تأثیر توأم تمرین طناب زنی و مصرف مکمل زیره سبز بر شاخصهای تن سنجی، ترکیب بدن، متابولیک، آنتی اکسیدانی و التهابی در مردان مبتلا به اضافه وزن: یک کارآزمایی بالینی کنترل شده تصادفی
مقدمه امروزه اضافهوزن و اختلالات متابولیکی همراه آن به یکی از نگرانیهای مهم حوزه سلامتی تبدیلشده است. هدف از پژوهش حاضر بررسی تأثیر تمرین طناب زنی و مصرف مکمل زیره سبز بر شاخصهای تنسنجی، ترکیب بدن، متابولیک، آنتیاکسیدانی و التهابی در مردان مبتلا بهاضافه وزن بود. روش کار در این مطالعه کار آزمایی بالینی،40 مرد جوان دارای اضافهوزن انتخاب و تصادفی به چهار گروه کنترل، تمرین، مکمل و ترکیبی تقسیم شدند. برنامه گروههای تمرین شامل هشت هفته تمرین طناب زنی منتخب بود. گروه مکمل و ترکیبی، روزانه 2 عدد قرص 25 میلیگرمی زیره سبز مصرف میکردند. خونگیری قبل و بعد از هشت هفته به دنبال 12-10 ساعت ناشتایی شبانه انجام شد. نتایج نتایج نشان داد پس از 8 هفته بین مقادیر وزن بدن تمام گروهها با گروه کنترل اختلاف معناداری وجود دارد (05/0p≤). همچنین بین مقادیر چربی بدن، BMI، لپتین و CRP گروههای تمرین و ترکیبی اختلاف معناداری با گروه کنترل مشاهده شد. که این اختلاف در مقادیر TAC تنها در گروههای مکمل و ترکیبی بارز بود (05/0p≤). نتیجه گیری با توجه به یافتههای پژوهش حاضر، طناب زنی و مصرف زیره سبز با کاهش میزان لپتین و عوامل التهابی و افزایش ظرفیت آنتیاکسیدانی تام، نقش مهمی در تنظیم وزن بدن مردان دارای اضافهوزن ایفا میکند. اگرچه به نظر میرسد؛ تمرینات طناب زنی و زیره سبز هرکدام بهطور مستقل تأثیرات قابلتوجهی از خود نشان میدهند.

آیا یک فعالیت پیش رونده هوازی، اشتهای زنان چاق و مبتلا به اضافه وزن را کاهش میدهد؟
مقدمه با توجه به افزایش بیماریهای ناشی از چاقی، روشهای درمانی کنترل یا پیشگیری از چاقی و اضافه وزن، مورد توجه محققین میباشد. در این میان دستکاری هورمونهای اشتها به واسطه تمرینات ورزشی، به عنوان یک عامل کم مخاطره و نوپا در فرایند سلامتی مورد مطالعه قرار گرفته است. روش کار 13 زن سالم و مبتلا به اضافه وزن یا چاقی درجه یک با میانگین سنی 25 سال و میانگین توده بدن 29/31 کیلوگرم بر مترمربع در مطالعه شرکت کردند. جلسه آزمون شامل اجرای تست وامانده ساز بالک با سه مرحله خونگیری (قبل از فعالیت، نیم ساعت و دو ساعت پس از فعالیت) جهت سنجش گرلین اسیلدار (AG) و پپتیدYY (PYY 3-36)، همچنین هفت مرحله تکمیل پرسشنامه ارزیابی اشتها (VAS) بود. دادههای حاصل، توسط نرم افزار SPSS نسخه 20 و با استفاده از روش تحلیل واریانس مکرر و آزمون تعقیبی LSD بررسی شدند. نتایج مقادیر پس از فعالیت AG وPYY 3-36، نسبت به حالت پایه، تفاوت معناداری با هم نداشتند (05/0< p) ولی میزان احساس گرسنگی، احساس سیری و میل به خوردن غذا در هفت مرحله با هم تفاوت معناداری داشتند (05/0< p). نتیجه گیری به نظر میرسد تغییرات هورمونهای اشتها همیشه با تغییرات حس اشتها همراه نمیباشد و عوامل متعدد دیگری در پاسخ این هورمونها دخیل هستند.

