اثرات موضعی اکسید روی بر زخم های ناشی از جراحی سینوس پیلونیدال

چکیده

مقدمه
سینوس پیلونیدال عارضه شایعی در افراد جوان است که عموما در پوست ناحیه ساکروکوکسیژآل، بالای آنوس
ودر خطوسط دیده میشود و درمان آن انجام جراحی میباشد. باتوجه به طولانی بودن مدت درمان این بیماران و
نیز وجود اطلاعاتی مبنیبر مفیدبودن ترکیبات روی دربهبود زخم به بررسی اثر درمانی پماد اکسیدروی در ترمیم
زخمهای حاد ناشی از سینوسپیلونیدال در مقایسه با دارونما پرداخته شد.
روش کار
در یک مطالعه کار آزمایی بالینی دوسوکور تصادفی شده با کنترل دارونما 16 بیمار مرد مبتلا به سینوس پیلونیـدال
از سال 1387-1386 در بیمارستان امیرالمومنین سمنان مورد بررسی قرار گرفتند. این بیماران کـه مبـتلا بـه دیابـت،
نارساییکلیه وسیروز نبوده و یا داروهای موثربر بهبودی زخم مثل کورتیکواستروییدها را مصرف نمی کردند، پس
از جراحی بهروش خارجنمودن کامل کیست با انجام پانسمان روزانه پماد اکسیدروی و دارونما در دو گروه هدف
و شاهد مورد مطالعه قرارگرفتند. هر 15روز با اندازهگیری حجم زخم، میانگین درصد بهبودی در دو گروه مقابسه
گردید. اطلاعات با استفاده از آزمون های اسمیرنوف و تی تست و نرم افزار SPSS تحلیل شد.
نتایج
در این مطالعه میانگین سـنی 24/5±4 سـال بود،کـه بـین دوگـروه تفـاوت معنـی داری وجودنداشـت (p=0/634).
میانگین درصد تغییرات حجم در 15 روز اول و دوم بعد از عمل جراحی در بین دو گروه تفاوت معنی دار نداشت
(p= 0/334) و (p=0/175). اما میانگین درصد تغییرات حجم در 15 روز سوم و همچنین در کـل 45 روز درمـان
بین دو گروه تفاوت معنیدار داشت (p=0/006) و (p=0/026).
نتیجهگیری
باتوجه به یافتههای این مطالعه و نیز مطالعات مختلف انجامشده، پانسمان بـا پمـاد اکـسیدروی درافـزایش سـرعت
بهبود زخمهای مزمن و حاد مفید است. اثر دارو در 15روز سوم بیشتر بود که احتمالا بـه دلیـل اثـربیـشتر روی در
مرحله اپیتلیالیزهشدن زخم است.

چکیده [English]

Introduction
Pilonidal sinuses typically occur in the midline of the sacrococcygeal skin of young men that is treatment is surgical management. Since its wound healing typically requires long time and information about benefits of zinc compounds in wound healing, we asked to evaluate of the effect of topical zinc oxide ointment on healing of the acute open wounds due to surgery of pilonidal sinus.
Materials and Methods
In double blind, randomized, placebo control study sixteen men were not been diabetic, renal failure or cirrhotic and not used corticosteroids, after surgery with complete simple excision approach, evaluated in two groups that have been dressed daily with topical zinc oxide ointment (case group) and placebo (control group). Every 15 days average of percentage of wound healing has been compared in two groups by measuring of volume of wounds.
Results
Mean of age in patients was 24.5±4, has no significant difference in groups (p= 0.634). Means of percentage of volume changes in first and second 15 days have no significant difference in groups (p= 0.334), (p= 0.175). But means of percentage of volume changes in third 15 days and in total 45 days duration of treatment have significant difference (p= 0.006), (p= 0.026).
Conclusion
In attention to this study and other searches, dressing with zinc oxide ointment is benefit in increase of healing rate of chronic and acute wounds. The effect of drug is significant in third 15 days that is properly because of principle effect of zinc in epithelialization phase.
