was 89 to 71 that difference between two groups were not statistically significant. In patients taking diclofenac suppository and acetaminophen 9 (11.3%) and 11 (13.8%) cases had mild pancreatitis, respectively. In the control group 2 cases (5.2%) and in case group any case had severe pancreatitis. Twenty-one patients (26.3%) receiving acetaminophen and 28 (5/35%) receiving diclofenac suppository had bleeding while cutting sphincter (Fisher's exact test=1.54; p-value=0.488). Conclusions: The results of this study showed that Diclofenac administered after ERCP had no effect on the incidence of pancreatitis and bleeding complications.}, keywords = {Acetaminophen,Diclofenac,ERCP,Pancreatitis,Bleeding,perforation}, title_fa = {تاثیر شیاف دیکلوفناک در بروز پانکراتیت و خونریزی بعد از ERCP در بیماران مبتلا به سنگ مجاری صفراوی}, abstract_fa = {مقدمه کلانژیوگرافی آندوسکوپیک عقب‌گرد (ERCP) به عنوان یک روش تشخیصی و درمانی مورد استفاده در بیماریهای مجاری صفراوی و پانکراس دارای عوارضی متعددی از جمله پانکراتیت، خونریزی، سوراخ شدگی و عوارض قلبی عروقی می باشد. در بررسی های اخیر نشان داده شد که استفاده از ضدالتهاب های غیر استیروئیدی قبل از ERCP  احتمال پانکراتیت بعد از ERCP را کاهش می دهد. مطالعه حاضر با هدف ارزیابی تاثیر استفاده از شیاف دیکلوفناک در کاهش عارضه خونریزی و پانکراتیت بعد از ERCP در بیماران مبتلا به سنگ مجاری صفراوی انجام شد.  روش کار  این مطالعه کارآزمایی بالینی تصادفی دو سویه کور در فاصله بهمن 1391 تا مهر 1392 بر روی  160 بیمار مبتلا به سنگ مجارای صفراوی که نیاز به ERCP داشتند انجام شد. بیماران به صورت تصادفی به دو گروه 80 نفره تحت درمان شیاف دیکلوفناک و تحت درمان شیاف استامینوفن تقسیم شدند. میزان بروز پانکراتیت و خونریزی بعد از ERCP بر اساس علائم بالینی و یافته های آزمایشگاهی ارزیابی شد. تجزیه و تحلیل داده ها با استفاده از نرم افزار آماری SPSS  و آزمون های آماری کولموگروف-اسمیرنوف، کای دو و آزمون دقیق فیشر، آزمون استیودنت  و من ویتنی انجام شد. میزان p-value کمتر از 05/0 معنی دار در نظر گرفته شد. نتایج میانگین سن بیماران 17.00±61.24 سال و نسبت زن به مرد 89 به 71 بود که از نظر سن و جنس در دو گروه شاهد و مورد تفاوت معنادار آماری نداشتند. در بیماران دریافت کننده شیاف دیکلوفناک  9 مورد (3/11 %) و در بیماران تحت درمان شیاف استامینوفن 11 مورد (8/13 %) پانکراتیت خفیف داشتند. در گروه شاهد 2 مورد  (5/2 %) و در گروه مورد هیچ موردی از پانکراتیت شدید ملاحظه نشد. بیست و یک بیمار (3/26 %) دریافت کننده شیاف استامینوفن و 28 بیمار (5/35 %) دریافت کننده شیاف دیکلوفناک حین برش اسفنکتر خونریزی نداشتند (488/0p-value=؛ 54/1Fisher's exact test =).  نتیجه گیری نتایج این مطالعه نشان داد تجویز دیکلوفناک تاثیری در کاهش بروز پانکراتیت و عارضه خونریزی بعد از ERCP  نداشت.}, keywords_fa = {استامینوفن,پانکراتیت,خونریزی,دیکلوفناک,ERCP}, url = {https://mjms.mums.ac.ir/article_6512.html}, eprint = {https://mjms.mums.ac.ir/article_6512_5da1c5dbccbff3b78a6359af50a34379.pdf} }
