بررسی عوامل مرتبط با تشکیل سنگ صفراوی در مبتلایان به چاقی مرضی که تحت عمل جراحی باریاتریک قرار گرفته اند
مقدمه نه تنها چاقی مرضی، بلکه کاهش سریع وزن پس از جراحی باریاتریک، به عنوان روش درمان چاقی مرضی، از جمله عوامل ایجاد سنگهای صفراوی محسوب میشوند. در مطالعه حاضر با ارزیابی عوامل مرتبط ایجاد سنگهای صفراوی، نیاز به کوله سیستکتومی پروفیلاکتیک حین عمل جراحی باریاتریک بررسی میشود. روش کار در مطالعه حاضر مقطعی اطلاعات کلیه بیماران مبتلا به چاقی مرضی که از فروردین ماه سال 1390 تا فروردین ماه 1395 در بخش جراحی بیمارستان امام رضا (ع) تحت عمل جراحی قرار گرفته بودند، مورد بررسی قرار گرفت. در نهایت کلیه متغیرهای استخراج شده از پرونده بیماران وارد نرم افزار SPSS شد و آنالیز دادهها انجام گرفت. نتایج در مطالعه حاضر 150 بیمار مورد بررسی قرار گرفتند. از میان 127 بیماری که سابقهی سنگ کیسه صفرا نداشتند، 59 نفر(5/46%) از نظر باکتری H. pylori مثبت بودند و از مجموع 23 نفری که سابقه سنگ کیسه صفرا و جراحی کیسه صفرا به دلیل سنگ را در گذشته داشتند، 17 نفر(9/73%) از نظر باکتری H. pylori در معده (با کمک تست تنفسی تشخیصی) مثبت بودند (05/0< p). نتیجه گیری به طور کلی در مطالعهی حاضر تنها عامل مرتبط با تشکیل سنگ کیسه صفرا، ابتلا به H. pylori بود. با توجه به نتایج، با احتساب عفونت با H. pyloriبه عنوان یکی از عوامل خطر درمان پذیرِ تشکیل سنگ صفراوی، میتوان قبل از انجام جراحی چاقی، برای افراد مبتلا درمان دارویی انجام داد تا از احتمال تشکیل سنگ کیسه صفرا در این افراد کاسته شود.

اثربخشی هیپنوتیزم درمانی بر استرس ادراک شده و فشار خون بیماران مبتلا به فشار خون اولیه
مقدمه فشارخون بالا از شایعترین عوامل خطرساز بیماریهای قلبی- عروقی است که منجر به مسائل و مشکلات زیادی میگردد. همچنین استرس ادراک شده منجر به افزایش فشار خون و افت عملکرد این افراد میشود. لذابا توجه به اهمیت بیماری فشار خون، این پژوهش با هدف بررسی اثربخشی هیپنوتیزم درمانی بر میزان استرس ادراک شده و فشار خون در بیماران مبتلا به فشار خون اولیه انجام گرفته است. روش کار روش پژوهش نیمه آزمایشی از نوع پیش آزمون- پس آزمون با گروه گواه بود. بدین منظور، به صورت نمونهگیری در دسترس از بین بیماران مبتلا به فشارخون اولیه مراجعه کننده به مراکز درمانی شهر مشهد در بازه زمانی تابستان 1395، تعداد 30 نفر انتخاب و به طور تصادفی در دو گروه آزمایش و گواه جایگزین شدند(هر گروه 15نفر). بیماران در دو مرحله قبل و بعد از مداخله به پرسشنامه استرس ادراک شده پاسخ دادند. به علاوه بیماران، چارت ثبت میزان فشارخون را به مدت 15روز قبل و بعد از مداخله، در دو نوبت صبحگاهی و شامگاهی تکمیل نمودند. گروه آزمایش سه جلسه تحت هیپنوتیزم درمانی قرار گرفتند، اما در گروه گواه هیچ مداخله روانشناختی صورت نگرفت. نتایج نتایج تحلیل کواریانس نشان دادکه هیپنوتیزم درمانی استرس ادراک شده و میزان فشار خون در بیماران مبتلا به فشار خون اولیه شهر مشهد را کاهش داده است(001/0p<). نتیجه گیری با توجه به نتایج این پژوهش، میتوان از هیپنوتیزم درمانی، برای کاهش استرس ادراک شده و فشار خون در بیماران مبتلا به فشار خون اولیه سود برد.