بررسی عوارض ناشی از شیمی درمانی در بیماران مبتلا به سرطانهای گوارشی در یک بیمارستان آموزشی
مقدمه عوارض جانبی دارویی یکی از مهمترین مشکلات در بیماران بستری در بیمارستان میباشد که خصوصا در بیماران مبتلا به سرطان معمولتر و مشکلساز تر است. هدف از مطالعه حاضر ارزیابی عوارض جانبی ناشی از شیمی درمانی در بیماران مبتلا به سرطانهای معده و کولورکتال در یک بیمارستان آموزشی است. روش کار این مطالعهی توصیفی از مردادماه تا دی ماه 1395 روی بیماران دریافت کننده رژیم شیمی درمانی سرطان معده و کولورکتال در بخش شیمیدرمانی سرپایی بیمارستان امام رضا (ع) مشهد انجام شد. در طول دریافت شیمیدرمانی هرگونه عارضهی جانبی از لحاظ علت، شدت و قابل پیشگیری بودن ارزیابی شد. نتایج 54 بیمار مبتلا به سرطان معده و 67 بیمار مبتلا به سرطان کولورکتال وارد مطالعه شدند. درمجموع 431 عارضهی جانبی مشاهده شد (249 مورد سرطان معده و 182 مورد سرطان کولورکتال)که شایعترین عوارض به ترتیب گوارشی و عصبی بود. در بیماران مبتلا به سرطان معده، عارضه تهوع و استفراغ به طور معنادار در بیماران دریافت کننده رژیم اپیروبیسین+ اگزالیپلاتین+کپسیتابین و عارضه نوروپاتی، اسهال، درد شکمی، و ضعف عضلانی در دریافت کنندگان رژیم لوکوورین+ فلورواوراسیل+ اگزالیپلاتین شایعتر بود و در بیماران مبتلا به سرطان کولورکتال تنها عارضهی نوروپاتی در بیماران دریافت کنندهی رژیم لوکوورین+ فلورواوراسیل+ اگزالیپلاتین-4 شایعتر بود. نتیجه گیری بیماران تحت شیمیدرمانی با شدت بیشتری در معرض بروز عوارض جانبی هستند که بسیاری از این موارد قابل پیشگیری و مدیریت هستند و بنابراین پایش منظم عوارض و اقدام به موقع میتواند در جلوگیری و مدیریت آن یاریرسان باشد و هزینهها و عوارض ناشی از آن را کاهش دهد.

بررسی ارتباط E/E′ ratio با فشار پایان دیاستولی بطن چپ به روش اندازهگیری تهاجمی
مقدمه تعیین فشار انتهای دیاستولی بطن چپ[1] که با روش تهاجمی کاتتریزاسیون اندازه گیری میشود، اقدامی اساسی برای پیش بینی پروگنوز و درمان در بیماران نارسایی قلبی است. هدف این مطالعه تعیین میزان دقت ′E/E اکوکاردیوگرافیک در پیش بینی غیرتهاجمی LVEPD در بیماران با EF متفاوت میباشد. روش کار در این مطالعه در 60 بیمار فشار انتهای دیاستولی بطن چپ(LVEDP) در کاتتریزاسیون قلب اندازهگیری شد. همچنین میانگین نسبت′E/E (نسبت E میترال به میانگین E′ متوسط سپتال و لترال) با استفاده از اکوکاردیوگرافی داپلر به دست آمد. در پایان ارتباط بین میانگین نسبت′E/E و LVEDP مورد ارزیابی قرار گرفت. نتایج میانگین سن بیماران 27/11±17/57 سال بود و 37 نفر (7/61 %) از بیماران مرد بودند. میانگین ′E/E بیماران 17/4±06/11 سانتی متر در ثانیه و میانگین LVEDP بیماران 96/3±9/12 میلی متر جیوه بود. در بررسی ارتباط بین ′E/E و LVEDP با استفاده از آزمون Pearson عدد همبستگی 717/0 با 001/0 p< به دست آمد. در بررسی بر اساس مقادیر مختلف EF نیز قوی ترین ارتباط بین ′E/E با LVEDPدر گروه با EF کمتر از 35 و بیشتر از 55% و ضعیفترین ارتباط در گروه با EF بین 35 تا 44 وجود داشت. نتیجهگیری نتایج این مطالعه در مجموع نشان میدهد که ارزیابی فشارپرشدگی بطن چپ با استفاده از روش غیر تهاجمی محاسبهی نسبت ′E/E در اکوکاردیوگرافی از دقت و اعتبار بالایی برخوردار است و میتواند جایگزین روش های تهاجمی شود. دستهبندی ژنتیکی ایزولههای اشریشیاکلی جدا شده از عفونتهای دستگاه ادراری در استان چهارمحال و بختیاری
