تاثیر مصرف دو دوز مختلف چای سبز بر متابولیسم سوبسترا و هزینه انرژی قبل، حین و پس از یک جلسه فعالیت هوازی در زنان دارای اضافه وزن و چاق
مقدمه هدف تحقیق حاضر بررسی اثر مصرف دو دوز مختلف چای سبز بر متابولیسم سوبسترا و هزینه انرژی، قبل، حین و پس از یک جلسه فعالیت هوازی در زنان دارای اضافه وزن و چاق بود. روشکار 10 زن دارای اضافه وزن و چاق (وزن 96/11±71/80 کیلوگرم) به صورت داوطلبانه در یک طرح متقاطع شرکت کردند. این افراد با فاصله 12 تا 14 روز، در سه دوره شامل مصرف دوز 500 میلیگرمی چای سبز، دوز 1000 میلیگرمی چای سبز و دارونما شرکت کردند. پروتکل ورزشی دویدن بر روی نوارگردان با شدت 75% حداکثر ضربان قلب برای مدت 30 دقیقه بود. گازهای تنفسی آزمودنیها با استفاده از دستگاه گاز آنالایزر به مدت 30 دقیقه قبل از فعالیت در حالت درازکش (پایه)، 30 دقیقه حین فعالیت و 30 دقیقه بعد از فعالیت (EPOC) برای اندازهگیری میزان اکسیداسیون کربوهیدرات، چربی و هزینه انرژی جمعآوری شد. دادهها با استفاده از آزمونهای آنالیز واریانس چند متغیره و آنالیز واریانس با اندازهگیری مکرر تحلیل شد (05/0= α). نتایج نتایج نشان داد مصرف دوز 500 و 1000 میلیگرم چای سبز باعث افزایش معنادار اکسیداسیون چربی در حین و پس از فعالیت در مقایسه با گروه دارونما شد. بین دو دوز مصرفی تفاوت معناداری مشاهده نشد. اکسیداسیون کربوهیدرات و هزینه انرژی در بین سه گروه در حالتهای پایه، فعالیت و پس از فعالیت تفاوت معناداری نشان نداد. نتیجه گیری زنان دارای اضافه وزن و چاق بهتر است برای کاهش چربی بدن از مکمل چای سبز با دوز 500 میلیگرم همراه با فعالیت هوازی با شدت متوسط استفاده کنند.

تأثیر یک دوره بی تمرینی پس از شانزده هفته تمرینات هوازی بر سطوح سرمی کلسی تونین و پاراتورمون زنان میانسال مبتلا به استئوپنی
مقدمه فعالیت بدنی یک فاکتور مهم برای افزایش یا حفظ چگالی مواد معدنی مطرح شده است. از این رو مطالعه حاضر با هدف بررسی تأثیر یک دوره بی تمرینی پس از شانزده هفته تمرینات هوازی بر سطوح سرمی کلسی تونین و پاراتورمون زنان میانسال مبتلا به استئوپنی انجام شد.روش کار در این مطالعه نیمه تجربی 16 نفر از زنان غیر فعال مراجعه کننده به مرکز پزشکی هسته ای البرز شهرستان بابل انتخاب شدند که به طور تصادفی به دو گروه تمرینی و کنترل دسته بندی شدند. برنامه تمرین هوازی با شدت 65 تا 70% ضربان قلب هدف به مدت 16 هفته و هفته ای سه جلسه انجام شد. دوره بی تمرینی نیز 4 هفته در نظر گرفته شد. مقادیر سرمی کلسی تونین و پاراتیروئید در واحد ng/l مورد سنجش قرار گرفت. کلیه عملیات آماری با نرم افزارهای SPSS با ویرایش 23 در سطح معنی داری (05/0≥α) به اجرا درآمد.نتایجنتایج مطالعه حاضر افزایش هورمون کلسی تونین و کاهش هورمون پاراتیروئید را پس از 16 هفته تمرین هوازی و کاهش هورمون کلسی تونین و افزایش هورمون پاراتیروئید را پس از یک دوره بی تمرینی نشان داده است.نتیجه گیری در مجموع، به نظر می رسد انجام فعالیت بدنی طولانی مدت با شدت متوسط باعث تعادل ترشح هورمونهای مؤثر بر متابولیسم استخوان و تنظیم کلسیم میشود و احتمالاً از تبدیل شدن استئوپنی به استئوپروز جلوگیری میکند و در مقابل بی تمرینی و کاهش فعالیت بدنی این تنظیم مثبت را برهم میزند.

