مسمومیت خانوادگی با مونواکسید کربن : گزارش موارد

چکیده

مقدمه: یکی از علل کاهش سطح هوشیاری که همیشه باید مد نظر باشد ، مسمومیت هاست. مونواکسید کرین گازی است بدون بو، رنگ و مزه که به شدت سمی است. اگر به فکر این مسمومیت نباشیم ، تشخیصش دشوار است و ممکن است با سایر مسمومیت ها اشتباه شود.
معرفی بیمار: اورژانس پیش بیمارستانی پس از اعلام ماموریت ، سه بیمار را در محل های مختلف و در وضعیت های مختلف در منزل یافته اند. مادر خانواده در کنار بخاری با تنفس های بریده بریده و سیانوتیک در حالی که سه قوطی کنسرو خالی در کنارش بوده است، یافت شده است. پدر خانواده را گیج در کنار توالت یافت شده بود. دختر 15 ساله خانواده را فاقد علائم بالینی در کنار مادرش پیدا کرده بودند. در اتوپسی دختر خانواده علت مرگ مسمومیت با مونواکسیدکربن با سطح کربوکسی هموگلوبین 50 گزارش شد. مسمومیت اتفاقی با مونو اکسید کربن در مقالات متعددی گزارش شده است، با توجه به علائم گمراه کننده بیماری در 3/1موارد ممکن است بیماری تشخیص داده نشود. تشخیص قطعی بیماری با اندازه گیری سطح کربوکسی هموگلوبین خون می باشد که در افراد غیر سیگاری سطح 2-3% و در افراد سیگاری سطح بالای 9% مطرح کننده مسمومیت با مونواکسید کربن است.
نتیجه گیری: مسمومیت با مونواکسید کربن شرایطی است که می تواند برای بیمار کشند ه باشد، این مسمومیت ممکن است تشخیص داده نشود و یا اشتباه تشخیص داده شود. در ابتدای شروع فصل سرد حتما باید به فکر این تشخیص باشیم.

Introduction: Poisoning is one of the causes of loss l of consciousness that should always be considered .This gas is a no-smelling, color and taste that is highly toxic. If we do not think of this poisoning, the diagnosis is difficult and may be mistaken.
Case reports: Emergency reported when arrived to patient’s home, found 3 patients in different areas. The family's mother, along with the stove, while cyanotic and having pulse and also found 3 canned fish near her .The father was found next to the toil while he was confused. The 15-year-old daughter was found along with her moth without pulse. The family girl autopsy have shown carboxy- hemoglobin level 50. Accidental poisoning with carbon monoxide has been reported in several articles. Due to the variable symptoms lead to misdiagnosis in 1/3 cases. The diagnosis is made by carboxy- hemoglobin measurement, in non- smoker patients level of carboxy- hemoglobin level of 2-3% and greater than 9% in smoker patients.
Conclusion: Carbon monoxide poisoning is a lethal condition, such poisoning may be miss-diagnosed or not diagnosis. At the beginning of the cold season, we must think about this diagnosis.
