چکیده

مقدمه: تومورهای جدار قفسه سینه جزء بدخیمیهای ناشایع هستند و از مدتها قبل مورد بحث جراحان بوده است. تا همین اواخر، تشخیص نادرست، رزکسیون
ناکامل یا ناتوانی در بازسازی موفق نقایص بزرگ جدار قفسه سینه از علل عمدة مرگ و میر بیماران و عود موضعی بوده است.
ا جراحی اصولی و علمی بهترین شانس درمان را برای بیماران با تومورهای جدار قفسه سینه فراهم میکندو شیوه جراحی باید براساس ویژگیهای بیماری
هرفرد پایهریزی شود. این مطالعه با هدف بررسی نتایج درمان جراحی تومورهای جدار قفسه سینه و گزارش موارد عمل شده، بوده است.
روش کار: این مطالعه توصیفی در سالهای 1375 - 83 در بیماران بستری در دو بخش جراحی توراکس بیمارستان ولیعصر (عج) دانشگاه علوم پزشکی تهران
و بیمارستان قائم (عج) دانشگاه علوم پزشکی مشهد، انجام شده است. تعداد 61 بیمار مورد ارزیابی و درمان قرار گرفتند. مشخصات فردی، شکایات بالینی، نتایج
آزمایشات خون و درمان و عوارض در پرسشنامه جمع آوری شده با استفاده از آمار توصیفی و جداول توزیع فراوانی پردازش شد.
نتایج: از 61 بیمار، 28 بیمار (45/9%) انواع تومورهای بدخیم اولیه جدار قفسه سینه، 4 بیمار (6/55%) تومور متاستاتیک به جدار سینه،10 بیمار (16/39%) تومور
خوشخیم نسج نرم و دندهها، 16 بیمار (26/22%) ضایعات التهابی غضروف و دندهها با نمای اولیه تومورال و 3 مورد 4/9% هم پاتولوژی مشخص نشده (متفرقه)
بوده است.
نتیجهگیری: نتایج حاصله از بیماران عمل شده در این گزارش، تائید میکند که رزکسیون وسیع جدار قفسه سینه برای تومورهای اولیه یا ثانویه و به تبع آن
بازسازی، یک درمان مؤثرو ایمن میباشد. دستیابی به عمر طولانی پس از جراحی امیدوارکننده است.

Introduction: Chest wall tumor is an uncommon malignancy; and incomplete diagnosis and resection and inability to reconstruction of expanded chest wall defect after resection, Causes mortality and morbidity in patients. Academic surgery provides the best choice for the patients, and surgery technique should be based on the individual characteristics. This study was done to evaluate the surgery results of chest wall tumors and to report the operated cases.
Material and Methodes: The descriptive research was performed in Tehran Valiasr Hospital and Ghaem Hospital (Mashhad University of Medical Sciences) from 1995 to 2003 (9 years) and 61 patients have been evaluated in retrospective study. Individual, clinical, laboratory, treatment and complication data were gathered in a questionnaire and analyzed by descriptive statistics.
Resultes: 28 (45.9%) patients had primary chest wall malignancy, 4 (6.55%) patients had metastatic tumors, 10 (16.39%) patients had benign tumors, 16 (26.22%) patients had inflammatory disease and 3 (4.9%) had undiagnosed pattern.
Conclusion: Results of our study and also review of other articles recommended that wide enblock exiscion of chest wall tumors with appropriate reconstruction is the best method of treatment.
