ابعاد نظری و کاربردی تبعیت از درمان در بیماران: یک مطالعه مروری
مقدمه تبعیت از درمان یکی از مهمترین موضوعها در زمینه بیماریهای جسمی و روانی است که در ایران توجه کمی به آن شده است. بسیاری از بیماران پایبند درمان نیستند و دستورات پزشک را نادیده میگیرند که باعث مشکلات اقتصادی، جسمی و فرهنگی برای فرد، خانواده و جامعه میشود. هدف از انجام این پژوهش بررسی ابعاد نظری و کاربردی تبعیت از درمان در نظام مراقبت سلامت بود. روش کار روش پژوهش حاضر توصیفی تحلیلی است. بدین نحو که مقالات مرتبط با ادبیات نظری و پژوهشی مرتبط با پایبندی به درمان در پایگاههای اطلاعاتی Scholar Google ،Nursing index ،Elsevier،psychinfo Proquest،Scopus و کتابهای موجود در این زمینه جست جو شد. جستجوی مقالات در بازه زمانی از سال 1990 تا 2019 میلادی و با استفاده از کلید واژههای تبعیت از درمان و بیمار و نظریه و الگو به صورت ترکیبی انجام شد. نتایج نتایج پژوهشهای مختلف نشان داد که عدم تبعیت از درمان در بسیاری از اختلالات میتواند صدمات جبران ناپذیری را وارد نماید. مدل رفتاری اندرسون و مدل عقل سلیم از مدلهای موجود در حوزه تبعیت از درمان است که میتوانند استفادههای درمانی و کاربردی داشته باشند. کودکان، سالمندان و بیماران با مشکلات روانشناختی از جمله گروههایی هستند که میزان تبعیت از درمان در میان آنها پایین است. نتیجه گیری بر اساس نتایج حاصل، پزشکان و متخصصان باید با بکارگیری و اجرای مدلهای تبعیت از درمان به تقویت آن در بیماران کمک کنند، چرا که عدم توجه به این پدیده به خصوص در برخی از گروهها با پیامدهای منفی فردی و اجتماعی بسیاری همراه خواهد بود.

اثربخشی طرحواره درمانی بر انعطافپذیری شناختی و تابآوری بیماران مبتلا به اضطراب فراگیر
چکیدهمقدمه: این پژوهش با هدف تعیین اثربخشی طرحواره درمانی بر انعطافپذیری شناختی و تابآوری بیماران مبتلا به اضطراب فراگیر انجام گرفت.روش کار: روش پژوهش حاضر از نوع نیمه آزمایشی با طرح پیش آزمون – پس آزمون با گروه کنترل بود. جامعه آماری این پژوهش را تمامی افراد مبتلا به اضطراب فراگیر منقطه 1 تهران تشکیل دادند. از طریق فراخوان و مراجعه به کلینیکهای روانشناسی منطقه 1 شهر تهران، 40 نفر مبتلا به اضطراب فراگیر انتخاب و به صورت تصادفی در دو گروه آزمایش و کنترل جایگزین شدند؛ بدین ترتیب ابتدا پرسشنامههای انعطافپذیری شناختی و تابآوری به عنوان پیش آزمون اجرا شد سپس گروه آزمایش تحت طرحوارهدرمانی به مدت 10 جلسه قرار گرفتند و در پایان از هر دو گروه پسآزمون به عمل آمد و دادههای پژوهش با استفاده از کواریانس مورد تحلیل قرار گرفت.یافتهها: نتایج کواریانس نشان داد که طرحواره درمانی بر افزایش انعطافپذیری شناختی و تابآوری بیماران مبتلا اضطراب فراگیر تاثیر مثبت و معنی داری دارد (p≤0/001).نتیجهگیری: با استفاده از رویکرد طرحواره درمانی میتوان انعطافپذیری شناختی و تابآوری بیماران مبتلا به اضطراب فراگیر را افزایش داد.