بررسی ارتباط پلیمرفیسم rs3745453 با ریسک ابتلا به بیماری مالتیپل اسکلروزیس
مقدمه مولتیپل اسکلروزیس(MS)، از بیماریهای خود ایمن سیستم عصبی مرکزی است، امروزه مشخص شده است که تغییر در توالی تک نوکلئوتیدها یا SNP با ابتلا یا مقاومت به بیماریها در ارتباط است. در مطالعات اخیر ارتباط پلی مرفیسم میکرو RNA با بیماریهایی از جمله سرطان و بیماریهای خود ایمنی گزارش شده است. میکروRNA گروهی از RNA های غیرکدکننده بوده که در انواع فرایندهای بیولوژیک سلولی مانند رشد، تقسیم سلولی و ایمنی اثرگذارند. روش کار از71 بیمار MS و71 فرد سالم، نمونه خون جمع آوری وDNA استخراج شد. آنالیز ژنوتیپی با تکنیک TARMS PCR بررسی شد. ارتباط پلی مرفیسم rs3745453 با بیماری ام اس با آزمونهای آماری ارزیابی شد. نتایج فراوانی ژنوتیپی TT ، TC وCC در افراد بیمار 34 ، 24،13 و در گروه کنترل 39، 11 و21 درصد بود. در هر دو گروه، ژنوتیپ TT غالب و فراوانی هتروزیگوتها در افراد بیمار نسبت به گروه کنترل بیشتر بود. نسبت شانس (OR) در ژنوتیپ TC برابر با 7/2 بود. به عبارتی افراد هتروزیگوت، 7/2 برابر نسبت سایر افراد شانس ابتلا به بیماری MS را دارند. نتیجه گیری مطالعه حاضر اولین گزارش از ارتباط بین پلی مرفیسم mir23a و بیماری MS میباشد. با توجه به نتایج به دست آمده میتوان گفت که پلی مورفیسم mir 23a با شناسه rs3745453 با MS در ارتباط است (029/0=p).

مقایسه میزان مرگ ناشی از فسفید آلومینیوم با سایر مرگهای ناشی از مسمومیت در استان گلستان در سالهای 1394 – 1389
مقدمه فسفید آلومینیوم از علل شایع مرگ ناشی از مسمومیت در جهان میباشد. بررسی حاضر به منظور مقایسه میزان مرگ ناشی از فسفید آلومینیوم با سایر مرگهای ناشی از مسمومیت در استان گلستان صورت گرفته است. روش کار مطالعه به شکل توصیفی- گذشته نگر بر روی تمامی اجساد ارجاعی به اداره کل پزشکی قانونی استان گلستان که در فاصله سالهای 1394 – 1389 به دلیل مسمومیت با فسفید آلومینیوم و سایر مسمومیتها فوت نموده اند انجام شد. اطلاعات مربوطه مانند سن، جنس، وضعیت تاهل، شغل، تحصیلات، محل سکونت، زمان و نوع مسمومیت، عامل مسمومیت و محل مسمومیت و فوت از پرونده افراد جمع آوری وپردازشگردیدهاست. نتایج از 420 مورد مرگ ناشی از مسمومیت، 105 مورد (%25) به دلیل فسفید آلومینیوم بوده که بعد از مواد مخدر دومین علت مرگ ناشی از مسمومیت میباشد. از کل 105 نفر، 8/63% (67 نفر) مرد و 9/42% (45 نفر) مربوط به طبقه 29-20 سال بودهاند. 7/46% موارد(46 نفر) متاهل و 40% آنها (42 نفر) تحصیلات تا مقطع متوسطه داشتهاند. از طرفی 9/82% (87 نفر) موارد ساکن شهر بودهاند. همچنین 99% (104 نفر) مرگهای ناشی از فسفید آلومینیوم به دلیل اقدام به خودکشی بوده که غالبا در زمستان (3/33 % ، 35 نفر) به وقوع پیوسته است. نتیجه گیری تایج نشان میدهد فسفید آلومینیوم دومین علت مرگ ناشی از مسمومیت در استان گلستان بوده که بیشتر در مردان و به دلیل اقدام به خودکشی رخ میدهد.

