ارزیابی کیفیت خدمات بیمارستانی از دیدگاه بیماران بستری در بیمارستانهای آموزشی دانشگاه علوم پزشکی جهرم در سال 1397
مقدمهمأموریت اصلی بیمارستان در نظام سلامت، تأمین مراقبت با کیفیت برای بیماران، و برآوردن نیازها و انتظارات آنها است. مطالعه حاضر با هدف ارزیابی کیفیت خدمات بیمارستانی از دیدگاه بیماران بستری در بیمارستانهای دانشگاهی شهرستان جهرم انجام شده است.روش کاردر این مطالعهی مقطعی طی سال 1397، دیدگاه 193 بیمار بستری در بیمارستانهای آموزشی شهرستان جهرم که به روش تصادفی انتخاب شده بودند، از طریق پرسشنامه SERVQUALمورد مطالعه قرار گرفت. این پرسشنامه مولفههای کیفیت را در ابعاد شش گانه شامل کیفیت خدمات فیزیکی و ملموس، تضمین، پاسخگوئی، قابلیت اطمینان خدمت، همدلی و دسترسی به مراقبت بررسی میکند. دادهها با استفاده از آزمونهای کروسکال والیس و من ویتنی تجزیه و تحلیل شدند.نتایجمیانگین سنی افراد تحت مطالعه 80/19± 19/45 سال بوده و 8/52% از بیماران مرد بودند. تفاوت معنی داری در میانگین نمره کیفیت در ابعاد شش گانه و بعد کیفیت کلی مشاهده شد. همچنین، اختلاف میانگین نمره کیفیت در تمامی ابعاد در بیمارستانها معنی دار بود. بود بیشترین میانگین نمره کیفیت به بعد کیفیت خدمات فیزیکی و ملموس (69/3± 63/20) و کمترین میانگین نمره به همدلی (79/3± 94/14) تعلق گرفت. هم چنین از بین ابعاد کیفیت خدمات بیمارستانی، تنها بین داشتن بیمه تکمیلی با بعد تضمین تفاوت معنی دار وجود داشت (024/0=p). ولی بین سایر ابعاد با متغیرهای دموگرافیک تفاوت معنی دار وجود نداشت(05/0 > p).نتیجهگیریکیفیت خدمات در بیمارستانهای آموزشی علوم پزشکی جهرم از دیدگاه بیماران در وضعیت مناسبی قرار داشت، لیکن اختلاف معنی دار نمره کیفیت بین ابعاد مختلف در هر بیمارستان، بیانگر لزوم توجه بیشتر مدیران نسبت به برنامههای بهبود کیفیت است تا رضایتمندی بیشتر بیماران را فراهم آورد.

بررسی ارزش تشخیصی مقاطع بافتی منجمد در بیماران با پاتولوژیهای پستان و تیروئید مراجعهکننده به بیمارستانهای آموزشی وابستهی دانشگاه علوم پزشکی بابل از سال 1390 تا 1395
مقدمهبا توجه به اهمیت نتایج مقاطع منجمد بافتی در تصمیم درمانی، این مطالعه با هدف تعیین ارزش تشخیصی مقاطع بافتی منجمد منجمد در مقایسه با پاتولوژی قطعی در جهت میزان اعتمادپذیری به آن در تشخیص ضایعات پستان و تیروئید در بیماران مراجعهکننده به بیمارستانهای آموزشی دانشگاه علوم پزشکی بابل حدفاصل سالهای 1390 تا 1395 انجام شد.روش کاربیمارانی که بدلیل ندول تیروئید یا توده پستان تحت نمونه برداری جهت تعیین نوع پاتولوژی با استفاده از روش مقاطع بافتی منجمد قرار گرفتند، وارد پژوهش شدند. اطلاعات دموگرافیک بیماران، تشخیص مقطع بافتی منجمد و تشخیص پاتولوژی قطعی جمعآوری و مورد آنالیز قرار گرفت.نتایجازمجموع 137 مورد ( 104 مورد تیرویید، 33 مورد پستان)، در تشخیص مقطع بافتی منجمد، 91 مورد(4/66%) از ضایعات خوشخیم و 46 مورد(6/33%) بدخیم گزارش شد که براساس پاتولوژی قطعی 52 مورد(38%) بدخیم و 85 مورد(62%) خوشخیم بودند. ارزش تشخیصی مقطع بافتی منجمد در تشخیص ضایعات بدخیم تیروئید با حساسیت 63%، ویژگی 99%، ارزش اخباری مثبت 92%، ارزش اخباری منفی 92% و دقت تشخیصی 3/92% تعیین گردید.در تمام موارد پستان نتیجه مقاطع منجمد بافتی با پاتولوژی دائمی یکسان بود لذا ارزش تشخیصی مقاطع منجمد بافتی در تشخیص ضایعات بدخیم پستان قابل محاسبه نمیباشد.نتیجه گیریبا توجه به میزان بالای همخوانی فروزن سکشن با برشهای دائمی ضایعات پستان و تیروئید میتوان نتیجه گرفت که روش تشخیصی مقطع بافتی منجمد روشی دقیق و با ارزش تلقی میشود.