مقدمه اشریشیاکلیهای یوروپاتوژنیک (UPEC) یکی از شایعترین عوامل میکروبی ایجادکننده عفونتهای دستگاه ادراری میباشند که سروتیپهای مختلفی از این گروه سرمی که دارای تنوع ژنتیکی هستند در ایجاد این عفونتها دخالت دارند. مطالعه حاضر با هدف دسته بندی ژنتیکی ایزولههای یوروپاتوژن اشریشیاکلی جدا شده از عفونتهای دستگاه ادراری در استان چهارمحال و بختیاری انجام شد. روش کار تعداد 67 ایزوله UPEC که از بیماران بستری در بیمارستانهای سطح استان چهارمحال و بختیاری جدا شده بودند، انتخاب و به روش ERIC-PCR آزمایش شدند. نتایج ایزولههای مورد مطالعه دارای الگوی باندی از محدوده 110 تا 3100 جفت باز بودند که در آنالیز الگوی باندینگ حاصله با ضریب تشابه تطابق ساده در سطح تشابه 91 درصد در 15 زیرگروه قرار گرفتند. بجز قرابت 100 درصدی که در 4 مورد دیده شد، سایر ایزوله ها دارای قرابت ژنتیکی بین 4/44 تا 1/48% بودند. قرار گرفتن ایزولههای مورد مطالعه در چند زیر گروه نشان گر قدرت تمایزدهی قابل قبول تکنیک ERIC-PCR در ژنوتایپینگ اشریشیاکلی و وجود منابع مختلف آلودگی دستگاه ادراری با این پاتوژن میباشد. نتیجه گیری روشERIC-PCR، روشی ساده، سریع و کمهزینه جهت توصیف تنوع ژنتیکی سویههای مختلف اشریشیاکلی ازجمله سویههایUPEC میباشد.

تأثیر کوآنزیم کیوتن و تمرین ورزشی بر برخی شاخصهای همودینامیکی و کیفیت زندگی بیماران مبتلا به نارسایی قلبی
نارسایی قلبی سندرمی است که با خستگی و تنگی نفس توصیف میشود ولی اغلب این بیماران با کاهش در عملکرد جسمی، اجتماعی و دیگر ابعاد سلامتی مربوط به کیفیت زندگی مواجه هستند. این پژوهش با هدف بررسی تاثیر کوآنزیم کیوتن و تمرین ورزشی بر فشارخون، ضربان قلب استراحتی، حاصلضرب دوگانه و کیفیت زندگی در بیماران با نارسایی قلبی انجام شد. روش بررسی در این مطالعه، 40 بیمار داوطلب به طور تصادفی در چهار گروه بازتوانی، کوکیوتن، بازتوانی+کوکیوتن و کنترل قرارگرفتند. هر گروه شامل 10 نفر بود، گروه بازتوانی تمرینات را سه بار در هفته، هر جلسه بین 25 تا 45 دقیقه، با شدت 60 تا 80% حداکثر ضربان قلب انجام دادند. گروه دوم مکمل کوکیوتن 100 میلی گرمی را روزانه دو بار و گروه سوم علاوه بر تمرین، کوکیوتن دریافت کردتد. قبل و بعد از 8 هفته مداخله، ضربان قلب، فشارخون استراحتی و حاصلضرب دوگانه بیماران اندازهگیری شد و پرسشنامه کیفیت زندگی توسط بیماران تکمیل گردید. یافتهها بر اساس آزمون تی زوجی افزایش معنیدار در میزان حاصلضرب دوگانه و ابعاد مختلف کیفیت زندگی وکاهش معنیدار در ضربان قلب و فشار خون سیستولی استراحتی در سه گروه مداخله مشاهده گردید (05/ 0˂p). این افزایش در گروه کنترل معنیدار نبود. آزمون آماری تحلیل کواریانس نشان داد که معیارهای فوق در سه گروه مداخله در مقایسه با گروه کنترل تفاوت معنیداری داشت (05/ 0˂p). بحث و نتیجه گیری انجام بازتوانی و مصرف کوکیوتن، به ویژه توام کردن آن دو منجر به افزایش حاصلضربدوگانه و کاهش فشارخون و ضربان قلب استراحتی شد که میتواند بهبود کیفیت زندگی بیماران را به دنبال داشته باشد.