بررسی ارتباط وضعیت امنیت غذایی خانوار و برخی عوامل اقتصادی- اجتماعی با ضریب هوشی کودکان در سال 1395درمشهد
مقدمه ناامنی غذایی از مقولههایی است که تاثیر جدی بر وضعیت تغذیه افراد جامعه برجای میگذارد. ضریب هوشی از شاخصههای ارزیابی میزان کارایی افراد است. تاکنون درباره وضعیت امنیت غذایی و عوامل اجتماعی- اقتصادی خانوار مرتبط با ضریب هوشی کودکان، مطالعات محدودی صورت گرفته است. مطالعه حاضر با هدف بررسی ارتباط وضعیت امنیت غذایی خانوار و برخی عوامل اقتصادی- اجتماعی با ضریب هوشی در کودکان پیش دبستانی شهر مشهد انجام شده است. روش کار در این مطالعه مقطعی _ توصیفی که از ابتدای شهریور 1394 تا پایان مهر 1395 در پیش دبستانیهای شهر مشهد انجام شد و براساس پرسشنامه امنیت غذایی 18 گویهای USDA، 200 کودک در گروه امن غذایی و 200 کودک در گروه ناامن غذایی قرار گرفتند. اعضای هر گروه به صورت 1 به 1 از نظر سن و جنس همسان سازی شدند. گروه سنی کودکان مورد مطالعه بین 6-4 سال قرار داشت. پرسشنامه امنیت غذایی 18 گویهای USDA و پرسشنامه اطلاعات عمومی، از طریق مصاحبه با مادر تکمیل شد. ارتباط بین متغیرهای کیفی و کمی با ضریب هوشی با آزمون کای مربع (Chi-Square)، تی مستقل(T-Independent) و تحلیل واریانس یک طرفه (ANOVA) تعیینشد. نتایج میانگین ضریب هوشی کلامی، عملی و کلی در گروه ناامن غذایی بدون گرسنگی به ترتیب 24/13±75/98، 56/12±94/102 و 04/11±84/100 بود که از میانگین ضریب هوشی کودکان در گروه امن غذایی، پایینتر بود. ضریب هوشی با شغل سرپرست خانوار (کارمند بودن)، شاغل بودن مادر، تحصیلات بالاتر سرپرست و مادر، شخصی بودن مسکن و وضعیت اقتصادی خوب ارتباط معنی داری داشت. نتیجه گیری امنیت غذایی از طریق تاثیر بر تغذیه و به عنوان یک فاکتور استرس زایی که کل خانواده را درگیر میکند، بر ضریب هوشی موثر است.

ارتباط علائم بالینی و عملکرد ریوی بیماران مسلول درمان شده در استان قم طی سالهای1383-1392
مقدمه اقدامات فراوانی در جهت پیشگیری، کنترل و درمان سل صورت گرفته است با این وجود جزء بیماریهای عفونی مهم در کشور میباشد. بیش از 80% موارد سل مربوط به سل ریوی است. با وجود درمان، در غالب موارد ضایعه در ریه خواهیم داشت که موجب کاهش عملکرد ریوی بیماران میشود. مطالعه حاضر با هدف بررسی علائم بالینی و عملکرد ریوی بیماران بعد از درمان سل میباشد. روش کار این مطالعه از نوع تحلیلی مقطعی و در دسترس بوده و بعد از بررسی معیارهای ورود و خروج، پژوهش بر روی 247 بیماری که از سال 1384 تا 1392 مراجعه کرده بودند صورت گرفت از همه بیماران اسپیرومتری ساده گرفته شد و پرسشنامه دموگرافیگ و سنت جورج اخذ گردید. سپس داده ها توسط نرم افزار SPSS (نسخه17 ) توسط شاخصهای آمار مرکزی و نیز توزیع فروانی و همچنین توسط آزمونهای T مستقل، ضریب همبستگی پیرسون و نیز کای اسکوئر در سطح معنی داری 05/0> p تجزیه و تحلیلشد. نتایج سن متوسط بیماران 31/48 سال و 64% خانم بودند. بیش از نیمی از بیماران علائم سرفه، خلط و نفس تنگی و درگیری ریوی در اسپیرومتری داشتند. ارتباط معناداری مابین علائم بالینی با اسپیرومتری و شدت درگیری آن ملاحظه شد(001/0< p). نتیجه گیری با وجود درمان موثر و به حتی به موقع سل، در غالب موارد ضایعه ای در ریه باقی میماند که موجب باقی ماندن علائم ریوی تا آخر عمر خواهد شد.