بررسی میزان آ گاهی دستیاران تخصصی طب اورژانس نسبت به مباحث مرتبط با پزشکی قانونی در دانشگاه علوم پزشکی مشهد در سال تحصیلی 1397-1396
مقدمه
علم پزشکی و مسایل قانونی به طور جدا ناپذیری به هم مرتبطاند. نیاز به اطلاع از مباحث پزشکی قانونی در کشور در حال حاضر از هر زمان دیگری بیشتر اهمیت دارد. لذا هدف از انجام این پژوهش تعیین میزان آگاهی دستیاران طب اورژانس در مورد مباحث مرتبط با پزشکی قانونی میباشد.
روش کار
تعداد 30 دستیار طب اورژانس دانشگاه علوم پزشکی مشهد وارد این مطالعه توصیفی مقطعی شدند. اطلاعات زمینهای شرکت کنندگان از قبیل سن، جنس، سابقه کار، و سال ورود ثبت شد. آگاهی دستیاران با استفاده از یک آزمون پنج گزینهای 30 سؤالی سنجیده شد. تمامیآزمونها در نرم افزار spss نسخه 16 و در سطح معنی داری 05/0 انجام گردید.
نتایج
نتایج مطالعه ما نشان دادکه میانگین درصد پاسخهای صحیح به سوالات 11± 38 درصد بود. بیشترین میزان آگاهی دستیاران در مورد نحوه رویکرد به بیماران الکلی و مواد مخدر (48 ±65) و کمترین میزان آگاهی در مورد دیهها و قصور پزشکی و جواز دفن وگواهی فوت (22± 28) بود. بررسی مطالعه ما نشان داد ارتباط مستقیم و معناداری بین سن و میزان آگاهی کلی وجود دارد (54/0r=،03/0p=). ولی بین درصد آگاهی کلی و سابقه کار ارتباط معناداری وجود نداشت (51/0r=،07/0p=).
نتیجه گیری
این مطالعه نشان داد آگاهی دستیاران طب اورژانس از مباحث پزشکی قانونی متوسط است. کمترین میزان آگاهی در ارتباط با دیهها، قصور پزشکی و جواز دفن وگواهی فوت بود. با توجه به این نتایج بنظر میرسد آموزش مباحث کاربردی و رایج پزشکی قانونی به دستیاران طب اورژانس از اهمیت بالایی برخوردار باشد.