بررسی ارتباط پلیمورفیسم گیرنده فعال کننده تکثیر پروکسی زوم آلفا و میزان کلسیفیکاسیون عروق کرونر در بیماران مبتلا به بیماری عروق کرونر
مقدمه مطالعات قبلی ارتباط میان پلیمورفیسم ژن PPARα و بیماریهای عروق کرونر (CAD)و آترواسکلروز به اثبات رسیده است و نشان داده شده که بیماریهای عروق کرونر به طور مستقیم با میزان کلسیفیه شدن عروق کرونری (CAC) ارتباط دارد. با این حال، ارتباط میان پلیمورفیسم ژن PPARα و CAC مورد بررسی قرار نگرفته است. هدف این پژوهش، بررسی ارتباط احتمالی بین پلیمورفیسم ژن PPARα و CAC در بیماران عروق کرونر بود. روش کار 90 بیمار عروق کرونر و ۹۰ نفر کنترل در این مطالعه ثبت نام کردهاند. آنژیوگرافی توموگرافی کامپیوتری (CT)برای همه موارد انجام شد و CAC کرونری چپ (left coronary)، عروق اصلی چپ (left main coronary)، عروق راست (right coronary) و سرخرگهای سیرکومفلکس (circumflex arteries) مورد بررسی قرار گرفت. پلیمورفیسم ژن PPARα توسط تکنیک PCR-RFLP مورد بررسی قرار گرفت و ارتباط احتمالی میان پلیمورفیسم PPARα و CAC در بیماران عروق کرونر و کنترلهای سالم مورد بررسی قرار گرفت. نتایج تفاوت قابل توجهی بین فراوانی آللها (G,C) یا ژنوتیپ (GG,GC) میان دو گروه به ترتیب (43/0=p؛ 9/28%=GC؛ vs 1/70 %=GG) ، (52/0=p ، 4/15 % =C .vs%6/84 =G ) وجود نداشت. اختلاف معناداری بین ژنوتیپ یا فراوانی اللها در بیماران یا کنترلهای سالم با میزان کلسیفیه شدن عروق کرونر وجود نداشت (41/0=p). نتیجه گیری با توجه به نتایج این مطالعه، ارتباط معناداری میان فراوانی آللها و ژنوتیپها با میزان CAC در بیمارانCAD وجود نداشت.

مقایسه تأثیر سه روش مکمل یاری دارچین، تمرین هوازی و ترکیب این دو (تمرین – مکمل) برگلوکز، انسولین ناشتای خون و مقاومت به انسولین مردان دیابتی نوع II
مقدمه تا کنون روشهای درمانی متعددی برای درمان بیماری دیابت پیشنهاد شدهاست، اما این که کدام یک از این روشها با توجه به شرایط بیمار، کارایی بیشتری دارد، هنوز به روشنی مشخص نشده است. لذا، هدف از پژوهش حاضر مقایسه تأثیر سه روش مصرف دارچین، تمرینات هوازی و ترکیب آنها بر گلوکز و انسولین ناشتای خون و مقاومت به انسولین مردان دیابتی نوع II بود. روش کار در این پژوهش که بهصورت پیش آزمون – پس آزمون با گروه کنترل انجام شد، 56 مرد دیابتی بهطور تصادفی به 4 گروه مصرف دارچین (14 نفر)، تمرین هوازی (14 نفر)، ترکیبی (14 نفر) و کنترل (14 نفر) تقسیم شدند. گروه دارچین روزانه 6 کپسول 500 میلیگرمی دارچین به مدت 8 هفته مصرف کردند، گروه تمرین هوازی هفتهای 3 جلسه به مدت35 تا 45 دقیقه و با شدت60 تا 70% ضربان قلب بیشینه در 8 هفته تمرین کردند، و گروه ترکیبی بهطور همزمان به تمرین هوازی و مصرف دارچین پرداختند. متغیرها در ابتدا و پس از 8 هفته مداخله اندازهگیری شد. مقایسه درونگروهی با آزمون t همبسته و مقایسه بینگروهی با تحلیل واریانس یکطرفه (ANOVA) انجام شد. نتایج در هر سه گروه مداخله؛ گلوکز، انسولین ناشتای خون و مقاومت به انسولین در مقایسه با گروه کنترل کاهش معناداری داشت (05/0>p). ولی در مقایسه بین گروهی تفاوت معناداری در کاهش گلوکز ناشتای خون و مقاومت به انسولین مشاهده نشد. نتیجه گیری مصرف دارچین، تمرین هوازی و روش ترکیبی میتواند روی کاهش گلوکز، سطح انسولین و مقاومت انسولینی اثرات مثبتی داشته باشد.

