مقایسه اثربخشی درمان شناختی رفتاری و مصاحبه انگیزشی بر سازگاری روانی اجتماعی، هورمون کورتیزول و تنظیم شناختی هیجان در بیماران عروق کرونر قلب

چکیده

مقدمه: بیماری قلبی عروقی بیماری جدی، مزمن و تهدید کننده حیات در جهان محسوب می شود. به همین دلیل این پژوهش با هدف بررسی اثربخشی درمان شناختی رفتاری و مصاحبه انگیزشی بر سازگاری روانی اجتماعی، میزان هورمون کورتیزول و تنظیم شناختی هیجان در بیماران عروق کرونر قلبی طرح ریزی شده است.
روش کار: بدین منظور از جامعه زنان 30 تا 50 ساله مبتلا به بیماری قلبی-عروقی مراجعه کننده به مرکز درمانی شهید رجایی شهر تهران تعداد 45 نفر به شیوه ی در دسترس انتخاب شدند و به طور تصادفی در گروه های آزمایشی و کنترل (هرگروه 15 نفر) قرار گرفتند. متغیرهای وابسته در دو نوبت پیش آزمون و پس آزمون با استفاده از مقیاس سازگاری روانی اجتماعی ، کیت کورتیزول بزاقی شرکت IBL آلمان و پرسشنامه تنظیم شناختی هیجان مورد اندازه گیری قرار گرفتند.
نتایج: تحلیل واریانس اندازه گیری مکرر نشان داد که هر دو گروه مداخله ی شناختی رفتاری و مصاحبه ی انگیزشی بر بهبود سازگاری روانی اجتماعی، کاهش هورمون کورتیزول در بیماران عروق کرونر قلب و بهبود تنظیم شناختی هیجان نسبت به گروه کنترل در هر دو مرحله ی پس ازمون و پیگیری اثربخشی معناداری داشته اند.
نتیجه گیری: هر دو درمان به یک اندازه بر بهبود سازگاری روانی اجتماعی بیماران موثر بوده اند اما مصاحبه ی انگیزشی در کاهش کورتیزول نسبت به درمان شناختی رفتاری موثر تر بود و درمان شناختی رفتاری نسبت به مصاحبه ی انگیزشی در بهبود تنظیم شناختی هیجان اثربخشی معنادارتری داشت.

چکیده [English]

Introduction: Cardiovascular disease is a serious, chronic and life-threatening disease in the world. Therefore, this study is designed to evaluate the effectiveness of cognitive-behavioral therapy and motivational interviews on social psychological adjustment, cortisol levels and cognitive emotion regulation in coronary heart disease patients.
Methods: For this purpose, a community of women aged 30 to 50 with cardiovascular disease referred to Shahid Rajaei Medical Center in Tehran, 45 people were selected in an accessible way and randomly in experimental and control groups (each group 15 People) were placed. The dependent variables were measured twice before and after the test using the Social Psychological Adjustment Scale, the IBL salivary cortisol kit, and the Cognitive Emotion Regulation Questionnaire.
Results: Frequent measurement analysis of variance showed that both cognitive-behavioral intervention and motivational interview groups improved psychosocial adjustment, decreased cortisol in patients with coronary heart disease, and improved cognitive emotion regulation compared to the control group in both postoperative stages. Examination and follow-up have been significant.
Conclusion: Both treatments were equally effective in improving patients' psychosocial adjustment, but motivational interviews were more effective in reducing cortisol than cognitive-behavioral therapy, and cognitive-behavioral therapy was more effective than motivational interviews in improving cognitive emotion regulation.