یافتههای نوار مغز سطحی صرع فوکال مقاوم به درمان در دو لوب گیجگاهی داخلی و خارجی
مقدمه صرع یکی از بیماریهای اعصاب نسبتا شایع در مغز میباشد، تعیین و مقایسه تغییرات الگوی الکتروانسفالوگرافیدر فازایکتال و اینترایکتالبیماران مبتلا به صرعهای داخلی لوب تمپورال و صرعهای ناشی از سایر ضایعات در خارج لوب تمپورال(نئو کورتکس). روش کار در این مطالعه مقطعی-توصیفی تعداد 80 بیمار شناخته صرع لوب گیجگاهی داخلی و خارجی که شرایط ورود به مطالعه را داشتند شرکت کردند، سپس از بیماران EEG مونیتورینگ گرفته شد و نتایج EEG در دو گروه صرع گیجگاهی داخلی و خارجی مقایسه شد. نتایج طبق نتایج بدست آمده از آنالیز دادههای بیماران، در تقسیم بندی نوع تشنج اختلاف معنی داری در نوع تشنج، سابقه تومور یا تروما یا هیپوکسی و مدت زمان سابقه تشنج بین دو گروه بیماران (صرع لوب گیجگاهی داخلی و خارجی) وجود نداشت (05/0 > p). محل شروع موج ایکتال در 5/52% بیماران در ناحیه گیجگاهی چپ و موج شروع ایکتال در 75/58% بیماران موج Slow بود، همچنین بین دو گروه بیمارن از نظر محل شروع موج ایکتال و موج شروع ایکتال اختلاف معنی داری وجود نداشت. نتیجه گیری طبق نتایج بدست آمده، EEG روش مناسبی برای تشخیص صرع لوب گیجگاهی میباشد ولی جهت تمایز نوع صرع گیجگاهی داخلی و خارجی کمکی نمیکند و به منظور افتراق این دو صرع بررسی EEG به تنهایی کمک کننده نیست و نیازمند روشهای تشخیصی دیگر است.

تأثیر 6 هفته تمرین هوازی و مصرف ژل خوراکی رویال بر فاکتورهای التهابی بیماران مالتیپل اسکلروزیس
مقدمهپاتوژنز MS شامل التهاب و فاز تخریب اعصاب است. MS با یک التهاب اتوایمیون به غلاف میلین و اغلب با یک حمله ناگهانی آغاز میشود؛ سپس وارد مرحله بهبود میشود که چند ماه تا چندین سال به طول میانجامد. پژوهش حاضر بهمنظور بررسی شش هفته تمرین هوازی و مصرف ژل رویال بر فاکتورهای التهابی TNF-α، hs-CRP و نوتروفیل در بیماران MS انجام شد.روش کاردر این پژوهش نیمه تجربی، 42 نفر بیمار مبتلابه MS شرکت کردند. آزمودنیها بهصورت هدفمند انتخاب و بهصورت تصادفی در 4 گروه:1-«ورزش+ژل رویال» (10 نفر)، 2-«ورزش+دارونما» (10 نفر)،3- «ژل رویال» (11 نفر) و 4-«دارونما» (11 نفر) تقسیم شدند. تمرین به مدت 6 هفته، هر هفته 3 جلسه روی چرخ کارسنج و باشدت 60% HRmax انجام شد. سپس بهصورت پلکانی افزایش یافت و در هفته ششم به 80% HRmax رسید. دوز مصرف روزانه ژل رویال 1000 میلیگرم بود. غلظت پلاسمایی فاکتورهای التهابی پیش و پس از پژوهش اندازهگیری شد.نتایجیافتهها نشان داد مقدار TNF-αدر گروههای «ورزش+ژل رویال» (039/0=P)، «ورزش+دارونما» (049/0=P) و «ژل رویال» (015/0=P) کاهش معنیداری داشته، اما در گروه «دارونما» معنیدار نبود (364/0=P). مقادیر hs-CRP در گروههای «ورزش+ژل رویال» (002/0=P)، «ورزش+دارونما» (047/0=P) و «ژل رویال» (042/0=P) تفاوت معنیداری داشت. نوتروفیل در گروههای «ورزش+ژل رویال» (028/0=P) و «ورزش+دارونما» (010/0=P) کاهش معنیداری داشت؛ این میزان در گروههای «ژل رویال» (252/0=P) و «دارونما» (365/0=P) معنیدار نبود.نتیجه گیرینتایج این پژوهش نشان داد تمرین هوازی و مصرف ژل رویال باعث بهبود TNF-α، hs-CRP و نوتروفیل در بیماران MS میشود.