بررسی میزان آگاهی دستیاران گروه کودکان از مولفه های اقتصادی درمان بیماران در دانشگاه علوم پزشکی مشهد
مقدمه به کار گیری علم اقتصاد در برنامه ریزی برای هزینههای ارائه خدمات درمانی، جزء جدانشدنی سیاستهای نظام سلامت است. به منظور اجراء یک برنامه اقتصادی صحیح، ارائه دهندگان و دریافت کنندگان این خدمات باید اطلاعات کافی دربارهی مولفههای اقتصادی در این زمینه داشته باشند. هدف ما از این مطالعه، بررسی سطح آگاهی دستیاران گروه کودکان از جنبه های اقتصادی درمان میباشد. روش کار مطالعهی حاضر از نوع مقطعی میباشد و بر روی کلیهی دستیاران گروه کودکان دانشگاه علوم پزشکی مشهد در بهار 1394 با پرسشنامهای حاوی 19 سوال که روایی آن توسط اساتید گروه دانشگاه علوم پزشکی مشهد و پایایی آن توسط ضریب آلفای کرونباخ تایید شد، در حیطه اقتصاد درمان انجام گردید. نتایج در مورد درصد پرداخت بیمه برای خدمات بستری و سرپایی در هر حیطه درمانی حداکثر 10% پاسخ صحیح داشتند، همچنین 24% از این افراد در مورد هزینه شبتخت بخش عمومی و 2% آن ها مبلغ شبتخت بخش مراقبت ویژه را درست عنوان کردند. هیچ یک هزینهی صحیح ایمونوگلوبین وریدی را نمیدانستند. 5% از هزینهی صحیح یک نسخهی شایع اطلاع داشتند. در سوالات هزینهی خدمات، نمرات دستیاران محاسبه و از ضعیف تا عالی تقسیمبندی گردید که 93% ضعیف و 7% متوسط بودند. براین اساس میانگین نمرات دستیاران بر حسب سال دستیاری اختلاف معنیداری نداشت(441/0=p) . نتیجه گیری این مطالعه نشان میدهد که اطلاعات دستیاران از هزینههای مراقبت از سلامت ناکافی است واین مساله ارتباطی با سنوات تحصیلی ندارد. بنابراین پیشنهاد میشود مداخلاتی جهت ارتقاء سطح اگاهی دستیاران انجام شود.

بررسی اپیدمیولوژیک بروسلوز در شهرستان فسا طی سالهای 1388-1396
مقدمه
بروسلوز یکی از بیماریهای عفونی مشترک بین انسان ودام است که همچنان در کشور ما یک معضل بهداشتی محسوب میشود. با توجه به این که شهرستان فسا جزء مناطق با شیوع بالا این بیماری میباشد، مطالعه حاضر با هدف بررسی وضعیت اپیدمیولوژیک بروسلوز طی سالهای 96-1388 انجام شد.
روش کار
پژوهش حاضر به صورت مقطعی انجام شد. اطلاعات مربوط به بیماری بروسلوزیس از معاونت بهداشتی دانشگاه علوم پزشکی فسا اخذ شد. دادهها کد گذاری و جهت تجزیه و تحلیل وارد نرم افزار SPSS نسخه 22شدند. از شاخصهای فراوانی، درصد فراوانی، میانگین و انحراف معیار جهت تجزیه و تحلیل دادهها استفاده شد و نتایج در قالب جدول و نمودار ارایه شد.
نتایج
بیشترین فراوانی مربوط به سال 88 با 67 مورد (8/17%) وکمترین فراوانی مربوط به سال96 با 25 مورد (6/6%) بود. از این میان 343مورد (2/91%) بیماری جدید گزارش شد و بقیه شکست درمان و یا عود بیماری بودند. همه بیماران ایرانی و بیشتر آنها از گروه مردان بودند. میانگین سنی افراد مبتلا 31/8±15/34 سال بود. بیشترین شغل مرتبط با این بیماری دامداری وکشاورزی با فراوانی37%(139مورد) و پس از آن خانه داری با فراوانی9/26%(101مورد)بود. اغلب شرکت کنندگان ساکن روستا بودند.
