تجربه و علل پزشکی تدافعی: یک مطالعه توصیفی مقطعی از دیدگاه متخصصین

چکیده

چکیده
مقدمه: پزشکی تدافعی به هر معاینه پزشکی یا درمانی گفته می‌شود که در درجه اول برای محافظت از پزشک در برابر اتهامات سهل انگاری یا شکایاتی که تحت عنوان خطاهای پزشکی می باشند، انجام می‌گیرد. هدف از این پژوهش، بررسی تجربه و علل پزشکی تدافعی از دیدگاه متخصصین دانشگاه علوم پزشکی جهرم می باشد. 
روش: این مطالعه توصیفی- مقطعی در بین 25 نفر از متخصصین بالینی دانشگاه علوم پزشکی جهرم در سال 1397 انجام گردیده است. ابزار جمع آوری اطلاعات پرسشنامه می باشد. داده‌های پرسشنامه بعداز گردآوری در نرم افزار SPSS نسخه 21 ثبت گردید و با استفاده شاخص های آماری توصیفی و روش آماراستباطی، آزمون فریدمن که سطح معنی داری 05/0 درنظر گرفته شد، تحلیل گردید.
یافته‌ها: متخصصین حاضر در این تحقیق دارای نمره پزشکی تدافعی با میانگین 6.87±42.20 بودند. طبق نتایج حاصل، میزان پزشکی تدافعی در زنان نسبت به مردان، در پزشکان متاهل نسبت به پزشکان مجرد، در پزشکان فاقد بیمه مسئولیت نسبت به پزشکان دارای بیمه مسئولیت بیشتر بود اما هیچ یک از این تفاوتها به لحاظ آماری معنی دار نبودند (p>0.05).
نتیجه گیری: نتایج نشان می‌دهد که پزشکی تدافعی در پزشکان دانشگاه علوم پزشکی جهرم نسبتاً بالا است، که مهم ترین علت این پدیده، نداشتن ریسک پذیری و اعتماد به نفس پایین پزشکان می‌باشد. در این راستا پیشنهاد می شود آموزش های مرتبط با رفع این پدیده و هدایت به سمت ابعاد مثبت آن به پزشکان داده شود و این در حالی است که با اقدامات کاملاً ساده این پدیده کاهش می یابد.

Abstract
Introduction: Defensive medicine is any medical or medical examination that is primarily performed to protect the physician against negligent allegations or complaints referred to as medical errors. The purpose of this study was to investigate the experience and causes of defensive medicine from the viewpoint of Jahrom University of Medical Sciences.
Method: This descriptive cross-sectional study was performed among 25 clinical specialists of Jahrom University of Medical Sciences in 1977. Data collection tool is a questionnaire. Questionnaire data were collected in SPSS software version 21 and analyzed by descriptive statistics and descriptive statistics, Friedman test with a significance level of 0.05.
Results: The specialists in this study had a defensive medical score of 42.20 میانگین 6.87. According to the results, the rate of defensive medicine in women was higher than that of men, in married physicians than single physicians, in physicians with no liability insurance, but none of these differences were statistically significant (p> 0.05).
Conclusion: The results show that defensive medicine is relatively high among physicians of Jahrom University of Medical Sciences. The most important cause of this phenomenon is lack of risk taking and low self-confidence of physicians. In this regard, it is recommended that physicians be educated on how to tackle this phenomenon and guide it to its positive dimensions, while reducing it by very simple measures.