مقدمه گرایش به تحقیق در زمینه طب سنتی /مکمل/جایگزین، حاصل تمایل متخصصین سلامت به خلق شواهد علمیدر این زمینه و تبدیل تجربیات، به شواهد حاصل از آزمایشها و تحقیقات علمیاست. پایان نامههای ثبت و دفاع شده میتواند روند علاقمندی دانشجویان و اساتید حوزههای مختلف تخصصی را به حیطه طب مکمل نشان دهند. لذا این تحقیق با هدف ارزیابی روند گرایش موضوعی پایان نامهها با موضوع طب سنتی /مکمل/جایگزین (به عنوان نمادی از جهت گیری کلی تحقیقات) انجام گرفت. روش کار این مطالعه از نوع توصیفی مقطعی میباشد که بامراجعه به نرم افزار جامع کتابخانه و بخش پایان نامههای کتابخانه از مجموع کل 7858 عنوان پایان نامه دانشگاه علوم پزشکی مشهد 418 عنوان آن مربوط به طب سنتی/مکمل/ جایگزین بود که استخراج و مورد بررسی قرار گرفت یعنی به ازای هر 19 عنوان پایان نامه یک عنوان پایان نامه مربوط به طب سنتی /مکمل/ جایگزین بود و این پایان نامهها مورد مطالعه قرار گرفتند. نتایج در این بررسی رشد پایان نامهها با موضوع طب سنتی کاملا مشهود است به این صورت که تعداد کل پایان نامهها در سال 1380 با 458 عنوان با رشد53/23% به 599 عنوان در سال 1395 رسیده است که در این مقایسه رشد تعداد پایان نامههای طب سنتی/ مکمل/ جایگزین از 24 عنوان در سال 1380 به 43 عنوان در سال 1395 رسیده یعنی با رشد 16/79% بین سال اول و آخر بررسی مواجه بوده است. نتیجهگیری باوجود رشد خوب پایان نامهها در زمینه طب سنتی /مکمل/ جایگزین طی سالهای اخیر ولی هنوز در مقایسه با تعداد کل پایان نامهها جایگاه مطلوبی ندارد. با ارائه سیاستهای تشویقی و اطلاع رسانی میتوان گرایش به این زمینه موضوعی را در میان پایان نامههای دانشگاه علوم پزشکی ارتقاء داد.

رابطه آشفتگی ذهنی و کیفیت خواب با حافظه کاری دانشجویان پزشکی
چکیده مقدمه: شناسایی عوامل تأثیرگذار بر حافظه کاری افراد، یکی از موضوعات مورد علاقه حوزه روانشناسی شناختی است. هدف پژوهش حاضر بررسی رابطه آشفتگی ذهنی و کیفیت خواب با حافظه کاری در دانشجویان پزشکی دانشگاه علوم پزشکی تهران بود.روش کار: روش این پژوهش توصیفی و از نوع همبستگی است و جامعه پژوهش، در این مطالعه، 120 نفر از دانشجویان پزشکی دانشگاه علوم پزشکی تهران با روش نمونه گیری در دسترس جهت شرکت در پژوهش انتخاب شدند.برای جمع آوری اطلاعات از پرسشنامه آشفتگی ذهنی، شاخص کیفیت خواب پیترزبورگ و نسخه نرم افزاری آزمون بلوکهای کرسی استفاده شد. اطلاعات جمع آوری شده با استفاده از نرم افزار Spss نسخه 21و آزمونهای آماری ضریب همبستگی پیرسون و رگرسیون چند متغیری به شیوه گام به گام مورد تحلیل قرار گرفت. همچنین برای بررسی روایی ابزارها، تحلیل عامل تاییدی با استفاده از نرم افزار AMOS انجام شد.نتایج: تحلیل دادهها نشان داد که آشفتگی ذهنی و کیفیت خواب با حافظه کاری همبستهاند. نتایج رگرسیون چند متغیری نشان داد که 30% از واریانس حافظه کاری دانشجویان توسط آشفتگی ذهنی و کیفیت خواب تبیین میشود.نتیجه گیری: تمرکز بر کاهش آشفتگی ذهنی و افزایش کیفیت خواب میتواند حافظه کاری را در دانشجویان بهبود ببخشد.