نتیجه گیری
نتایج مطالعه نشان داد که روند بروز بیماری کاهشی بوده است. ولی این بیماری هنوز یک مشکل بهداشتی در شهرستان است که نیازمند آموزش و همکاری بین بخشی برای کنترل بیماری میباشد.

بررسی مداخله نوروفیدبک بر میزان توجه و تمرکز، مشکلات رفتاری و سازگاری هیجانی- اجتماعی کودکان دارای اختلال نارسایی توجه/ فزون جنبشی 6 تا 12 ساله
مقدمه: هدف مطالعه حاضر بررسی تاثیر مداخله نوروفیدبک بر میزان توجه و تمرکز، مشکلات رفتاری و سازگاری هیجانی- اجتماعی کودکان دارای اختلال نارسایی توجه/ فزون جنبشی بود.روش کار: طرح پژوهش شبه آزمایشی،در قالب پیش آزمون-پس آزمون با گروه گواه بود. جامعه آماری مورد مطالعه دراین پژوهش را، تمامی کودکان 6تا12ساله مبتلا به نارسایی توجه/فزون جنبشی و والدین آنها تشکیل دادند که در منطقه 2 شهر تهران که جهت درمان و راهنمایی های لازم در سال 2016- 2017 به مراکز مشاوره و خدمات روانشناختی مراجعه نمودند که نمونه ای به تعداد 40 نفر به صورت نمونه گیری هدفمند و داوطلبانه به شیوه گمارش تصادفی در دو گروه آزمایشی و گواه تخصیص داده شدند. تشخیص اختلال نارسایی توجه / فزون جنبشی با استفاده از فهرست بازبینی مصاحبه بالینی بر اساس راهنمای تشخیصی و آماری اختلالهای روانی، ویرایش پنجم،2013و مصاحبه تشخیصی توسط روانپزشک انجام شد و همچنین از آزمون عملکرد پیوسته رزولد و همکاران (1956)، سازگاری دانش آموزان سینها و سینگ (1993) و سیاهه رفتارکودک آخنباخ در مرحله پیش آزمون و پس آزمون استفاده شد. آموزش نوروفیدبک به تعداد 20 نفر در طول 20جلسه و هر هفته سه جلسه در گروه آزمایشی انجام شد. برای این افراد در این پژوهش از محاسبات آمار توصیفی و استنباطی تحلیل کوواریانس با استفاده از نرم افزار آماری SPSS استفاده شد.نتایج: یافته های بدست آمده نشان داد اثربخشی مداخله نوروفیدبک بر کاهش مشکلات کودکان ADHD در سه زمینه موثر بوده است (001/0>P).نتیجه گیری: بر اساس یافته های این پژوهش نوروفیدبک یک روش درمانی مناسب در درمان مشکلات توجه و کاهش علائم نارسایی توجه/فزون جنبشی در کودکان 6تا 12سالADHD است.

آلودگیهای قارچی در هوای اتاقهای عمل 13 بیمارستان در مشهد
مقدمه
اسپورهای قارچی هوا بُرد منابع آلودگی مهمی در اتاق عمل و بخشهای مراقبت ویژه بیمارستانها میباشند. هدف از این مطالعه، بررسی میزان و تنوع آلودگیهای قارچی فرصت طلب در هوای اتاقهای عمل عمل بیمارستانهای مشهد بوده است.
روش کار
این مطالعه مقطعی در13 بیمارستان مشهد که اجازه مطالعه را به گروه تحقیق دادند درسالهای 97-1396 انجام شد. ازهوای تمام 104 اتاق عمل موجود در 13 بیمارستان به روش پلیت باز نمونه برداری به عمل آمد. پس از 7-10 روز کلنیهای رشد کرده قارچهای رشتهای توسط روشهای متداول و ملکولی تعیین توالی شناسایی شدند. در این مقاله با توجه به اهداف مجله دانشکده پزشکی بیشتر به جنبههای عمومی و بیمارستانی و نتایج روشهای متداول پرداخته شده است. نتایج مر بوط به آزمایشات ملکولی و تعیین توالی در مقاله دیگری ارائه خواهد شد. نتایج با استفاده از نرم افزار spss نسخه 16تجزیه و تحلیل شد .
