بررسی ارتباط بین مورفه آ و سیتومگالوویروس در یک مطالعه مقطعی
مقدمه اسکلرودرمی موضعی، جزئی از طیف بیماریهای اسکلروتیک است که به طور اولیه پوست را تحت تاثیر قرار میدهد و شایعترین آنها مورفه آ میباشد. تا به حال علل و پاتوژنز مورفه آ به طور کامل شناخته نشده است اما فرض بر این است که یک فرایند چند عاملی باعث شروع آبشار التهابی و فیبروتیک میشود. عوامل محیطی احتمالی دخیل شامل بیماری لایم، تروما، پرتوتابی، داروها و عفونتهای ویروسی است که از میان عوامل عفونی، به سایتومگالوویروس اشاره شده است. هدف از این مطالعه بررسی ارتباط بین بیماری مورفه آ و سایتومگالوویروس است. روش کار در این مطالعه مقطعی دو گروهی روی ۴۱نمونه از بافت پارافینه بیماری مورفه آ تایید شده و۴۰ نمونه پوست سالم، تکثیرDNA سایتومگالوویروس به وسیله PCR ردیابی شد. دادهها با استفاده از نرم افزار spss (نسخه16) آنالیز و سطح معنیداری کمتر از 05/0 درنظرگرفته شد. نتایج از ۸۱ بیمار 13 نفر مذکر (1/16 %) و 68 نفر مونث (9/83 %) بودند. کمترین و بیشترین سن بیماران به ترتیب۱۱ و ۷۲ سال و میانگین سنی ۶/۴۱ سال بود. از بین 81 نمونه بتاگلوبین مثبت، CMV DNA تقریبا در تمام نمونههای مورفه آ و پوست سالم یافت شد و فقط یک مورد(4/2 %) از گروه مورد و 2 نمونه(5%) از گروه شاهد از نظر CMV DNA منفی شد (61/0=p). نتیجهگیری در مطالعه حاضر ارتباط آماری معنیداری بین تکثیر ژن سایتومگالوویروس با بیماری مورفه آ یافت نشد.

اپیدمیولوژی آنسفالیت کودکان در مشهد
مقدمه آنسفالیت از علل نسبتاً ناشایع بستری در بخشهای کودکان است که با عوارض ناتوانکننده درازمدت همراه است. هدف از مطالعه حاضر تعیین فروانی و علت شناسی آنسفالیت در بخش اعصاب کودکان بیمارستان قائم مشهد است. روش کار مطالعه حاضر یک تحقیق گذشتهنگر توصیفی درباره کودکانی است که با تشخیص آنسفالیت (در بازه زمانی فروردین 1387 تا شهریور 1393) از بخش نورولوژی کودکان بیمارستان قائم مشهد (تنها بخش نورولوژی کودکان در شرق کشور) مرخص شدهاند. اطلاعات از طریق مراجعه به پرونده بیماران به دست آمده است. تنها آزمایش ویروسشناسی به کار گرفتهشده در این مطالعه HSV PCR مایع نخاعی میباشد. برای تعیین میزان عوارض عصبی آنسفالیت ، با خانواده بیماران (در دسترس) تماس تلفنی گرفته شد و پرسشنامه مربوط به عوراض تکمیل شد. نتایج در مدت 6 سال و 5 ماه، 55 کودک با تشخیص آنسفالیت از بخش اعصاب کودکان مرخص شدند که میانگین سنی آنها 4 سال و 10 ماه بود و 51% پسر و 74% ساکن شهر بودند. کمترین فراوانی آنسفالیت در بهار مشاهده شد (14%). پلئوسیتوز مایع نخاع(3mm5> WBC) در 41% موارد وجود داشت. HSV PCR مایع نخاع در49% بیماران انجام شد که در 3 بیمار مثبت بود. علتهای آنسفالیت در این بررسی عبارتند از: ADEM* 16%، ویروس تبخال (HSV) 16% که موارد PCR مثبت 5% (مورد قطعی) و 11% با PCR منفی (مورد محتمل)، آنسفالیت آبلهمرغان3% و 63% ناشناخته.آسیکلویر در 20% بیماران در طول بستری تجویز نشد. شروع آسیکلویر در اولین بیمارستان در 77 % موارد بود. نتیجهگیری ADEM شایعترین علت آنسفالیت کودکان در خراسان است. متخصصین کودکان خراسان در اکثر موارد در مواجهه ابتدایی با کودک مبتلا به آنسفالیت، اسیکلوویر را شروع میکنند.

