بررسی رابطه کوتاهی قد و آ ندوکرینوپاتیها در بیماران تالاسمی ماژور

چکیده

مقدمه تالاسمی ماژور، کم خونی ارثی وابسته به تزریق خون می باشد. اضافه بار آهن آن منجر به اختلال عملکرد ارگان ها آو ندوکرینوپاتی می شود. لذا درمان شلاتور با دفروکسامین صورت می پذیرد. کوتاهی قد این بیماران به دلیل آندوکرینوپاتی، پذیرش ضعیف شلاتور و مسمومیت با دفروکسامین شایع است. لذا هدف این مطالعه بررسی وضعیت رشدی این بیماران، ارتباط آن با آندوکرینوپاتی و تعیین نقش اضافه بار ثانویه آهن در این اختلالات است . روش کار این مطالعه توصیفی مقطعی در سال 1389 بر 280 بیمار تا 56 5/3 سال تحت درمان با تزریق خون و دسفرال، در مرکز تالاسمی امیرکلا صورت گرفت. بعد از ارزیابی اولیه و تعیین SDS قد، بیماران بر اساس فریتین سرم به دو گروه مطلوب و غیرمطلوب تقسیم شدند . بررسی از نظر هیپوتیروئیدی، دیابت، هیپوپاراتیروئیدیسم و هیپوگنادیسم انجام پذیرفت. سپس اطلاعات مورد تجزیه و تحلیل قرار گرفت. p< /0 05 معنی دار تلقی شد . از نرم افزار SPSS و آزمون آماری مجذور کای دو برای بررسی داده ها استفاده شد . نتایج از280 بیمار با میانگین 19 6/ ± 5/8 سال، 161 نفر /57 )5 ) % مونث بوده اند. کوتاهی قد (2- SDS >) در 90 نفر /32 )1%) وجود داشت. میانگین قد بیماران 151 4/ ±17 3/ cmبود. 162 نفر /57 )9%) آندوکرینوپاتی داشتند. بین کوتاهی قد باهیپوپاراتیروئیدیسم (p= /0 009)، دیابت (p= /0 003) و هیپوگنادیسم (p= /0 005) ارتباط معنی دار وجود داشت. ولی بین کوتاهی قد با هیپوتیروئیدی (p= /1 000) و سطح فریتین (p= /0 091) ارتباط معنی داری وجود نداشت . نتیجه گیری با توجه به شیوع بالاتر هیپوپاراتیروئیدیسم، هیپوگنادیسم و دیابت در بیماران تالاسمی کوتاه قد، توصیه می شود غربالگری با حساسیت بیشتری از نظر این اختلالات در بیماران مذکور، انجام شود

Introduction
Thalassemia major is a hereditary anemia that becomes blood transfusion dependent. Iron overload consequence leads to multiple organ dysfunction and endocrinopathy. So deferoxamine (desferal) is used for chelation therapy. The aim of this study was to evaluate the growth rate of these patients and its correlation with endocrine complications and determine the role of secondary iron overload in these situations.
Materials and Methods
This cross-sectional study was performed for a year (2010) on 280 patients (aged 3.5- 56 years) who received blood transfusion and chelation therapy at Amirkola Thalassemia Center. After basic evaluations, determination of height standard deviation, they were divided by serum ferritin level into two groups (good and poorly controlled).
