بررسی الگوی مسمومیت حاد در شمال شرق ایران

چکیده

 مقدمه: مسئولین مراقبت های بهداشتی با شناسایی الگوی منطقه ای مسمومیت ها  قادر به برنامه ریزی صحیح  و متمرکز بر پیشگیری و مدیریت بهینه پادزهرها می گردند. لذا این مقاله با هدف بررسی الگوی مسمومیت حاد در شمال شرق ایران انجام شد.
روش کار: در این مطالعه مقطعی اطلاعات تمامی بیمارانی که به علت مسمومیت حاد به بخش اورژانس بیمارستان موسی بن جعفر شهرستان قوچان از ابتدای دی‌ماه 1396 الی پایان آذر 1397  مراجعه نموده بودند، واردمطالعه گردید. اطلاعات با استفاده از چک­لیست به صورت مصاحبه با بیمار یا همراه و بررسی پرونده تکمیل شد.
  نتایج: 222 بیمار در مطالعه شرکت کردند. میانگین سنی افراد 15.7±28.9 سال، اکثرا در گروه سنی 11 تا 30 سال (3/60 درصد)، نیمی مونث (50 درصد)، متاهل (55 درصد)، دیپلمه  (33.7درصد)، بیکار (26.1 درصد) و ساکن شهر (68درصد) بودند.  اکثر مسمومیت ها از نوع عمدی یا  به قصد خودکشی (6/44 درصد) بود، پس از آن  مصرف بیش از حد (1/31%) و مصرف تصادفی (3/24%) گزارش گردید. شایع­ترین عامل مسمومیت، داروها (60.4 درصد)، اپیوم(تریاک و شیره) (12.1 درصد) و آفت­کش­ها یا سموم ارگانوفسفره (8.5 درصد) بود. بنزودیازپین­ها شایع­ترین (38.8 درصد) داروی مصرفی در کلیه رده­های سنی بود.
نتیجه­گیری: بــا توجــه بــه فراوانــی موارد مسمومیت عمدی (خودکشی) انجام مشاوره روان‌پزشکی در مسمومین ضروریست.

چکیده [English]

 Introduction: Health care authorities are able to arrange proper planning on prevention programs and optimized management of antidote stockpiles by identifying regional pattern of poisoning. So this study was designed to evaluate the pattern of acute poisoning cases treated in a tertiary care hospital in north- eastern Iran.
Methods. This cross sectional study was conducted at emergency department of Mosabn-Jafar hospital in Quchan during December 2017 to the end of November 2018. All cases of poisoning admitted to the hospital were included in this study. The patients’ data were obtained from medical records and were documented on a pre-structured proforma.
 Results: A total of 222 cases of acute poisoning were studied. The average of people age was 28.9+15.7 year old, most of the patients aged 11 to 30 years (60.3%) ,(50%) were  women, most of the patients reside in urban areas (68%), half of them were married (55%), diploma (33.7%) and unemployed (26.1%). Intentional poisoning or suicidal was the main cause of poisoning (44.6%), overload (31.1%) and Accidental poisoning was recorded for (24.3%) of cases. Pharmaceutical medication (60.4%), opioids (12.1%) followed by pesticides (8.5%) were the most common exposures. The most common drug used in all age was Benzodiazepine (38.8%).
 
Conclusion: Attempted suicide was the most common route of poisoning which necessitates attention
to psychiatric counseling.