فراوانی سندروم متابولیک پس از پیوند کلیه و تأثیر آن بر عملکرد کلیه پیوندی
مقدمه سندروم متابولیک به عنوان مجموعه ریسک فاکتورهای قوی برای بیماری های قلبی، عروقی به شمار میرود و شامل فشار خون بالا، سطح سرمی تریگلیسرید افزایش یافته، سطح HDL کلسترول سرمی پایین، چاقی و اختلال تحمل گلوکز میباشد. باتوجه به اینکه بیماری های قلبی و عروقی شایعترین علت مورتالیتی در گیرندگان پیوند کلیه و کاهش عملکرد دراز مدت کلیه پیوندی میباشد هدف ما از این مطالعه بررسی فراوانی سندروم متابولیک در جمعیت گیرندگان پیوند کلیه و بررسی تاثیر آن بر عملکرد کلیه پیوندی میباشد. روش کار در این مطالعه 110 گیرنده پیوند کلیه بدون سابقه قبلی دیابت و با عملکرد پایدار کلیوی در اولین سال پس از پیوند به عنوان پایه و سپس در فواصل شش ماه و یک سال پس از بررسی اولیه از نظر فراوانی سندروم متابولیک بر اساس کرایتریاهای Adult Treatment Panel III مورد ارزیابی قرار گرفتند و سپس عملکرد کلیوی از طریق محاسبه فیلتراسیون گلومرولی و سطح سرمی Cr در دو گروه مبتلا و غیر مبتلا به سندروم متابولیک مقایسه شد. نتایج بررسی های انجام شده نشان داد که شیوع سندروم متابولیک در ایتدای مطالعه در 30% گیرندگان پیوند بود که در طول مدت پیگیری یک ساله به 44.5% افزایش پیدا کرد. همچنین در این مطالعه میان عملکرد کلیه پیوندی با توجه به فیلتراسیون گلومرولی و سطح سرومی Cr میان دو گروه مبتلا و غیر مبتلا به سندروم متابولیک اختلاف معنا داری وجود نداشت ( P > 0.05 ). نتیجهگیری یافته های ما در این مطالعه نشان می دهد سندروم متابولیک در جمعیت گیرنده پیوند کلیه شیوع بالایی دارد و با باتوجه به این که ریسک بیماری های قلبی عروقی را به طور قابل ملاحظه ای بالا می برد تشخیص و درمان به موقع بیماران مبتلا به سندروم متابولیک می تواند منجر به بهتر شدن پیش آگهی دراز مدت بیماران شود.

تجدید نظر در محدوده مرجع آمینوترانسفرازها
مقدمه آمینوترانسفرازها به عنوان مارکرهای سرمی ارزان و قابل دسترس که میتوانند آسیب به سلولهای کبدی را در مراحل اولیه نمایان کنند، آزمونهایی بسیار کاربردی هستند. چنان که علاوه بر استفاده این آنزیمها در تشخیص اولیه بیماریهای کبدی، ارتباط آنها با بیماریهای رو به گسترش، نظیر سندرم متابولیک و بیماریهای قلبی عروقی مد نظر قرار گرفته است. این شاخصها، نخستین بار حدود 60 سال قبل به عنوان شاخصی برای آسیبهای کبدی مطرح شدند ولی گسترش کاربرد آنها در دهه 1980 میلادی و برای تشخیص هپاتیت غیرA و غیرB (هپاتیت C) اتفاق افتاد. در اواخر قرن بیستم اصلاح محدوده مرجع آمینوترانسفرازها با استفاده از گروههای بزرگی از اهداکنندگان خون مد نظر قرار گرفت و نتایج متفاوتی حاصل شد. در سالهای اخیر نتایج این مطالعات به این دلیل که حاصل بررسی گروههای خاصی از جامعه بودند مورد تردید قرار گرفته است و پژوهشهای گستردهای بر روی جمعیتهای عمومی مناطق مختلف جهان آغاز شده است. با این حال در مطالعات مختلف نتایج متفاوتی بهدست آمده است و این مسئله لزوم انجام مطالعات بومی در نژادهای مختلف و بر پایه جمعیتها را بیان میکند. متاسفانه تا کنون در ایران علی رغم شیوع روز افزون بیماریهای قلبی عروقی و سندرم متابولیک، پژوهشی بر روی محدوده مرجع آمینوترانسفرازها که هم قابلیت تعمیم به سطح جامعه را داشته باشد و هم در آن عوامل مخدوشگر در سطح سرمی این آنزیمها مورد توجه واقع شده باشد، انجام نشده است. لذا انجام مطالعهای با این خصوصیات در مورد محدوده مرجع این شاخصها پیشنهاد میگردد.

