Hyperparathyroidism, glucocorticoid treatment, and impairment of renal tubular phosphate reabsorption are the most common causes of hypophosphatemia in these patients. It is well-known that dipyridamole enhances renal tubular phosphate reabsorption in some clinical conditions. We did not find any information about the effect of dipyridamole in renal transplant recipients (RTRs) with hypophosphatemia. For this reason, we decided to give dipyridamole 28 RTRs with hypophosphatemia. Subjects & methods:A total of 28RTRs whose serum phosphate and creatinine levels were below 2.5 mg/dl and 2 mg/dl, respectively, were included in this study. None of the patients received drugs altering phosphate metabolism and they did not change their routine diets. Urinary phosphate excretion and tubular phosphate reabsorption (TPR) were calculated before 3 weeks after dipyridamole treatment. Results:The mean levels of serum and urine (daily) phosphate and TPR were 1.68±0.36 mg/dl, 725.32±544.25 mg/day and 999.44±0.4, respectively. After treatment, the mean levels of serum-urine phosphate and TPR were 2.47±0.72 mg/dl, 667.91±259.85 mg/day and 999.69±0.22, respectively. Serum phosphate level and TPR were found to be significantly different before and after dipyridamole therapy. No significant change was observed in phosphate level before and after dipyridamole therapy. Conclusion:On Short-term dipyridamole treatment no negative effect was observed on renal functions in these cases. Although the number of cases included in this study is small, dipyridamole is an effective choice in management of hypophosphatemic RTRs.}, keywords = {hypophosphatemia,Renal transplantation,dipyridamole}, title_fa = {بررسی تاثیر دی پیریدامول در درمان هیپوفسفاتمی در گیرندگان پیوند کلیه}, abstract_fa = {مقدمه  هیپوفسفاتمی می­تواند در بیماران گیرنده پیوند کلیه دیده شود. دی پیریدامول سبب افزایش جذب توبولی فسفات در بعضی از شرایط کلینکی میشود. در حال حاضر اطلاعات اندکی در باره اثرات این دارو در بیماران پیوندی وجود دارد. روش کار  در مطالعه حاضر 28 بیمار پیوند کلیوی که کمتر از 9 ماه از پیوند سطح سرمی فسفرکمتر از 2 میلی گرم در دسی لیتر آن­ها می­­­گذشت، انتخاب شدند و پس از بررسی اولیه از نظر کلسیم، فسفر، PTH ، GFR و renal tubular reabsorption of pi دوز تجمعی استروئید، سطح سرمی iminoral آن­ها،این بیماران تحت درمان با دی پیریدامول 75 میلی گرم 4 بار در روز قرار گرفتند و سپس در پایان 3 هفته مجددا بررسی فوق تکرار شد و نتایج به­دست آمده مقایسه شدند.  نتایج هیچ کدام از بیماران دارویی که متابولیسم فسفات را تغییر دهد دریافت نمی­کردند. متوسط سطح سرمی و سطح ادراری فسفات قبل از درمان به­ترتیب برابر با 36/0± 68/1 و 32/735 میلی­گرم در دسی لیتر و پس از درمان متوسط سطح سرمی و ادراری فسفات به­ترتیب برابر با 72/0 ± 47/2 و 91/667 بود. در سطح سرمی فسفات و مقدار جذب توبولی آن قبل و پس از درمان با دی پیریدامول اختلاف قابل توجه­ای مشاهده شد، اما در سطح ادراری اختلاف قابل توجه­ای مشاهده نشد. نتیجه­گیری  درمان کوتاه مدت با دی پیریدامول می­تواند سبب افزایش سطح سرمی و افزایش جذب توبولی فسفات شود. در این مطالعه اثرات منفی بر فانکشن کلیه با این درمان مشاهده نشد.}, keywords_fa = {هیپوفسفاتمی,پیوند کلیه,دیپیریدامول}, url = {https://mjms.mums.ac.ir/article_9661.html}, eprint = {https://mjms.mums.ac.ir/article_9661_0df62365285499d11d93510e3ffa7345.pdf} }