بررسی آگاهی، نگرش و عملکرد پزشکان متخصص غیر روانپزشک نسبت به درمانهای دارویی روانپزشکی، شهر زاهدان، سال 1396
مقدمه انگ زدن به بیماران روانی یکی از مشلات بزرگ آنها میباشد که تشخیص و درمان بهموقع این بیماران را دچار چالش و محدودیت میکند. بنابراین بررسی نگرش و آگاهی و عملکرد پزشکان متخصص غیر روانپزشک در زمینه درمانهای روانپزشکی، بهعنوان اولین سطح مراجعه بیماران از اهمیت به سزایی برخوردار میباشد. روش کار مطالعه حاضر از نوع توصیفی مقطعی می3باشد. حجم نمونه بهصورت سرشماری و شامل تمامی متخصصین غیر روانپزشک کلینیکال شهر زاهدان در سال 1396 بود. روش گردآوری دادهها با استفاده از پرسشنامه محقق ساخته ارزیابی آگاهی، نگرش و رفتار بود. برای تجزیه و تحلیل آماری اطلاعات گردآوریشده از آزمونهای کای دو ( K2) و T استفاده شد. نتایج تعداد 100 نفراز متخصصین بالینی غیر روانپزشک در دو گروه داخلی و جراحی در این پژوهش مشارکت داشتند. میانگین نمرهی سطح آگاهی نسبت به درمانهای دارویی روانپزشکی 42 بوده است (در محدودهی خوب)، میانگین نمرهی سطح نگرش 27 بوده است (در محدودهی نگرش مثبت)، میانگین نمرهی نوع عملکرد 24 بوده است (در محدودهی عملکرد خوب). تفاوتی ازنظر نوع تخصص، سن و جنس آزمودنیها و مدتزمان فعالیت بالینی در نتایج به دست نیامد. فقط ازنظر عضویت یا عدم عضویت هیئتعلمی دانشگاه تفاوت معنیداری نشان داد (P<0/05) که در گروه اعضای هیئتعلمی از نمرات بالاتری در زمینه آگاهی، نگرش و رفتار نسبت به درمانهای روانپزشکی حاصل شد. نتیجهگیری پزشکان متخصص غیر روانپزشک درمجموع از سطح آگاهی، نگرش و عملکرد خوبی نسبت به درمانهای دارویی روانپزشکی برخوردارند و سطح آگاهی اعضای هیئتعلمی بالاتر از سایر شرکتکنندگان بوده است. لزوم گسترش برنامههای بازآموزی بیشتر احساس میشود.

مقدمه بیماری عروق کرونر علت اصلی مرگ و میر بالغین است. تستهای غیر تهاجمی زیادی وجود دارند که در کنار معاینه و شرح حال به کشف زودرس بیماری عروق کرونر کمک کنند. هدف از مطالعه حاضر بررسی اهمیت استرین (کشش) میوکارد و آنالیز سرعت موج نبض در آئورت در ارزیابی غیر تهاجمی وسعت درگیری عروق کرونر است. روش کار در این مطالعه مقطعی 52 بیمار وارد مطالعه شده روز قبل از آنژیوگرافی، تحت اکوکاردیوگرافی قرار گرفتند. استرین طولی کلی میوکارد (GLS, Global Longitudinal strain) و سرعت موج نبض آئورت (PWV,Pulse Wave velocity) با اکوکاردیوگرافی اندازه گیری شد. تنگی کرونر در آنژیوگرافی مشخص شد. رابطه آماری بین PWV آئورت و نیز GLS (در اکو) با وجود و شدت تنگی کرونر در آنژیوگرافی کرونر، مورد بررسی قرار گرفت. نتایج از 52 بیمار این مطالعه، 37 بیمار (15/71 %) مذکر و 15 نفر (85/28 %) مونث بودند. بین GLS و وجود یا عدم وجود تنگی قابل توجه کرونری رابطه آماری معنی دار وجود داشت.(مقادیر p در شریان کرونری قدامی نزولی، سرکومفلکس چپ و راست به ترتیب 03/0 ، 04/0 ، 03/0). در حالیکه در مورد سرعت موج نبض، این ارتباط مهم نبود. نتیجه گیری در موارد مشکوک به بیماری عروق کرونر، کشش طولی گلوبال میوکارد در اکوکاردیوگرافی، یک پیشگویی کننده مستقل بیماری عروق کرونر قابل توجه است. در مقابل، سفتی شریانی از طریق اندازهگیری سرعت موج نبض آئورت فاکتور مستقلی در این زمینه محسوب نمیگردد.

