اثربخشی طرحواره درمانی ذهنیت‌مداربر خوردن هیجانی و شاخص تودۀ بدنی زنان چاق دارای اختلال پرخوری

چکیده

مقدمه: مشـکلات هیجانـی و اسـترس ناشـی از آن می‌توانـد به‌عنــوان مقدمـ‌ـه‌ای بــرای پرخــوری محســوب شــوند و حتــی پرخــوری به‌عنــوان یــک مکانیســم تنظیــم هیجــان ناســازگارانه در افـراد دارای اضافـه‌ وزن یـا مبتلا بـه چاقـی مطـرح شـده اسـت. پژوهش حاضر با هدف بررسی اثربخشی طرحواره درمانی ذهنیت‌مداربر خوردن هیجانی و شاخص تودۀ بدنی زنان چاق دارای اختلال پرخوری انجام شد.
روش کار: پژوهش حاضر نیمه تجربی با طرح پیش آزمون-پس آزمون-گروه کنترل بود. جامعه آماری پژوهش شامل کلیۀ زنان چاق با اختلال پرخوری دارای پرخوری هیجانی مراجعه‌کننده به کلینیک‌های مشاوره منطقه 2 شــهر تهران در سال 1402 بود. از بین این افراد تعداد 30 نفر به روش نمونه‌گیری در دسـترس و با توجه به ملاکهای ورود و خروج انتخاب شدند. ابزار پژوهش شامل  مقیاس خوردن هیجانی آرنو و شاخص تودۀ بدنی بود. آزمودنی‌های گروه آزمایش طی 20 جلسۀ یک ساعته، تحت مداخلۀ طرحواره درمانی ذهنیت مدارقرار گرفتند. تجزیه و تحلیل داده‌ها با استفاده از نرم افزار SPSS-26 و آزمون تحلیل کوواریانس چند متغیره و آزمون تعقیبی بن‌فرونی با رعایت مفروضه های آماری انجام شد.
نتایج: نتایج حاصل از آزمون کوواریانس چند متغیره نشان داد که بین گروه طرحواره‌ درمانی ذهنیت‌مدار و کنترل در ترکیبی از متغیرهای خوردن هیجانی و شاخص تودۀ بدنی تفاوت معناداری وجود دارد (F=22/41 و P<0/05). همچنین نتایج آزمون تعقیبی بن‌فرونی نشان داد که میانگین گروه دریافت کننده برنامه طرحواره درمانی ذهنیت‌مدار هم در متغیر خوردن هیجانی و هم در متغیر شاخص تودۀ بدنی از میانگین گروه کنترل به‌طور معناداری کوچکتر است (P<0/05).
نتیجه‌گیری: با نظر به یافته‌های پژوهش، به‌منظور کمک به مشکل اضافه وزن و خوردن هیجانی زنان چاق دارای اختلال پرخوری، استفاده از طرحواره درمانی ذهنیت‌مدار می‌تواند مثمرثمر باشد.

Introduction: Emotional problems and it’s following stress can be a starter for binge eating and binge eating is considerd as a a maladaptive emotion regulation mechanism in overweight or obese people. The present study was conducted with the aim of investigating the effectiveness of schema therapy mode model on emotional eating and BMI of obese women with binge eating disorder.
Methods: The research method was semi-experimental with a pre-test-post-test design with a control group. The statistical population of the study included all the obese women with binge eating who referred to counseling clinics in Tehran’s district 2 in 1402. Among these people, 30 people were selected by available sampling and according to the entry and exit criteria; And they were randomly placed in the experimental group and the control group (15 people in each group). The participants completed the Emotional Eating Scale (EES) and the Body Mass Index (BMI) in the pre-test and post-test stages. The experimental group received the Psychotherapy sessions based on Schema therapy mode model in 20 sessions of 60 minutes. Data analysis was done using SPSS-24 software and multivariate analysis of covariance test in compliance with statistical assumptions.
Results: The results of the multivariate covariance test showed that there is a significant difference between the schema therapy mode model group and the control group in a combination of variables of emotional eating and body mass index (F=41.22 and P<0.5). Also, the results of Bonferoni's post hoc test showed that the average of the group receiving the schema therapy mode model program is significantly less than the average of the control group in both the variable of emotional eating and the variable of body mass index (P<0/05).
Conclusion: According to the results of the research, it can be said that the Schema therapy mode model can be an effective treatment for overweightness and emotional eating of obese women with binge eating disorder.
