ضرورت انجام سی تی اسکن مغز قبل از بزل مایع نخاع در بیماران بیشتر از 60 سال مشکوک به مننژیت

چکیده

مقدمه: عفونت های سیستم اعصاب مرکزی به ویژه مننژیت جزء اورژانسهای پزشکی بوده که باید در صورت شک با بزل مایع نخاع و تجزیه مایع مغزی
نخاعی اثبات گردد. از طرفی به دلیل احتمال بروز عوارض تهدیدکننده حیات از قبیل فتق مغزی به خصوص در سن بالاتر از60 سال انجام آن بدون سی تی اسکن
قبلی مورد بحت است. این مطالعه با هدف بررسی ضرورت انجام سی تی اسکن قبل از بزل مایع نخاع در بیماران بالای60 سال مشکوک به مننزیت انجام شد.
روش کار: این مطالعه توصیفی در سال 1382 الی 84 در بیماران مشکوک به مننزیت مراجعه کننده به درمانگاه بیماریهای عفونی بیمارستان امام رضا (ع)
مشهد انجام شد.90 بیمار مشکوک به مننزیت که سیتی اسکن و بزل مایع نخاع داشتند مورد مطالعه قرار گرفتند. از کلیه بیماران شرح حال و معاینه فیزیکی به عمل
آمد. نتایج شرح حال معاینات فیزیکی مشخصات فردی نتایج سیتی اسکن و بزل مایع نخاع در پرسشنامه جمع آوری شد. اطلاعات بدست آمده با استفاده از آمار
توصیفی و جداول توزیع فراوانی و نرم افزارssps و آزمونهای هبستگی یکطرفه واسپیرمن پردازش شد.
نتایج: در90 بیمار مشکوک به مننژیت که تحت انجام سی تی اسکن و بزل مایع نخاع قرار گرفتند، 81 نفر اختلال هوشیاری، 2 نفر سیر مزمن و 7 نفر بدون
معیار بالینی ممنوعیت بزل مایع نخاع بودند. تعداد 75 نفر سی تی اسکن طبیعی و 15 نفر سی تی اسکن غیرطبیعی داشتند، که در هیچ کدام ضایعه فضاگیر با
اثرفشاری یا انحراف خط وسط وجود نداشت. تشخیص نهایی در 14 نفر مننژیت حاد باکتریال و7 نفر مننژیت آسپتیک و 7 نفر مننژیت با علل دیگر و در 1 نفر
آنسفالیت و در 61 نفر سایر موارد به جز عفونتهای سیستم اعصاب مرکزی بوده است.
نتیجهگیری: با توجه به این که علیرغم وجود ممنوعیتهای بالینی دیگر دراکثریت بیماران ، در75 نفر (83 % موارد) سی تی اسکن مغز طبیعی بوده و فقط در
15 نفر(17% افراد) سی تی اسکن غیر طبیعی وجود داشت که هیچ کدام آنها نیز ممنوعیت بزل مایع نخاع مثل توده فضاگیر با اثر فشاری یا شیفت خط وسط
نبودند لذا نقش «سن بالای60 سال» به عنوان یک ملاک قطعی برای انجام سی تی اسکن، با توجه به امکانات و شرایط موجود در کشور ما قابل قبول به نظر
نمی رسد.

Brain Computed Tomography Scan Nessecity before Lumbar Puncture in Meningitis Patients with More than 60 Years Age

Introduction: Central Nervous System (CNS) infections especially meningitis is a medical emergency that once suspected should be proved through Lumbar Puncture (LP) and cerebrospinal Fluid (CSF) analysis. On the other hand due to life – threatening complications such as brain herniation in the people older than 60, the procedure safety is debated. This study was done in order to determine the necessity of the Lumbar Puncture (LP) in the patients older than 60 years old who are suspected to meningitis.
Material and Methodes: This descriptive study was done in Infectious Clinic of Imam Reza Hospital, Mashad, Iran from fall 2003 to fall 2005. 90 patients who were suspected to meningitis and undergone brain CT scanning and LP were studied. The results of patient's history, physical examination, demographic information, CT scanning and LP all collected in a questionnaire and analyzed by spss and statistics examinations.
Results: Among 90 patients suspected of meningitis who underwent Brain CT-scan and LP, 81 suffered from loss of consciousness, 2 were chronic course and 7 without clinical LP contraindication other than age. 75 had normal CT-scan and 15 had abnormal CT-scan, amongwhich was no space occupying lesion with mass effect or midline shift. Final diagnosis was acute bacterial meningitis in 14 cases and aseptic meningitis in 7 cases and other causes of meningitis in 7 cases, Encephalitis in 1, and 61 cases other CNS infection.
Conclusion: Despite other clinical contraindications for LP in most of the patients, in 75 cases
(83 %) brain CT-scan was normal and in only 15 cases (17%) there was abnormal CT-scan amongwhich none was a contraindication for LP (i.e space occupying lesion with mass effect or midline shift). Therefore the role of «age over sixty» as an absolute indication for Brain CT-scan, regarding the existing medical facilities and conditions in our country, did not seem acceptable.
