شیوع توکسوکاریازیس و ریسک فاکتورهای موثر در بیماران همودیالیزی و دیابتی در زاهدان

چکیده

مقدمه: توکسوکاریازیس انسانی یک بیماری انگلی است که توسط لارو توکسوکارا کنیس یا کتی ایجاد میشود و از نظر بالینی میتواند عوارض متعدد بالینی در کبد، ریه، چشم، مغز وکلیه ایجاد کند این انگل علیرغم تاثیر قابل توجه برای گروه های پرخطر(افراد با کاهش سیستم ایمنی) معمولا نادیده گرفته میشود مطالعه حاضر با هدف بررسی شیوع سرمی و عوامل خطر توکسوکاریازیس در بیماران دیابت و افراد تحت همودیالیز در زاهدان انجام شد.
روش کار: این مطالعه در استان سیستان و بلوچستان، شهر زاهدان و 400 بیمار دیابتی و همودیالیزی انجام شد. یک نمونه لخته جهت انجام تست الایزا و یک پرسشنامه حاوی اطلاعات دموگرافیک اخذ گردید، تمام نمونه های سرم برای حضور آنتی بادی های IgG علیه گونه های توکسوکارا با روش سرولوژی الیزا مورد آزمایش قرار گرفتند. تجزیه و تحلیل داده های با استفاده از نرم افزار SPSS نسخه 22 و آمار توصیفی و آزمون chi-square انجام شد
نتایج: شیوع سرمی توکسوکاریازیس در بیماران دیابتی و همودیالیزی به ترتیب 8/2% و 5/3% گزارش شد در بین متغییرهای مطالعه در گروه بیماران دیابتی تماس با سگ و گربه ( 06/48 – 80/295% CI: 60/11 OR) و درگروه بیماران همودیالیزی تماس با سگ و گربه (02/1 – 70/0 95% CI: 85/0 OR) و گروه خونی ارتباط معنی داری با شیوع سرمی توکسوکاریازیس بدست امد .P value≤0.05
نتیجه گیری: با توجه به مطالعات اندک در دنیا ،مطالعات اپیدمیولوژیک بیشتری برای درک این مطلب که دیابت و فرایند همودیالیز میتواند به عنوان فاکتور در توکسوکاریازیس معرفی شوند نیاز است

چکیده [English]

Introduction: Human toxocariasis is a parasitic disease caused by the larvae of Toxocara canis (T. canis) and can cause many clinical complications in the liver, lungs, eyes, brain and kidney. Despite its significant effect on high-risk groups (people with reduced immune system), this parasite is usually ignored. The present study was conducted with the aim of investigating the serum prevalence and risk factors of toxocariasis in diabetic patients and hemodialysis patients in Zahedan (southeast of Iran).
Methods: This study was conducted in Sistan and Baluchistan province, Zahedan city with 400 diabetic and hemodialysis patients. A clot sample was taken to perform the ELISA test along with a questionnaire containing demographic information. All serum samples were tested for the presence of IgG antibodies against Toxocara species by ELISA serology method. Data analysis was done using SPSS version 22 software and descriptive statistics and chi-square test
Results: The prevalence of serum toxocariasis in diabetic and hemodialysis patients was reported as 2.8% and 3.5%, respectively. Among the variables of the study, contact with dogs and cats in the group of diabetic patients (OR=11.60 –% 95CI: 2.80-48.06). and in the group of hemodialysis patients contact with cats and dogs (OR=0.85- %95CI: 0.70-1.02) and blood group had a significant relationship with the prevalence of toxocariasis. P value ≤ 0.05.
Conclusion: Considering the few studies in the world, more epidemiological studies are needed to understand that diabetes and the hemodialysis process can be introduced as a factor in toxocariasis.