بررسی فراوانی عفونت سارکوسیستیس درگوسفندهای ذبح شده در کشتارگاه مشهد، به روش و ماکروسکوپی و هیستوپاتولوژی در طی سالهای 93-92
مقدمهآلودگی به سارکوسیستیس یکی از شایعترین عفونتهای تک یاختهای مشترک بین انسان و دام میباشد. باتوجه به اهمیت بهداشتی این انگل، در مطالعه حاضر میزان آلودگی به کیستهای ماکروسکوپی و میکروسکوپی سارکوسیستیس در گوسفند ذبح شده درکشتارگاه صنعتی مشهد مورد بررسی قرارگرفت.روش کاردر این مطالعه مقطعی، 105 لاشه گوسفند در طی فروردین ماه سال 92 تا دی ماه 93 جمع آوری گردید، نمونه برداری از مری، دیافراگم، قلب و عضلات بین دندهای آنها تهیه و به منظور یافتن کیستهای ماکروسکوپی و میکروسکوپی، نمونههای تهیه شده به دو روش مشاهده مستقیم (ماکروسکوپی) و هیستوپاتولوژی مورد بررسی قرار گرفتند. اساس تشخیص کیستهای میکروسکوپی، تهیه لام ازنمونهها و رنگ آمیزی هما توکسیلین ائوزین، سپس مشاهده میکروسکوپی برادی زوآیتهای انگل بوده است. آنالیز آماری با نرم افزار Spss صورت گرفته و سطح معنی داری 05/0 در نظر گرفته شده است.نتایجدر بررسی نمونههای مربوط به هر کدام از لاشههای گوسفندان از لحاظ وجود کیست سارکوسیستیس، در روش ماکروسکوپی 28/34% و در روش هیستوپاتولوژی90/81%، مثبت تشخیص داده شدند. همچنین اختلاف آماری معنی داری بین میزان آلودگی در عضلات مختلف، مشاهده گردید (05/0≤ p). در بررسی ماکروسکوپی بیشترین موارد آلودگی به سارکوسیستیس در دیافراگم و در روش میکروسکوپی مربوط به قلب بود. آلودگی در جنس نر بیشتر از ماده مشاهده شد. آلودگی به سارکوسیستیس درسن زیر یک سال بیشتر بوده است. نتیجه گیریپیشنهاد میشود بررسی دقیق لاشهها و حذف موضعی یا کلی لاشهها درکشتارگاه، صورت پذیرد و همچنین از مصرف گوشتهای گوسفند به صورت کبابی و نیم پز خودداری گردد.

نقش میانجیگر انگیزش تحصیلی در تبیین روابط بین خودکارآمدی تحصیلی و خود تنظیمی تحصیلی با فرسودگی تحصیلی در دانشآموزان دختر
مقدمهعوامل متعددی در فرسودگی تحصیلی دانشآموزان نقش دارند و مرور پژوهشها نشان دادند که بین خودکارآمدی و خودتنظیمی تحصیلی و فرسودگی تحصیلی ارتباط وجود داد، اما آیا انگیزش تحصیلی میتوان رابطه بین خودکارآمدی و خودتنظیمی تحصیلی با فرسودگی تحصیلی را میانجیگیری بکند؟این پژوهش با هدف بررسی نقش میانجی انگیزش در روابط بین خودکارآمدی و خود تنظیمی تحصیلی با فرسودگی تحصیلی دانشآموزان دختر پیشدانشگاهی انجام شد.روش کارروش تحقیق مطالعۀ حاضر، همبستگی بر اساس مدلیابی معادلات ساختاری بود. با استفاده از روش نمونهگیری تصادفی خوشهای تعداد 350 نفر دانشآموز از جامعۀ آماری دانشآموزان پیشدانشگاهی شهر تهران انتخاب شدند و به پرسشنامههای فرسودگی تحصیلی برسو و همکاران (1997)، پرسشنامه استاندارد خودکارآمدی تحصیلی میجلی و همکاران (2000)، پرسشنامه خودتنظیمی تحصیلی زیمرمن و مارتیز-پونز ( ۱۹۸۶) و انگیزش تحصیلی والرند و همکاران (1992) پاسخ دادند.نتایجنتایج آزمون مدل ساختاری نشان داد که خودکارآمدی تحصیلی با هر سه مؤلفه فرسودگی تحصیلی(خستگی تحصیلی، بی علاقگی و ناکارآمدی درسی) همبستگی منفی دارد (0/001≥p). انگیزه تحصیلی درونی به صورت منفی و بیانگیزگی تحصیلی و انگیزه تحصیلی بیرونی به صورت مثبت با مؤلفه های فرسودگی تحصیلی همبسته بودند (0/001≥p). در نهایت خود تنظیمی تحصیلی به صورت با مؤلفه های سه گانه فرسودگی تحصیلی همبسته بوده و ابعاد انگیزش تحصیلی به صورت معنادار رابطه بین خودتنظیمی تحصیلی و خودکارآمدی تحصیلی با فرسودگی تحصیلی را میانجیگری میکند (0/001≥p). در نهایت شاخص های برازندگی به دست آمده مدل کلی با داده های گردآوری شده، برازش مطلوب را دارد.نتیجه گیری انگیزش تحصیلی عنصری مهم و ضروری در دوره تحصیل است که میتوان با تقویت و بهبود آن در دانشآموزان، موفقیت تحصیلی آنها را تضمین نمود.

