بررسی تغییرات توده های کلیه در فاصله زمانی 15 ساله

چکیده

هدف:سرطان کلیه فراوانی 3 درصدی میان کل سرطانها داشته و همچنان رو به افزایش است. در این مطالعه به بررسی شیوع، stage و نوع تومورهای کلیه طی 15 سال در مشهد پرداختیم.
روش :
پژوهش حاضر یک مطالعه مقطعی میباشد که در آن پاتولوژی های بیماران نفرکتومی شده در سالهای (1380-1381) و (1395-1396) در بیمارستان امام رضا (ع) مشهد به دنبال تشخیص تومور، مورد بررسی قرار گرفت. کلیه پرونده های بیماران در دو بازه زمانی ذکر شده بررسی گردید و اطلاعات دموگرافیک و اطلاعات مربوط به توده، شامل تعداد، سایز و نوع پاتولوژی نیز جمع آوری شد.
یافته ها:
میان 145 بیمار مورد مطالعه، 45 (31%) مورد درسال های 80-81 ثبت شده بودند و 100 (69%) مورد درسال های 95-96 ثبت شده بودند. 71% موارد جدید و 55.56% موارد قدیمی مرد بودند و در هردو گروه، اکثر بیماران در Grade 2 بودند. تفاوت معنی داری در توزیع فراوانی متغیرهای جنسیت (P=0.07)، Grade بیماری (P=0.75)، محل توده (P=0.24)، نوع رزکشن (P=0.89)، تهاجم لنفوواسکولار (P=0.74) و خارج کپسولی (P=0.84) بین دو گروه وجود نداشت. تشخیص نهایی 95% موارد جدید و 84.4% موارد قدیمی کارسینوم سلول کلیوی (RCC) بود و تفاوت معنی داری در توزیع فراوانی تشخیص نهایی بین موارد قدیمی و جدید نبود (P=0.02).
نتیجه گیری:
این مطالعه، الگوی کلی و افزایش میزان بروز کانسر سلول کلیوی (RCC) در مشهد را نشان می دهد که نیاز به مطالعات بیشتری جهت بررسی اتیولوژی و فاکتورهای خطر بیماری و حتی المقدور برطرف کردن آنها می باشد.

چکیده [English]

Objective: kidney cancer has a frequency of 3% in all cancers and this frequence has increased recently. We tried to investigate the prevalence, stage and type of renal tumors within 15 years in Mashhad.
 
Methods & Materials: In this cross-sectional study, the pathologie of patients undergone nephrectomy due to tumor in previous (2001-2002) and recent(2016-2017)years in Imam Reza Hospital in Mashhad, were examined. All records of both groups were reviewed including demographic information and  mass informations such as number, size and type of pathology.
 
Results: Among 145 patients, 45 (31%) were prevoiusly registered in 2001-2002 and 100 (69%) were recently registered in 2016-2017. 71% of new cases and 55.56% of prevoius cases were male and both groups were in second group. There was no significant differences in the frequency distribution of gender (P = 0.07), disease grade (P = 0.75), mass location (P = 0.24), resection type (P = 0.89), lymphovascular invasion (P = 0.74) and extracapsular (P = 0.84) between old and new cases. The final diagnosis in 95% of new cases and 84.4% of old cases was renal cell carcinoma (RCC) and there was a significant difference in the frequency distribution of the final diagnosis between old and new cases (P = 0.02).
 
Conclusion:
This study shows the general pattern and increasing incidence of RCC in Mashhad, which requires further studies to investigate the etiology and risk factors and even eliminate them if possible.
