Early diagnosis of this complication can prevent more severe morbidity as well as enormous economic costs. Materials and Methods Based on the duration of the disease, 110 diabetic patients were divided in two groups of less than 10 and more than 10-year history. After taking complete history about the symptoms of neuropathy, the patients were examined neurologically. Then, electrodiagnostic studies were performed on the patients. Results 96 patients (87/2%) had diabetic neuropathy. The prevalence of neuropathy had a significant relationship with the duration of the disease (P value= 0.004). 61 patients (55/4%) complained of neuropathic symptoms. The results of the neurological exams were abnormal in 65 patients (59.09%). The most common symptom was paresthesia (50% of patients) and the most frequent sign was abnormal Achilles' reflex. 77 patients (70%) had abnormal indices in nerve conduction studies the most common of which was decrease amplitude of peroneal and sural nerves. 16 patients (14/5%) showed alterations in NCS in the absence of clinical signs or symptoms and 19 patients (17/2%) had normal NCS indices despite the presence of signs or symptoms of neuropathy. Conclusion High sensitivity of taking history and a careful neurological examination in the diagnosis of diabetic neuropathy necessitates physicians to pay more attention to clinical examinations and patient complaints, and avoid costly electrodiagnostic investigations. Thus, early diagnosis of sub-clinical neuropathy in these patients and providing them with necessary recommendations, irreversible complications such as amputation can be prevented.    }, keywords = {Diabetes Mellitus,Electrodiagnostic,Polyneuropathy}, title_fa = {بررسی فراوانی پلی نوروپاتی دیابتی بر اساس یافته های بالینی و فرابالینی}, abstract_fa = {مقدمه  نوروپاتی محیطی علت اصلی زخم پای دیابتی و قطع اندام شناخته شده است. بـا تشـخیص زودرس ایـن عارضـه میتوان از به وجود آمدن عوارض شدیدتر و نیز از تحمیل هزینه هنگفت اقتصادی جلوگیری نمود.  روش کار  این مطالعه توصیفی مقطعـی در سـال هـای 1388 - 1387 در مرکـز تحقیقـات غـدد و متابولیسـم دانشـگاه علـوم پزشکی مشهد انجام شده است . 110بیمار مبتلا به دیابت بر اساس سابقه ابتلا به بیماری به دو گروه با سـابقه بـیش از 10 و کمتر از 10 سال تقسیم شده و پس از اخذ شرح حال کامل در رابطه با علائم نوروپـاتی ، تحـت معاینـات عصبی قرار گرفتند. سپس مطالعات الکترودیاگنوستیک بر تمام بیماران انجام شد . اطلاعـات بـه دسـت آمـده بـا نرم افزار SPSS ، آزمون تی و کای دو تجزیه و تحلیل شد.  نتایج 96  نفر /87 )2 )% از بیماران دارای نوروپاتی دیابتی بودند. شیوع نوروپاتی با مدت بیماری رابطه مستقیم داشت (61 .( =p /0 004 بیمار /55 )4%) از علائم نوروپاتی شکایت داشتند و در 65 بیمار(59/09%) معاینهعصبی غیرطبیعی بود. شایعترین شکایت بیماران احساس گزگز و مورمور(50% بیماران) و شایعترین یافته معاینه مختل بودن رفلکس آشیل /47 )2%) بود. 77 بیمار(70%) یافتههای غیرطبیعی ه در شاخص ای هدایت عصبی داشتند که شایعترین آن کاهش آمپلیتود اعصاب پرونئال و سورال بود. در 16 بیمار /14 )5 %) تغییرات الکترودیاگنوستیک بدون حضور علائم و نشانههای بالینی مشهود بود و در 19 نفر /17 )2 )% علیرغم وجود علائم یا نشانههای عصبی نوروپاتی، مطالعات هدایت عصب طبیعی بود.  نتیجه گیری  حساسیت بالای شرححال و معاینه عصبی دقیق در تشخیص نوروپاتی دیابتی، ضرورت توجه بیشتر پزشکان به معاینه بالینی و شکایات بیمار ، و خودداری از بررسیهای پرخرج الکتروفیزیولوژی را نشان میدهد. پیشنهاد میشود با توجه به هزینه بالا و محدودیت مراکز تخصصی، انجام مطالعات الکترودیاگنوستیک فقط به بیمارانی محدود گردد که علیرغم ابتلای طولانیمدت به دیابت، علایم و نشانه ای به نفع نوروپاتی ندارند }, keywords_fa = {دیابت شیرین,پلینوروپاتی,الکترودیاگنوستیک}, url = {https://mjms.mums.ac.ir/article_5285.html}, eprint = {https://mjms.mums.ac.ir/article_5285_06ac86a4a50c7232ef3b65107743cd2a.pdf} }