مدلی برای پیشبینی طلاق عاطفی بر مبنای سواد عاطفی، سبکهای مقابلهای، کیفیت زندگی و هیجان خواهی با میانجیگری تنظیم هیجان
مقدمه: هدف پژوهش حاضر ارائه مدلی برای پیشبینی طلاق عاطفی بر مبنای سواد عاطفی، سبکهای مقابلهای، کیفیت زندگی و هیجان خواهی با میانجیگری تنظیم هیجان بود.روش کار: روش این پژوهش از لحاظ هدف کاربردی و از نوع توصیفی- همبستگی با رویکرد مدل ساختاری بود. از جامعه زنان متأهل شاغل در مدارس ابتدایی دولتی شهر تهران با روش نمونه گیری تصادفی چند مرحلهای 350 نفر انتخاب شدند. به منظور جمع آوری دادهها پرسشنامه طلاق عاطفی گاتمن، کیفیت زندگی واروشریون، سبکهای مقابلهای لازاروس – فولکمن، هیجان خواهی زاکرمن، کارکردهای روانشناختی خانواده ایرانی کیمیایی و پرسشنامه دشواری تنظیم هیجان گراتز مورد استفاده قرار گرفت.نتایج: آزمون رگرسیون نشان داد که سواد عاطفی و کیفیت زندگی تاثیر منفی بر طلاق عاطفی دارند. هیجان خواهی، تنظیم هیجان و سبک گریز اجتناب تاثیر مثبت بر طلاق عاطفی دارند. سواد عاطفی با 37/0- بیشترین تاثیر را بر طلاق عاطفی دارد. ضرایب تاثیر غیر مستقیم با واسطه گری تنظیم هیجان نشان میدهد دوری گزینی و گریز-اجتناب تاثیر مثبتی بر طلاق عاطفی دارد. رویارویی، حمایت اجتماعی، کیفیت زندگی و مسئولیت پذیری دارای ضرایب تاثیر غیرمستقیم منفی هستند که به معنای تاثیر منفی آنها بر طلاق عاطفی با میانجیگری تنظیم هیجان است. دو متغیر گریز-اجتناب و کیفیت زندگی به هر دو شکل مستقیم وغیر مستقیم بر طلاق عاطفی اثر دارند.نتیجه گیری: بین سواد عاطفی، سبکهای مقابلهای، کیفیت زندگی و هیجان خواهی با طلاق عاطفی ارتباط وجود دارد که تنظیم هیجان میتواند این ارتباط را میانجیگری بکند.

