It can also interrupt wound healing and affect the amputation rate. Materials and Methods: This prospective study was done from November 2005 until July 2007 in Vascular Surgery Department of Imam Reza Hospital, Mashhad University of Medical Sciences. Infection rate of 100 CLU on amputated limbs were evaluated. Underlying disease was arthrosclerosis in 12%, Buerger’s Disease in 20% and Diabetes in 68% of the patients. Ulcers with duration of more than 3 months were considered chronic. The patients with ulcer duration more than 1 month whose leg amputation was performed due to severe pain (Buerger’s Disease), gangrene or signs of severe infection were also included. Infection was suggested by tissue culturing from the base of the ulcer before amputation. 2-5cc sterile isotonic saline from the intact clean skin was injected to the base of the ulcer and aspirated for culturing to compare with the results of tissue culture. Results: The sensitivity of needle-aspiration test for aerobic culture was 91%, the specifity 81%, positive predictive value 95% and negative predictive value 71%. For anaerobic needle-aspiration culture, sensitivity 90%, specifity of 87.5%, positive predictive value of 94% and negative predictive value of 80% were found. Conclusion: Since the signs of infection in ischemic ulcers can be furtive and due to presence of high antibiotic resistance of attained pathogens in this study and owing to the fact of resemblance between the results of tissue culture and aspiration culture, performing aspirated culture from every CLU and the antibiotic therapy based on the results of antibiogram may be suggested.}, keywords = {Needle-aspiration culture,Antibiotic,Infection,Chronic leg ulcers,Limb amputation}, title_fa = {برخورد با زخم های مزمن عروقی}, abstract_fa = {مقدمه: عفونت در زخم های مزمن ایسکمیک مسئله ای پیچیده است، چرا که از یک سو باعث عدم ترمیم زخم مزمن شده و از طرف دیگر خود یک عارضه است. کنترل عفونت در کنار کاهش و یا رفع سایر علل آسیب زا می تواند در حفظ عضو بیماران نقش مؤثری داشته باشد. روش کار: در مطالعه ای آینده نگر توصیفی به مدت 19 ماه از آبان ماه 1383 لغایت مرداد ماه 1385 در بخش جراحی عروق بیمارستان امام رضا مشهد،میزان عفونت در 100 زخم مزمن عروقی(شریانی) در اعضای قطع شده مورد بررسی قرار گرفت. بیماری زمینه ای در 12% موارد آترواسکلروز،20% بیماری بورگر و 68% دیابت بود. زخمی مزمن در نظر گرفته شد که بیش از 3 ماه از ازمان آن گذشته باشد. زخم هایی با طول مدت بیش از یک ماه که عضو به علت پیشرفت زخم به سمت گانگرن و یا درد شدید در بیماران مبتلا به بورگر و یا عفونت تهدید کننده حیات، قطع شد، نیز وارد مطالعه شدند. اثبات عفونت بر اساس نتیجه کشت بافتی بوده است. در این مطالعه پیش از تهیه نمونه پانچ جهت کشت بافتی، سطح زخم با سرم نرمال سالین و الکل شستشو داده شده و سپس 5-2 میلی لیتر نرمال سالین از سمت سالم پوست به طرف قاعده زخم تزریق و آسپیره شده و کشت هوازی و بیهوازی بر آن صورت گرفت تا نتایج آن که عملی غیر آسیب زا برای عضو ایسکمیک است، با نتایج حاصل از کشت بافتی مقایسه گردد. نتایج: میزان عفونت زخم بر اساس کشت بافتی 87% بوده است. برای جرم های هوازی حساسیت این روش 91% ، ویژگی 81%، ارزش اخباری مثبت 95% و ارزش اخباری منفی 71% یوده است. همچنین برای جرم های بیهوازی حساسیت این روش 90%، ویژگی 5/87%، ارزش اخباری مثبت 94% و ارزش اخباری منفی80% بوده است. نتیجه گیری: با توجه به درصد بالای عفونت زخم های مزمن عروقی، احتمال مخفی بودن علائم بالینی عفونی زخم به علت ایسکمی بافتی خصوصاٌ در اندام های فاقد نبض، مقاومت آنتی بیوتیکی جرم های به دست آمده و از طرفی همخوانی حساسیت و ارزش اخباری مثبت بالای کشت ترشحات آسپیره که روشی کم-تهاجمی است، پیشنهاد می شود از تمامی زخم های مزمن عروقی شریانی کشت ترشحات آسپیره انجام شده و آنتی بیوتیک تراپی مناسب بر اساس آنتی بیوگرام برای بیماران صورت گیرد.}, keywords_fa = {زخم مزمن عروقی,عفونت,قطع عضو,کشت اسپیراسیون بافتی,آنتی بیوتیک}, url = {https://mjms.mums.ac.ir/article_5528.html}, eprint = {https://mjms.mums.ac.ir/article_5528_616ec2237104a6e69b1efb60aa5dbef8.pdf} }