تأثیر هشت هفته تمرین ترکیبی و مصرف مکمل کوآنزیم کیوتن بر عملکرد حرکتی در بیماران مبتلا به مولتیپل اسکلروزیس
مقدمه عملکرد حرکتی در بیماران مبتلا به مولتیپل اسکلروزیس محدود میشود و انجام تمرینات ورزشی و مصرف مکمل کوآنزیم کیوتن میتواند در بهبود این وضعیت مؤثر باشد. هدف از این پژوهش، بررسی تأثیر 8 هفته تمرین ورزشی ترکیبی و مصرف مکمل کوآنزیم کیوتن بر برخی شاخصهای عملکرد حرکتی بیماران مبتلا به مولتیپل اسکلروزیس بود. روش کار در این تحقیق نیمهتجربی، 28 بیمار (16 مرد و 12 زن؛ میانگین سنی= 18/7±71/37 سال) مبتلا به مولتیپل اسکلروزیس (5-3 =EDSS) با روش نمونهگیری هدفمند انتخاب شدند ودر سه گروه تجربی (تمرین ورزشی ترکیبی+مکمل کیوتن، تمرین ورزشی ترکیبی+دارونما، مکمل کیوتن) و یک گروه کنترل (دارونما) مورد بررسی قرار گرفتند. تمرین ورزشی به مدت 8 هفته (دو جلسه مقاومتی و یک جلسه استقامتی در هفته) انجام شد و دوز مصرف مکمل کیوتن به میزان روزانه 200 میلیگرم بود. قبل و بعد از مداخله، بیماران آزمونهای عملکردی را انجام دادند. دادهها توسط روشهای آماری تحلیل کوواریانس، t همبسته و آزمون تعقیبی بنفرونی در سطح معناداری 05/0> pتحلیل شدند. نتایج نتایج آزمونهای تحلیل کوواریانس و t همبسته حاکی از کاهش معنادار مدت زمان انجام آزمونهای عملکردی 25 فوت راه رفتن و بلند شدن و راه رفتن زماندار و افزایش معنادار رکورد آزمونهای برخاستن از صندلی و 6 دقیقه پیادهروی در گروههای تمرین+مکمل و تمرین+ دارونما در مقایسه با گروههای مکمل و کنترل بعد از اتمام دورة مداخله بود (0001/0=p)؛ اما بین دو گروه تمرینی تفاوت معناداری وجود نداشت (05/0p ≥). نتیجه گیری بر این اساس، بیماران مبتلا به مولتیپل اسکلروزیس به منظور بهبود عملکرد حرکتی خود میتوانند از تمرین ترکیبی ارائه شده، با و یا بدون مصرف مکمل کوآنزیم کیوتن استفاده کنند. همچنین مصرف مکمل کوآنزیم کیوتن در بهبود عملکرد حرکتی این بیماران تأثیرگذار نیست.

مقایسه اثر 8 هفته تمرین ترکیبی با دو شدت متفاوت بر میزان هورمون ایریزین و شاخصهای گلیسمیک زنان دیابتی نوع
مقدمه ایریزین مایوکاینی است که از پروتئین غشایی FNDC5 آزاد میشود و تأثیرات مثبتی بر متابولیسم کربوهیدراتها دارد. پژوهش حاضر به بررسی اثر دو شدت تمرین ترکیبی بر میزان ایریزین، قند خون ناشتا، HbA1c و مقاومت انسولینی پرداخته است. روش کار در این مطالعه 26 زن دیابتی نوع 2 به گروه های، تمرین ترکیبی شدید، متوسط و کنترل تقسیم شدند. گروههای تمرینی، 8 هفته تمرین شامل 5 روز تمرین هوازی و 2 روز تمرین قدرتی با کش داشتند. تمرین هوازی گروه تمرینی شدید با 70 تا 89% HRmax و 75 دقیقه در هفته و تمرین هوازی گروه تمرین متوسط با 55 تا 69% HRmax و 150 دقیقه در هفته اجرا شد. قبل از شروع تمرینات و 48 ساعت پس از آخرین تمرین، نمونه خونی گرفته شد. از آزمون آنوا برای تحلیل دادهها استفاده گردید (p ˂ 05/0). نتایج تمرین ترکیبی منجر به افزایش هورمون ایریزین و کاهش قند خون ناشتا، HbA1c و مقاومت انسولینی در هر دو گروه شد. اگر چه افزایش هورمون ایریزین در گروه تمرینی شدید افزایش بیشتری داشت اما تفاوت معناداری در میزان قند خون، HbA1c و مقاومت انسولینی بین دو گروه مشاهده نشد. نتیجه گیری تمرینات ترکیبی جهت افزایش میزان ایریزین، کاهش قند خون ناشتا، HbA1c و مقاومت انسولینی و در نتیجه کنترل بیماری دیابت سودمند میباشند و میتوان از این تمرینات جهت بهبود شرایط گلیسمی استفاده نمود.