بالیدگی زیستشناختی و فعالیت بدنی دختران نوجوان مازندران در سال 1396: نقش تعدیلکننده حمایت اجتماعی
مقدمه زمان بالیدگی زیستی بهعنوان عاملی بالقوه برای واریانس فعالیت بدنی نوجوانان شناخته شده است. از اینرو، پژوهش حاضر با هدف بررسی آثار اصلی و تعدیلکننده وضعیت بالیدگی زیستی و حمایت اجتماعی بر سطح فعالیت بدنی دختران نوجوان (بر اساس مدل زیست فرهنگی واریانس مرتبط با بالیدگی در فعالیت بدنی نوجوانان به عنوان چارچوبی مفهومی) انجام شد. روش کار 630 دانشآموز دختر دوره اول متوسطه استان مازندران در سال تحصیلی 96-95 بر اساس روش نمونهگیری خوشهای چند مرحلهای تصادفی انتخاب شدند. ابزارهای مورد استفاده جهت جمعآوری دادهها شامل تخمین درصد قامت بزرگسالی، پرسشنامههای حمایت اجتماعی و جمعیتشناختی و گامشمار بود. تحلیل دادهها با استفاده از نرمافزارهای SPSS23 و PLS2 در سطح معناداری05/0P≤ صورت گرفت. نتایج تحلیل دادهها نشان داد که رابطه معکوس معناداری (289/0 -) بین وضعیت بالیدگی زیستی و فعالیت بدنی دانشآموزان در سطح معنیداری 01/0 وجود دارد. نتایج مدلسازی معادلات ساختاری نشانگر برازش خوب مدل بود و نشان داد حمایت اجتماعی تعدیلکنندهای مهّم برای تغییر بالیدگی مرتبط با فعالیت بدنی است. نتیجهگیری حمایت اجتماعی ممکن است نقش منفی بالیدگی زودرس در سطح فعالیت بدنی را کاهش دهد. تفسیر والدین به تغییرات مرتبط با بلوغ و تشویق نوجوانان به قبول بلوغ بهعنوان رویدادی مثبت و طبیعی (نه یک مانع برای فعالیت بدنی) ممکن است نقش مهّمتری را در کاهش هرگونه آثار منفی بالیدگی زودرس بر فعالیت بدنی ایفا کند.

مقایسه پاسخ های حاد و مزمن فشار خون، PWV و ABI به دو پروتکل متفاوت تمرین تناوبی
مقدمه تحقیق حاضر به مقایسه پاسخ های حاد و مزمن فشار خون، سختی سرخرگی و بیماری عروق محیطی به 11 هفته تمرین تناوبی با دو شیوه متفاوت پرداخت. روش کار تعداد 31 نفر مبتلا به فشار خون بالا به شکل تصادفی به سه گروه کنترل (10n=)، SDHIIT (11n=) و LDHIIT (10n=) تقسیم شدند. تمرین گروه SDHIIT شامل 27 تناوب 30 ثانیهای با شدت 100-80 درصد VO2peak و فواصل بازیابی 30 ثانیهای با شدت 20-10 درصد VO2peak بود. گروه LDHIIT، 4 تناوب 4 دقیقهای با شدت 90-75 درصد VO2peak و 4 تناوب 4 دقیقهای بازیابی با شدت 30-15 درصد VO2peak را انجام دادند. پاسخ های فشار خون، PWV و ABI قبل وبعد از یک جلسه تمرین درابتدا و بعداز 11 هفته تمرین اندازهگیری شد. نتایج بعد از یک جلسه فعالیت ورزشی، تفاوت معنیداری در PWV دو گروه دیده نشد (05/0p≥)، در حالیکه فشارخون سیستول، دیاستول و ABI در دو گروه به طور معنیداری کاهش یافت (05/0p≤). پاسخ حاد این متغیرها بعد از 11 جلسه تمرین، کاهش معناداری در گروه LDHIIT در فشار خون سیستول (03/0p=) و متغیر ABI (01/0p<) در گروه SDHIIT نشان داد. PWV وفشار خون تغییر معناداری نداشتند. اثر مزمن دومدل تمرین نشان داد همهی متغیرها غیراز ABI در گروه LDHIIT تغییر معناداری داشتند. نتیجهگیری هر دو مدل تمرین در شکل حاد خود فشار خون و عوامل همودینامیک را بهبود بخشیدند. با این حال در طولانی مدت، SDHIIT در مقایسه با LDHIIT اثرگذاری نسبتاً بیشتری بر فشار خون و دیگر متغیرهای وابسته به آن نشان داد.

