بررسی پیامد درمان جراحی شکستگی گردن ران کودکان

چکیده

مقدمه: شکستگیهای گردن ران اطفال برخلاف بالغین ناشی از آسیب های شدید و عموماً همراه با آسیب سایر نقـاط بـدن مـی باشـند .
کم بودن تجربه خیلی از جراحان میزان وقوع این عوارض را در شکستگی های گردن ران اطفال اضافه می نماید . این مطالعـه بـا هـدف
بررسی نتایج و عوارض جراحی شکستگی گردن ران اطفال انجام شده است.
روش کار: این مطالعه توصیفی بین سالهای 1372 – 82 در 68 کودک با شکستگی گردن ران بـستری در بخـش ارتوپـدی بیمارسـتان
شهید کامیاب مشهد انجام شده است. مشخصات فردی، علائم بـالینی و نتـایج آزمایـشگاهی و عـسکبرداری، نتـایج درمـان جراحـی در
پرسشنامه جمع آوری گردید. اطلاعات جمع آوری شده با استفاده از آمار توصیفی و جداول توزیع فراوانی پردازش شد.
نتایج: در کل72% درصد از نظر رادیولوژیکی نتایج خوبی را نشان می دادند . عوارض عبارت بودند از: نکـروز آواسـکولردر 10 مـورد
(14/7%) کوتاهی اندام در 9 مورد (13/2%) فیوژن زودرس اپی فیز در 5 مورد (7/4%) کوکسا والگا در 4 مورد (5/9%) کوکـسا وارا در
2 مورد (2/96%) اسلیپ در 1 مورد (1/47%) و نان یونیون در هیچ مورد دیده نشد (0%).
نتیجه گیری: جاانداختن آناتومیک و جراحی زودرس از بسیاری از عوارض پیشگیری می کند آسیب وارده به استخوان در حال رشد
همیشه مستعد عوارض طولانی مدت می باشد پس هر قدم اشتباهی در درمان کودک، او را به طرف مـشکلاتی در طـول زنـدگی سـوق
می دهد. عوارضی که می تواند بر کل زندگی بیمار تاثیر داشته باشد . پس یک درمان خوب و دقیـق در ایـن چنـین شکـستگی هـا بایـد
منجربه ادامه زندگی راحت و با عملکرد مناسب باشد.

ntroduction:Femoral neck fractures in children, despite adults, are the result of high-energy trauma and, usually, accompanied with injuries of other parts of the body. The aim of this study was to evaluate the outcome and complications of femoral neck fracture surgeries. Material and Methods: This descriptive study was done on 68 children with displaced femoral neck fracture in Mashhad Kamiab Hospital between 1993 and 2003. Demographic characteristics, clinical, laboratory, radiography, and surgery results were collected by a questionnaire and analyzed by descriptive statistics and frequency distribution tables.
Results: Of 68 patients, 72% had good radiographic results. Complications included AVN in 10 (14/7%), limb shortening in 9 (13/2%), premature epiphysial fusion in 5 (7/4%), coxa valga in 4 (5/9%), Coxa vara in 2(2/96%) slip in 1 (1/47%). Nonunion was not seen.
Conclusion: Anatomical and surgery reduction of the fractured femoral neck lowers the risk of complications. Because of the rarity of this fracture that constitutes 1 % of the all pediatric fractures, Orthopedic surgeons treat only a few of such patients. The outcome of this fracture is strongly related to initial management.