مقدمه: آسم به عنوان یک بیماری جسمانی تحت تأثیر مسائل روانشناختی قرار دارد. هدف پژوهش حاضر بررسی فعالیت سیستم مغزی رفتاری و مکانیسمهای دفاعی در افراد مبتلا به آسم و افراد عادی میباشد.روش کار: به منظور انجام پژوهش تعداد 50 نفر بیمار مبتلا به آسم، مراجعه کننده به کلینیکهای تخصصی و مطبهای پزشکان متخصص در این زمینه و با استفاده از روش نمونهگیری در دسترس و غیر تصادفی و با 50نفر از افراد عادی و غیر مبتلا مقایسه شدند. شرکت کنندگان از طریق ابزار سنجش تحقیق شامل مقیاس مکانیسم دفاعی (DSQ) (1993) اندروز و همکارانش و پرسشنامهی شخصیتی گری-ویلسون جهت بررسی فعالیت سیستم مغزی رفتاری بودند. دادهها با آزمون تی مستقل تجزیه و تحلیل شد. تحلیل دادهها به کمک نرمافزارهای SPSS16و LISREL8/54و به روش آزمون تحلیل واریانس چند راهه و تحلیل مسیر انجام شد.نتایج: بیماران در بعد دلیل تراشی نسبت به افراد سالم در سطح بالاتری قرار دارند، نتایج جدول همچنین نشان میدهد در سایر ابعاد واکنش وارونه، بازگشت با جا به جایی با، انکار با،خیالبافی با والایش با، تبدیل با و توجیه عقلی با در سطح001/0 >p معنی داری است و بیماران در ابعاد فوق نسبت به افراد سالم در سطح بالایی قرار دارند،بین دو گروه در هر دو متغیر تفاوت معناداری داشتند(p < 0/05).نتیجه گیری: نتایج بدست آمده نشان داد که در تمام ابعاد مکانیسم دفاعی و فعالیت سیستم مغزی رفتاری کلی افراد مبتلا آسم با افراد سالم تفاوت معناداری دارند.

اثربخشی برنامه آموزشی مبتنی بر پذیرش و تعهد بر تعامل مادر-کودک مادران دارای کودک مبتلا به اختلال نارسایی توجه/بیش فعالی
مقدمه: تعامل مادر-کودک به عنوان یکی از مهمترین ارتباطهای کودکان در زندگی، اهمیت زیادی دارد. هدف این پژوهش تعیین اثربخشی برنامه آموزشی مبتنی بر پذیرش و تعهد بر تعامل مادر-کودک مادران دارای کودک مبتلا به اختلال نارسایی توجه/بیش فعالی بود.روش کار: در این پژوهش از روش نیمهآزمایشی و طرح پیشآزمون - پسآزمون با گروه کنترل و پیگیری سه ماهه بود. جامعه آماری این پژوهش را تمامی مادران دارای فرزند مبتلا به اختلال نارسایی توجه/بیشفعالی شهرستان مرند تشکیل دادند. از این جامعه، 40 نفر بصورت نمونه گیری در دسترس انتخاب شدند و بصورت تصادفی در دوگروه (20 تایی) آزمایشی و کنترل جایگزین شدند. پرسشنامه تعامل مادر-کودک رابرت (1961) در سه مرحله پژوهش اجرا گردید، برای گروه آزمایش هشت جلسه آموزش مبتنی بر پذیرش و تعهد اجرا شد. یافتههای پژوهش در نهایت با استفاده از نرمافزار SPSS نسخه 23 و آزمون اندازهگیری مکرر تحلیل شدند.نتایج: نتایج حاصل از اندازه گیری مکرر و آزمون های تعقیبی نشان داد آموزش مبتنی بر پذیرش و تعهد بر بهبود سبک تعامل پذیرش، تعدیل سبک تعامل بیشحمایتگری، سبک تعامل سهلگیری و سبک تعامل طرد مادران دارای کودک مبتلا به اختلال نارسایی توجه/بیش فعالی اثر بخش بوده و اثر برنامه آموزشی در طول زمان پایدار بوده است (001/0p ≤).نتیجه گیری: آموزش مبتنی بر پذیرش و تعهد بر تعامل مادر-کودک مادران دارای فرزند مبتلا به اختلال نارسایی توجه/بیش فعالی تاثیر دارد؛ لذا مشاوران و روانشناسان میتوانند از این رویکرد برای بهبود تعامل مادر-کودک، بهره ببرند.