بررسی تاثیر محیط مسکونی در افزایش خطر ابتلا به ایدز (نمونه موردی شهر مشهد)
مقدمه با وجود اینکه تحقیقات زیادی در رابطه با تاثیر محیط مسکونی بر وضعیت سلامت انجام شدهاست، اما در ارتباط با شیوع بیماریهای رفتاری در بین ساکنین شهر، روستا و حاشیه، مطالعهای صورت نگرفتهاست. به همین منظور، مطالعه حاضر با استفاده از نمونهای در حدود 300 نفر در مشهد انجام شدهاست. روش کار مطالعه حاضر یک بررسی توصیفی-تحلیلی است که در سالهای 1395 تا 1396 در شهر مشهد و اطراف آن به انجام رسیدهاست. از آنجا که تعداد افراد در معرض خطر بیماریهای رفتاری در این محدوده مشخص نبود، با روش آماری شکار مجدد[1]، تعداد جمعیت مبتلا تعیین و از بین مبتلایان 300 نفر مورد بررسی قرارگرفتند. دادههای گردآوریشده، با استفاده از نرمافزار SPSS مورد تجزیه و تحلیل قرار گرفت. نتایج نتایج نشان داد که میزان شیوع ایدز در میان حاشیهنشینان 94/1 نفر در هر 10 هزار نفر و سایر نقاط شهر 32/1 نفر و برای روستائیان 013/0 نفر است. رواج این نوع بیماری در بین مردان بیشتر از زنان بوده، ولی این اختلاف در بین جمعیتهای حاشیهنشین بسیار اندک است. با این که بین قومیت و الگوهای ابتلا به نوع بیماری تفاوتهایی وجود دارد، اما تاثیر این عامل به مکان فعلی سکونت افراد ارتباط بیشتری دارد. نتیجهگیری بر اساس این یافتهها، به دلیل رواج بیشتر این نوع بیماریها در مناطق فقیر و حاشیهنشین، ضرورت دارد تا مداخلات درمانی و آموزشهای مربوط به پیشگیری از آن، در این مناطق با شدت بیشتری پیگیری شود. مقایسه اثر بخشی مدل درمانی پاسخ محور و مدل درمانی – آموزشی با تاکید بر یکپارچگی حسی برنشانگان بالینی دانش آموزان با اختلال طیف اوتیسم
مقدمه اختلال طیف اوتیسم نوعی اختلال عصبی ـ رشدی است که بر اساس معیارهای پنجمین ویرایش راهنمای تشخیصی و آماری اختلالات روانی (DSM-5) با نقص در دو حیطه ارتباط اجتماعی، تعامل اجتماعی و نقص در رفتارها، علایق و فعالیتهای محدود و تکراری مشخص می شود. هدف این مطالعه مقایسه اثربخشی مدلهای درمانی پاسخ محور و درمانی ـ آموزشی با تاکید بر یکپارچگی حسی بر نشانگان بالینی دانش آموزان با اختلال طیف اوتیسم است. روش کار 36دانش آموز (21پسرو15دختر) با اختلال طیف اوتیسم از بین دانش آموزان مراکز تخصصی اوتیسم شهر مشهدکه در سال تحصیلی 97-1396 مشغول به تحصیل بودند، پس از غربالگری با روش نمونه گیری هدفمند انتخاب، و به صورت تصادفی در یک گروه کنترل و دو گروه آزمایش جایگزین شدند و به مدت 2ماه مداخلات درمانی را دریافت کردند. نتایج تجزیه و تحلیل نتایج نشان داد که مدل درمانی پاسخ محور و درمانی ـ آموزشی برکاهش نشانگان بالینی دانش آموزان با اختلال طیف اوتیسم تاثیر معنی داری داشته اند. اما با مقایسه اثر بخشی مدل ها، نتایج نشان داد که مدل درمانی پاسخ محور برکاهش نشانگان بالینی دانش آموزان آموزان با اختلال طیف اوتیسم به طور معنی داری اثر بخشی بیشتری از مدل درمانی ـ آموزشی دارد. نتیجه گیری تجزیه و تحلیل نتایج نشان داد که مدل درمانی پاسخ محور (PRT) ودرمان و آموزش کودکان اوتیستیک و ناتوانیهای ارتباطی مرتبط با آن (TEACCH) برکاهش نشانگان بالینی دانش آموزان با اختلال طیف اوتیسم تاثیر معنی داری داشته اند. اما با مقایسه اثر بخشی مدل ها، نتایج نشان داد که مدل درمانیPRT برکاهش نشانگان بالینی دانش آموزان با اختلال طیف اوتیسم به طور معنی داری اثر بخشی بیشتری از مدل درمانی (TEACCH) دارد. نتایج این مطالعه بیانگرآن است که با توجه به افزایش شیوع اوتیسم در دهه گذشته، توجه بیشتر،کاربرد مداخلات رفتاری آموزشی و برنامههای پیشگیری ضروری است.

