This study was aimed to outline the clinical presentation and natural history of HU in children with hematuria, dysuria, discoloration of urine, and recurrent abdominal / flank pain. Material and Methodes: In this cross sectional study, from October 2002 to May 2006, 88 children with Hyperuricosuria at Dr Sheikh Hospital, Mashhad were evaluated. Urinalysis, urine culture, 24-hour urine collection for measurement of uric acid, Calcium and Creatinine, serum evaluation for BUN, Creatinine, uric acid, Calcium, Phosphorous and Hco3- and renal ultrasonugeraphy were carried out done for patients. All data were recorded and analyzed with descriptive statistical methods. Results: From 88 evaluated patients, 51% were boys and 48.8 % were girls. The mean age of children was 5.3± 1.2 years. The mean Urine Uric Acid was 13.4 mg/kg/24h and mean Urine PH was 5.3. Hematuria was not among the presenting symptoms in 41 patients. Thirty three patients had normal urinalysis at the first examination, 28 of them had microcalculi (< 3 mm in diameter) at renal ultrasonography and 12 had stones (4-13 mm). Dysuria and abdominal / flank pain were present in 22 and 17 patients, respectively. There was no microscopic hematuria in 24.3% of patients with microcalculi or stone. Family history of urolithiasis was positive in 63 of the children. Conclusion: Hyperuricosuria has to be suspected in children with dysuria, recurrent abdominal / flank pain, discoloration of urine, and familial history of urolithiasis. The lack of hematuria is not predictive of the absence of urolithiasis.}, keywords = {Hyperuricosuria,Urolithiasis,Hematuria,Dysuria,Abdominal pain}, title_fa = {علائم بالینی افزایش اسید اوریک در کودکان}, abstract_fa = {مقدمه: هیپریوریکوزوری ( افزایش ترشح اسیداوریک ادرار ) علائم متفاوتی در کودکان دارد و یکی از علل ایجاد سنگ کلیه است . هدف از انجام این مطالعـه بررسی علائم بالینی و تعیین شیوع هیپریوریکوزوری در کودکان مبتلا به هماچوری، دیزوری، درد راجعه شکم / پهلو و تغییررنگ صورتی یا نارنجی ادرار بوده است. روش کار : این مطالعه توصیفی از مهر ماه 1381 لغایت اردیبهشت 1385 در بیمارستان دکتر شیخ مشهد بر روی 88 کودک مبتلا به هیپریوریکوزوری انجام شده است. از کلیه کودکان از شروع تحقیق آزمایش کامل و کشت ادرار، اندازه گیری تصادفی و 24 ساعته اسید اوریـک، کلـسیم و کـراتینین ادرار، بررسـی میـزان اوره، کراتینین، اسید اوریک، کلسیم، فسفر و بی کربنات سرم و سونوگرافی از کلیـه و سیـستم ادراری انجـام شـد . مشخـصات فـردی، سـابقه بیمـاری، علائـم بـالینی، نتیجـه آزمایشات و سونوگرافی در پرسشنامه ای ثبت گردید. اطلاعات جمع آوری شده با استفاده از آمار توصیفی و جداول توزیع فراوانی پردازش شد. نتایج : از 88 کودک مورد مطالعه 51/1% پسر و 48/8% دختر بودنـد. میـانگین سـنی بیمـاران 5/3 ± 1/2 سـال بـود. میـانگین میـزان اسـیداوریک ادرار 24 سـاعته 13/4 mg/kg و میانگین PH ادرار 5/3 بود. در 41 بیمار هماچوری به عنوان علائم اولیه مراجعه وجود نداشت و در 33 بیمار آزمایش کامل ادرار اولیـه طبیعـی بـود. در سونوگرافی انجام شده در 28 کودک میکرولیتیازیس (قطر کمتر از 3 mm) و در 12 نفر سنگ کلیه (4-13 mm) گزارش شد. در آزمایش کامـل ادرار 24/3% از کودکان مبتلا به میکرولیتیازیس یا سنگ کلیه هماچوری وجود نداشت . دیزوری و درد راجعه شکم/ پهلو بـه ترتیـب در 22 و 15 بیمـار وجـود داشـت. سـابقه فامیلیـال سنگ کلیه در 63 بیمار مثبت بود. نتیجه گیری: در کودکان مبتلا به دیزوری، درد راجعه شـکم / پهلـو، تغییـر رنـگ صـورتی یـا نـارنجی ادرار بـا سـابقه فامیلیـال مثبـت از نظـر سـنگ کلیـه بایـد هیپریوریکوزوری را مدنظر داشت. همچنین فقدان هماچوری رد کننده سنگ کلیه نیست. }, keywords_fa = {هیپریوریکوزوری,سنگ کلیه,هماچوری,دیزوری,درد شکم / پهلو}, url = {https://mjms.mums.ac.ir/article_5558.html}, eprint = {https://mjms.mums.ac.ir/article_5558_d9fa5869f01c194a75bd709d80a284c8.pdf} }