اثربخشی شناخت درمانی مبتنی بر ذهن آگاهی بر سبکهای مقابله با استرس و کاهش افسردگی بیماران مبتلا به ام اس (مطالعه موردی بیماران مبتلا به ام اس در شهر تهران)
مقدمه پژوهش حاضر با هدف اثربخشی شناخت درمانی مبتنی بر ذهن آگاهی بر سبکهای مقابله با استرس و کاهش افسردگی بیماران مبتلا به ام اس شهر تهران در سال 1398 انجام شد.روش کار روش پژوهشی حاضر نیمه آزمایشی است و برای جمعآوری دادهها از طرح پیشآزمون-پسآزمون با گروه کنترل استفاده شده است. جامعه آماری پژوهش را 17000 بیمار عضو جامعه ام اس شهر تهران تشکیل دادهاند که از بین آنها، 30 نفر به روش نمونهگیری در دسترس انتخاب و بهطور تصادفی در گروه آزمایش (15 نفر) و گروه کنترل (15 نفر) گمارده شدند. گروه آزمایش درمان شناختی مبتنی بر ذهن آگاهی به مدت 8 جلسه 90 دقیقهای تحت آموزش قرار گرفتند و گروه کنترل در لیست انتظار ماندند. ابزارهای مورد استفاده در پژوهش حاضر شامل سبکهای مقابله با استرس لازاروس و فولکمن (1984) و افسردگی بک (1961) بودند که در دو مرحله پیشآزمون و پسآزمون مورد استفاده قرار گرفتند. تجزیهوتحلیل اطلاعات به دست آمده از اجرای پرسشنامه از طریق نرمافزار spss22 در دو بخش توصیفی و استنباطی (تحلیل کوواریانس) انجام گرفت.نتایجیافتهها نشان داد که شناخت درمانی مبتنی بر ذهن آگاهی باعث افزایش سبک مقابله ای مساله مدار و کاهش سبک مقابله ای هیجان مدار در بیماران مبتلا به ام اس شد. همچنین نتایج نشان داد که شناخت درمانی مبتنی بر ذهن آگاهی باعث کاهش افسردگی در بیماران مبتلا به ام اس گردید.نتیجه گیری به نظر میرسد، شناخت درمانی مبتنی بر ذهن آگاهی میتواند باعث کاهش افسردگی و تکنیکهای موثری را برای مقابله با استرس و هیجانهای منفی در بیماران مبتلا به ام اس ارائه دهد.

تاثیر برنامه چند مدلی مدیریت درد بر درد ذهنی، فاجعه سازی درد در زنان و مردان مبتلا به سندرم کرونری حاد
خلاصهمقدمه: هدف پژوهش حاضر بررسی تاثیر برنامه چند مدلی مدیریت درد بر درد ذهنی، فاجعه سازی درد در زنان و مردان مبتلا به سندرم کرونری حاد در سال 1398 بود.روش کار: روش پژوهش حاضر از نظر شیوه جزء طرحهای نیمه آزمایشی با پیش آزمون و پس آزمون با گروه کنترل است. جامعه آماری این پژوهش متشکل از همه بیماران زن و مرد بیمارستان رجایی قلب تهران در سال 1398 – 99 که در نوار قلبی خصوصیات سندرم کرونری حاد را داشته و تشخیص به تایید پزشک متخصص رسیده اند؛ بود. برای دستیابی به گروه نمونه به روش نمونهگیری در دسترس با افرادی که تمایل به شرکت در پژوهش داشتند انتخاب شدند و با توجه به جدول کوهن و میزان خطای 1 درصد 30 نفر بهصورت تصادفی در دو گروه 15 نفری، آزمایشی و گروه کنترل قرار گرفتند. در این مرحله پرسشنامههای درد ذهنی و فاجعه سازی درد به عنوان پیش آزمون از هردو گروه آزمایش و کنترل گرفته شد، سپس گروه آزمایش 8 جلسه برنامه چند مدلی مدیرت درد را دریافت کردند، پس از پایان 8 جلسه، از هردوگروه پس آزمون گرفته شد. برای تحلیل دادهها از آزمون تحلیل کوواریانس استفاده گردید.نتایج: دادهها حاکی از تفاوت معنادار گروه آزمایش و کنترل در مقیاس درد ذهنی و فاجعه سازی درد بود.نتیجه گیری: نتایج پژوهش تاثیر برنامه چند مدلی مدیریت درد را به عنوان یکی از روشهای حمایتی در کاهش درد ذهنی و فاجعه سازی درد در بیماران مبتلا به سندرم کرونری حاد نشان داد.