بررسی الگوی مصرف مواد مخدر در مراجعین به مرکز اجباری نگهداری و درمان و کاهش آسیب معتادین ارومیه در سال 1394
مقدمه مطالعه حاضر با هدف بررسی الگوی سوء مصرف مواد در کمپ ترک اعتیاد دولتی یاغمور اعلی در شهرستان ارومیه در سال 1394 انجام شد. روش کار در مجموع اطلاعات 812 نفر که در سال 1394 در کمپ ترک اعتیاد یاغمور اعلی شهرستان ارومیه پذیرش شده بودند به روش مقطعی – توصیفی مورد ارزیابی قرار گرفت. اطلاعات بیماران در فرمهای از پیش طراحی شده جمع آوری و ثبت گردید و با استفاده از نرم افزار SPSS نسخه 20 و Excel مورد آنالیز قرار گرفت. نتایج تمامی بیماران مرد با میانگین سنی41/8 ± 60/36 سال بودند. حدود80%ساکن ارومیه،7/43% متاهل،2/51% بیکار بودند و 2/40% آنها شغل آزاد داشتند. حدود 9/56% معتادان سابقه ترک نداشتند. بیشترین و کمترین نوع ماده مخدر مصرفی در بیماران مورد بررسی به ترتیب هرویین تدخینی و حشیش بوده است (به ترتیب 7/50 و 9/0%). مصرف کلونازپام بیشترین داروی گزارش شده بود درحالیکه 75% سابقه مصرف دارویی نداشتند. سابقه زندانی شدن در 6/84 % افراد مشاهده گردید. مصرف 2 گرم ماده مخدر در روز بیشترین فراوانی را داشت. اغلب مراجعین ترکیبی از علایم روانی و جسمانی را گزارش نمودند. حدودا نیمی از مراجعین سابقه رفتار پرخطر داشتند. نتیجهگیری هرویین تدخینی و حشیش شایعترین ماده مصرفی بوده وتقریبا نیمی از مراجعان سابقه ترک پیشین داشتند. با توجه به شیوع بالای آسیبهای اجتماعی نظیر پایین بودن سطح تحصیلات، بیکاری ومتاثر شدن کانون خانواده در افراد متاهل، ضروروت مداخله هدفمند و برنامه ریزی جهت پیشگیری و کاهش عوارض این معظل ضرورت پیدا میکند.

یاورهای ایمونولوژیک: مروری بر عملکرد پذیرندههای Toll-like در راه اندازی پاسخهای ایمنی
چکیده سایتوکاینها نقش مهمی در کنترل سیستم ایمنی بدن دارند و پیام آنها پاسخ ایمنی را به سمت ایمنی هومورال یا ایمنی با واسطه سلولی هدفمند میکند. این مولکولها در پاسخهای ایمنی ذاتی و اکتسابی شامل بلوغ سلولهای عرضه کننده آنتی ژن، تمایز سلولهای Th1 و Th2 و القا سلولهای کشنده طبیعی و لنفوسیتهای T سیتوتوکسیک نقش کلیدی دارند. بنابراین، میتوانند به عنوان یاور واکسن در افزایش پاسخ ایمنی علیه عوامل بیماریزا موثر باشند. سلولهای دندریتیک مسوول جذب و ارایه آنتی ژن به سلولهای T، و هدف اصلی برای فعالیت یاور هستند. چندین پروتئین در تنظیم و حفظ هوموستاز سیستم ایمنی نقش دارند. خانواده پروتئینی TLR قادرند تولید سایتوکاینها شامل اینترفرونهای نوع I و سایتوکاینهای پیش التهابی را فعال کنند که به تنظیم و بیان ژنهای مرتبط با سیستم ایمنی منجر میشود. این پروتئینها تنظیم کنندههای کلیدی تولید اینترفرون در فیبروبلاستها و سلولهای دندریتیک تخصص یافته هستند و نقش آنها به عنوان یاور در افزایش پاسخهای ایمنی ارزیابی شده است. در این جستار، ساختار و عملکرد این پذیرندهها در تحریک پاسخهای ایمنی با تکیه بر مسیرهای پیام رسان آنها مرور شده است.