نتایج
در این مطالعه کلنیهای قارچی رشد کرده در 403 پلیت شمارش و شناسایی گردیدند. گونههای آسپرژیلوس و پنی سیلیوم، بیشترین میزان آلودگی و آلترناریا کمترین میزان آلودگی را در اتاقهای عمل داشتند. آسپرژیلوس فلاووس شایعترین گونه آسپرژیلوس در بین سایر قارچها بود. بیمارستانهای شماره 3 و11 به ترتیب دارای بیشترین و کم ترین آلودگی قارچی بودند.
نتیجه گیری
آلودگی با قارچهای هوا بُرد در اتاقهای عمل بیمارستانهای مشهد وجود دارند که میتواند برای بیماران نقص ایمنی یک عامل خطر باشد آسپرژیلوس فلاووسگونۀ غالب در بین قارچهای فرصت طلب موجود در هوای اتاقهای عمل میباشد. با توجه به اینکه اکثر قارچها با روشهای ضد عفونی مقاوم هستند، تحقیق بیشتری برای یافتن روش مناسب جهت کم کردن بار آلودگی قارچی در هوای اتاق عمل توصیه میشود.

انطباق و بررسی روایی تجربه دوره کوتاهمدت آموزش کارکردهای اجرایی (استپ) در مبتلایان به اختلال اضطراب: تکآزمودنی
مقدمهاختلال اضطراب، اختلالی جدی است که دیگر فرایندهای روانشناختی، ارتباطی، اجتماعی و هیجانی افراد را نیز با آسیب مواجه میسازد. بر همین اساس پژوهش حاضر با هدف انطباق و بررسی روایی تجربه دوره کوتاهمدت آموزش کارکردهای اجرایی (STEP) در مبتلایان به اختلال اضطراب انجام گرفت.روش کاردر این پژوهش از روش پژوهش موردمنفرد با طرح A-B استفاده شد. چهار فرد 23-19 سال بر اساس روش نمونهگیری هدفمند انتخاب شدند. شرکتکنندگان پژوهش قبل از اجرای مداخله به پرسشنامه اضطراب حالت- صفت (اسپیلبرگر، 1970)، آزمون کارکردهای اجرایی ویسکانسین (رینولدس و هارتون، 2008) و پرسشنامه تنظیم هیجانی (گروس و جان، 2003) به عنوان ابزارهای پژوهش پاسخ دادند. پس از اندازهگیری خط پایه، مداخله آموزشی کارکردهای اجرایی استپ (کانتور و همکاران، 2014) در قالب 12 جلسه با محتوای حل مسئله، تنظیم هیجان و توجه به چرخه ایجاد هیجان، اجرا و پس از آن مرحله پسآزمون اجرا شد. یافتههای این پژوهش بر اساس تحلیل دیداری و شاخصهای آمار توصیفی مورد تجزیه و تحلیل قرار گرفت.نتایجطی تحلیل دیداری نمودار دادهها، آموزش کارکردهای اجراییدر مورد هر چهار آزمودنی اثربخش بود. بدین صورت که این درمان توانسته منجر به کاهش اضطراب حالت-صفت و بهبود کارکردهای اجرایی و تنظیم هیجانی افراد مبتلا به اختلال اضطراب شود.نتیجه گیری نتایج حاصل از پژوهش حاکی از احتمال سودمندی آموزش کارکردهای اجراییبر اضطراب حالت-صفت، تنظیم هیجانی و کارکردهای اجرایی افراد مبتلا به اختلال اضطراب بود که مبتنی بر این یافته، میتوان از این مداخله در جهت کاهش اضطراب حالت-صفت و بهبود تنظیم هیجانی و کارکردهای اجرایی افراد مبتلا به اختلال اضطراب بهره برد.