نقش باالقوه نورون زایی در درمان آلزایمر
مقدمه بیماری آلزایمر شایعترین شکل دمانس در افراد سالخورده است. ویژگیهای آسیبشناسی بافتی آن انباشت برون سلولی پروتئین آمیلوید بتا و هایپرفسفولاریزاسیون پروتئین تاو است. باور بر این است که این بیماری تحلیل برنده نورونی با اختلال عملکرد سیناپسی و مرگ نورونی پیاپی آغاز میشود. پژوهشگران اعتقاد دارند که نورون زایی در بزرگسالی مشاهده میشود؛ بنابراین جبران نورونهای از دست رفته میتواند یک رویکرد درمانی برای بهبود و کنترل آلزایمر باشد. رشته در حال توسعه سلولهای بنیادی، پتانسیل درمانی بزرگی را برای بیماریهای مزمن نورولوژیک مانند آلزایمر به خصوص اگر با رویکردهای درمانی چند هدفی همراه باشد پیشنهاد میدهد. نشان داده شده است که محیط غنی اثرات مثبتی را در عملکرد شناختی بیماران از طریق چند مکانیسم از جمله تنظیم نورون زایی دارد. نتیجه گیری پیشرفتهای صورت گرفته در نورون زایی، سلول بنیادی درمانی و محیط غنی در آلزایمر در این مقاله مرور شده اند و چالشهای احتمالی استفاده از این درمانها در آلزایمر بحث شده اند.

بررسی عوامل موثر بر کنسلیهای اعمال جراحی الکتیو در بیمارستان شهید عارفیان ارومیه
مقدمه تامین بهداشت و درمان یکى از نیازهاى اساسى و حیاتى هر جامعه بوده و از جمله عواملى است که نقصان آن یکى از نقاط ضعف دولتها تلقى شده و سبب نارضایتى وسیع اجتماعى مىشود. از طرفی با افزایش روز افزون هزینههای درمانی، یافتن راهکارهای عملی جهت کاهش هزینههای بیماران در این برهه از زمان امری ضروری بنظر میآید. روش کار این تحقیق بصورت کاربردی و براساس پنج فاز اصلی چرخه ششسیگما در بیمارستان شهید عارفیان ارومیه اجرا شده است. با برگزاری جلسات طوفان فکری از بین عوامل متعدد کاهش هزینه بیماران، کنسلی اعمال جراحی الکتیو مورد مطالعه قرار گرفته است. نتایج یافته ها نشان میدهد علل اصلی کنسلی اعمال جراحی که باعث افزایش هزینه های بیماران میگردد شامل عوامل پاراکلینیکی، عواملی بالینی، عوامل مربوط به جراح، عوامل مربوط به بیمار و عوامل سیستمی می باشند. با اجرای فازهای ششسیگما، سیگمای فعلی فرآیند که محاسبه گردید مقدار 3.1827 بدست آمد که برای یک واحد درمانی نامناسب میباشد. چرا که وجود بینظمی در این بخش در بسیاری از موارد جبران ناپذیر میباشد. نتیجهگیری در نهایت با برگزاری جلسات طوفان فکری با اعضای دخیل در پروژه راهکارهای عملی کاهش عوامل کنسلی پیشنهاد و دو فلوچارت اصلاحی بدین منظور ارائه گردید. پیش بینی میگردد با اجرای راهکارهای ارائه شده سیگمای فرآیند به 5/3 ارتقاء یابد.
