ایجاد تسکین طی آندوسکوپی دستگاه گوارش در کودکان مقایسه دو روش

چکیده

مقدمه
آندوسکوپی یک روش تهاجمی و دردناک در کودکان است. کودکان نمی توانند اضطراب و استرس را تحمل
کنند، لذا برای آندوسکوپی نیاز به استفاده از روش های کاهش اضطراب و استرس می باشد. هدف این مطالعه
مقایسه پروپوفل در مقابل میدازولامداخل وریدی جهت کاهش اضطراب در کودکان است.
روش کار
این مطالعه به صورت کار آزمایی بالینی تصادفی، بر 103 بیمار (14-2 سال) در مدت 9 ماه در سال 1385 در
بیمارستان قائم (عج) مشهد انجام شد. بیمارانی که نیاز به آندوسکوپی داشتند به سه گروه تقسیم شدند. در گروه
اول، بیماران پروپوفل و در گروه دوم میدازولام داخل وریدی دریافت کردند و در گروه سوم هیچ آرام بخشی
دریافت نکردند. در طی آندوسکوپی ضربان قلب، تعداد تنفس و اشباع اکسیژن قبل از آندوسکوپی و در طی آن با
فاصله 1 دقیقه ثبت شد، ترمور، تعریق و معیار اضطراب با استفاده از مقیاس آنالوگ بینایی ثبت شد. اطلاعات با
استفاده از نرم افزار SPSS تجزیه و تحلیل و از آزمون های آنوا و کای دو یا فیشربرای بررسی داده ها استفاده شد.
معیار جهت معنی دار بودن آماری p<0/05.
نتایج
افزایش در ضربان قلب به طور واضح در همه گروهها وجود داشت (p=0/038)، تعداد تنفس در همه گروهها
افزایش یافته بود. اشباع اکسیژن در گروه پروپوفل کاهش یافت اما مشخص نبود (p=0/017)، معیاراضطراب در
گروه پروپوفل به طور مشخص پایین تر از گروه میدازولام بود (20±22 در مقابل p<0/001 ،50±27)، هیچ عارضه
جانبی قابل توجه در گروه پروپوفل رؤیت نشد.
نتیجه گیری
به نظر می رسد داروی پروپوفل در کودکان یک داروی بسیار مفید و موثر است و تجویز آن سبب ایجاد تسکین
بی خطر، حین آندوسکوپی در کودکان می گردد.

Introduction
 
Endoscopy is an invasive and painful procedure in children. Since children can not tolerate pain and stress, sedation is necessary in pediatric endoscopy. The aim of this study was comparison of propofol versus intravenous midazolam for reducing anxiety in children.
 
Materials and Methods
 
In this study, 103 patients (2-14 years old) in whom endoscopy was indicated were divided into three groups. In the first group, patients received propofol, in the second group, intravenous midazolam was given and patients in the third group received no sedation. Procedures were performed in the endoscopy room. Heart rate, respiratory rate and oxygen saturation were recorded before and during endoscopy in 1 minute intervals. Tremor, sweating and pain scores were recorded by using Visual Analogue Scale (VAS).
 
Results
 
A significant increase in heart rate was documented in all groups (p=0.038). Respiratory rate also increased in all groups. Too Oxygen saturation decreased in the propofol group, but it was not statistically significant (p=0.17). Pain score in propofol group was significantly lower than those receiving midazolam (20±22 vs 50±27, p<0.001). No noticeable side effect was seen in the propofol group.
 
Conclusion
 
It seems that propofol is a very useful and effective drug for reducing pain and stress in children and its administration leads to a safe sedation in pediatric endoscopy.
