بررسى رابطه اضافه وزن و چاقى با سن منارک و وضعیت تغذیه دختران 15-11 ساله شهر مشهد

چکیده

مقدمه
ارتباط بلوغ با چاقی در دختران موضوع مورد اختلافی است. هدف مطالعه حاضر بررسی ایـن موضـوع در دختـران
سنین بلوغ در مشهد میباشد.
روشکار
تعداد 1300 دختر دانش آموز سالم 15-11 ساله شهر مشهد در مطالعه ای توصـیفی توصـیفی مقطعـی بـه روش خوشـه اى دو
مرحلهاى از مدارس دولتى درسال 1386 بررسى شدند.اطلاعات دموگرافیک دموگرافیک و تنسنجى (قـد و وزن) و سـن بـروز
منارک (افراد بالغ )و دریافت غذایى با تکمیل یک یادآمد 24 سـاعته (216 نفـر) و پرسـشنامه پرسـشنامه عـادات غـذایى(230
نفر) به دست آمد. BMI بین صدک 85 و 95 بهعنوان اضافه وزن و BMI ≥ صدک 95 بهعنوان چاقى (بر اساس
صدکهاى BMI سن و جنس جمعیت مرجع WHO) درنظر گرفته شد. بـراى محاسـبات محاسـبات آمـارى از روش هـای
تحلیل بقا، تی مستقل،من مستقل،من ویتینی،کای ویتینی،کای دو، ضریب همبستگی همبستگی اسپیرمن اسپیرمن استفاده گردید.
نتایج
شیوع اضافه وزن وچاقی به ترتیب 11/5% و 10/3% بود. 63% افراد با میانگین سن منارک 13 ± 0/06 سال تجربه
منارک داشتند. BMI با بروز و سن منارک رابطه داشت( به ترتیب r= 0/33و p < 0/001 ، r= -0/2).
سن منارک در افراد داراى اضافه وزن و چاق 12/2 ± 0/07 سال و درافراد غیرچاق 13/1 ±0/07 سال بود
(p < 0/001 ). دختران داراى اضافه وزن و چاق نسبت به سایرین صبحانه کمتر و میان وعدههای بیشتر مصرف
کرده بودند.در این گروه بسامد مصرف ماکارونى، بستنى، شیرینىها ، مغزها، چیپس و نوشابههاى صنعتى بیشتر
بود.
نتیجهگیرى
چاقی در دختران با سن پایین ترمنارک در ارتباط است.الگوى کیفـی غـذای مـصرفی در ایـن افـراد در مقایـسه مقایـسه بـا
دختران دارای BMI طبیعی متفاوت است

چکیده [English]

Introduction
 
The relationship between obesity and puberty is still controversial. This study examined this issue in girls in pubertal age in Mashhad.
Materials and Methods
 
A total of1300 healthy girls aged 11-15, in Mashhad public guidance schools were recruited from all 7 urban educational regions using double-stages cluster sampling method. Demographic and anthropometric data were collected using standard methods. Data of dietary habits and intakes were collected using a valid questionnaire (230 subjects) and a 24-h recall (216 subjects) respectively. Age of menarche was also recorded for pubertal girls. BMI between 85-95 percentiles was defined as overweight and BMI≥95 percentile as obesity (based on BMI percentiles of WHO references for age and sex).Survival Analysis method was used.
Results
Prevalence of overweight and obesity were 11.5% and 10.3% respectively.63% of subjects had experienced menarche with mean menarche age of 13 ± 0.06 yrs. There was shown a significant correlation of BMI with menarche occurrence and menarche age (r=0.33, r=-0.2 respectively, P< 0.001].Menarche age was 13.1±0.07 yrs in normal group and 12.2±0.07 yrs in overweight and obese subjects (P< 0.001).Overweight and obese girls had consumed less frequently breakfast and more snacks than normal ones. Their frequency of intake for spaghetti, ice cream, sweets, nuts, chips and sweetened beverages was also significantly higher than that of normal girls.
Conclusion
 
In girls with higher BMI menarche are more likely occurred in lower ages and their dietary patterns are different compared to those with normal BMI.
