Ventricular tachycardia attacks can be treated using implantable cardioverter defibrillator (ICD) and this device prevents death due to Ventricular tachycardia attacks. For optimal use of this treatment method, it is better that individuals initially be selected based on clinical or laboratory criteria. This study aimed to evaluate clinical findings predictive of recurrence of heart attacks in patients undergoing ICD therapy. Subjects & Methods: In this retrospective cohort study, all patients referring to Ghaem Hospital in Mashhad from the beginning of 2005 until the end of September 2008 and underwent ICD for various clinical reasons, were included. Data analysis was conducted using R software. The frailty model was used to investigate the effect of factors, including age, gender, family history, ICD manufacturer, QTC and QRS time on receiving appropriate shocks. Findings: The mean age of patients was 46/63±15.51 years. A total of 30 patients (69.8%) were male and 38 patients (88.4%) do not have a family history of heart disease. In these patients, the mean and standard deviation of QTC and QRS duration were 467.4± 62.87 ms and 110.063± 31.64 ms, respectively. According to the results of frailty model fitting , QRS was the only variable affecting the appropriate shock. (P= 0.0016) Conclusion:Patients with longer QRS receive more appropriate shocks that are due to recurrent ventricular tachycardia}, keywords = {Key words: Recurrent Events,Implantable Cardiac Defibrillator,Cardiovascular Disease,Frailty Model}, title_fa = {مدت زمان QRS شاخصی در پیش گویی تاکیکاردی بطنی راجعه، در بیماران با دستگاه دفیبریلاتور داخل قلبی}, abstract_fa = {مقدمه : حملات تاکی‌کاردی بطنی یکی از مهم‌ترین علل مرگ قلبی است. حملات تاکی کاردی بطنی با تعبیه دفیبریلاتور داخل قلبی (ICD)   قابل درمان است و از مرگ به علت حمله قلبی پیش‌گیری می‌کند . برای استفاده بهینه از این روش درمانی بهتر است افراد در ابتدا براساس معیارهای بالینی یا آزمایشگاهی انتخاب شوند. این مطالعه با هدف بررسی یافته‌های بالینی که پیشگویی‌کننده عود حملات قلبی در بیماران تحت درمان ICD  هستند، انجام شده است. روش کار: در این مطالعه همگروهی تاریخی، کلیه بیمارانی که از ابتدای سال 1384 لغایت پایان شهریور ماه  سال 1387، به مرکز قلب بیمارستان قائم (عج) مشهد مراجعه کرده و به دلایل مختلف بالینی برای آنها ICD تعبیه گردید، مورد مطالعه قرار گرفتند و تأثیر عواملی چون سن، جنس، سابقه خانوادگی، شرکت سازنده دستگاه ICD، مدت QTC و مدت QRS  بر دریافت شوک‌های به‌جا را با استفاده از مدل شکنندگی که یکی از روش‌های تحلیل پیشامدهای بازگشتی می‌باشد، تجزیه و تحلیل شد. نتایج: میانگین و انحراف معیار سن بیماران51/15 ±63/46 سال می‌باشد. تعداد  30 بیمار (8/69 %) مرد هستند و 38 نفر (4/88 %) سابقه خانوادگی ابتلا به بیماری‌های قلبی را ندارند. میانگین و انحراف معیار مدت زمان QTC در این بیماران 87/62 ±4/467 هزارم ثانیه و میانگین و انحراف معیار مدت زمان QRS نیز 64/31±063/110 هزارم ثانیه می‌باشد. با توجه به نتایج برازش مدل‌ شکنندگی تنها متغیر QRS معنی‌دار گزارش شده‌است (0016/0=p) و بدین صورت است که بیماران با QRS طولانی‌تر حملات بیشتری را دریافت می‌کنند. نتیجه گیری: بیماران با QRS  طولانی‌تر شوک های به جای بیشتری را دریافت می‌کنند که به دلیل حملات تاکی کاردی بطنی راجعه می باشد.}, keywords_fa = {کلمات کلیدی:پیشامدهای بازگشتی,دفیبریلاتور کاشتنی قلبی (ICD),بیماری قلبی,مدل شکنندگی}, url = {https://mjms.mums.ac.ir/article_9524.html}, eprint = {https://mjms.mums.ac.ir/article_9524_aa3052fb004eca50570fdcee172b7de7.pdf} }