مقدمه بسیاری از مطالعات و بررسیها نشان دادهاند که ویروسها میتوانند پس از ورود به سلول میزبان با میتوکندری تعامل کرده و عملکرد آن را مختل گردانند به ویژه اینکه برخی تحقیقات مشخص کردهاند که این اثرات میتواند تغییراتی در ساختمان میتوکندری و همچنین تغییراتی در مسیر تولید انرژی سلولی در زنجیره انتقال الکترون ایجاد نماید که این امرگاها باعث کاهش شدید انرژی سلولی در انتهای فرایند انتقال الکترون خواهد شد. علاوه بر این، اکنون دادههای جدید نشان می دهد که تعدادی از ویروسها ممکن است روی مسیر میتوکندریایی آپوپتوز نفوذ ودر نتیجه باعث اثر در زنجیره تنفس در میتوکندری شوند. چنین اثراتی میتواند پیآمدهای بالینی بویژه در زمینه عفونتهای پایدار ایجاد کرده که در نهایت میتواند به عنوان علت احتمالی برای سندرم ضعف مزمن (CFS) در نظر گرفته شود. بنابراین بعنوان یک فرضیه میتوان نتیجه گیری کرد که شاید عفونتهای ویروسی یکی از اتیولوژیهای احتمالی اصلی برای این سندروم باشد.

ارزش بالینی فلوروکوئینونها در درمان سل ریوی و سویه های مقاومMDR-TB
مقدمه بیماری سل یکی از معضلات مهم بهداشتی و یکی از کشندهترین بیماریهای عفونی در بسیاری از کشورهای در حال توسعه میباشد که به واسطه باسیل مایکوباکتریوم توبرکلوزیس ایجاد شده که برای درمان آن از آنتی بیوتیکهای ایزونیازید، ریفامپین، پیرازینامید و اتامبوتول استفاده میشود، با توجه به کاهش فعالیت این آنتی بیوتیکها و بروز مقاومت نسبت به آنها و افزایش سویه های MDR در سراسر جهان، نیاز به بررسی آنتی بیوتیک های خط دوم درمان سل مانند فلوروکوئینونها و درمانهای جایگزین احساس میشود. فلوروکوئینونها به دلیل خواص فارماکوکنتیک و فارماکودینامیک بالاتر، فعالیت گسترده و قوی بر علیه مایکوباکتریوم توبرکلوزیس و عوارض جانبی و سمیت کمتر مورد توجه هستند و در حال حاضر جهت درمان سل حساس به دارو مورد تایید سازمان بهداشت جهانی میباشند و استفاده از آنها باعث کوتاه شدن طول درمان و بهبود نتایج میشود. روش کار در مطالعه حاضر با مرور 170 مقاله به بررسی اثرات فلوروکوئینونها در درمان سل ریوی و MDR-TB پرداخته شد. نتایج فلوروکوئینونها تازه ترین کلاس و قوی ترین داروی مورد استفاده در خط دوم درمان بیماری سل میباشند، که علاوه بر موثر بودن در درمان MDR-TB، جایگزین مناسبی برای آنتی بیوتیک های خط اول در رژیم استاندارد محسوب میشوند. این گروه آنتی بیوتیکی شامل سیپروفلوکساسین، افلوکساسین، گاتی فلوکساسین، لوفلوکساسین، موکسی فلوکساسین و... میباشد، که از میان آنها موکسی فلوکساسین و لوفلوکساسین بیشترین فعالیت را داشته وفعالیت بالاتری نسبت به افلوکساسین دارند اما افلوکساسین ایمنی بالاتری دارد همچنین اسپارافلوکساسین نسبت به افلوکساسین و سیپروفلوکساسین فعالتر است و مقاومت آن نسبت به آنها کمتر میباشد. نتیجه گیری فلوروکوئینون ها، به عنوان آنتی بیوتیکی مناسب در درمان MDR-TB و همچنین جایگزین مناسبی برای آنتی بیوتیک های خط اول در رژیم استاندارد محسوب میشوند.
